  • Permit Name/Ownership Change Form

  • Buffer Authorization

    The buffer authorization form should be used to seek approval for activities listed as allowable or allowable with mitigation in the table of uses applicable to each protected buffer when the proposed activities do not impact streams, wetlands, or open water. If stream, wetland, or open water impacts are proposed along with activities within the buffer, the Pre-Construction Notification (PCN) Application should be used.

  • Major Variance

    The variance application form is required for applicants seeking approval for activities within North Carolina’s protected riparian buffers that are listed as prohibited or are not listed in the table of uses of the buffer rule applicable to the property. For projects within the Neuse and Tar-Pamlico River Basins and the Randleman Lake and Jordan Lake watersheds, a major variance is required for prohibited activities that will impact any portion of Zone 1 of the riparian buffer. For projects within the Goose Creek watershed, a major variance is required for prohibited activities that impact any portion of the riparian buffer.

    The variance application form is required for applicants seeking approval for activities within North Carolina’s protected riparian buffers that are listed as prohibited or are not listed in the table of uses of the buffer rule applicable to the property.   • Pre-Construction Notification (PCN) Application

    The Pre-Construction Notification (PCN) should be used for applicants seeking 401 Water Quality Certification approval for federally permitted or licensed activities that involve a discharge to jurisdictional waters, buffer authorization approval for projects that also impact jurisdictional waters, or an Isolated and Other Non-404 Jurisdictional Wetlands and Waters Permit for projects that involve impact to waters the U.S. Army Corps of Engineers determines are not jurisdictional under Section 404 of the Clean Water Act.

  • Stream Origin/Buffer Applicability Determination

    The Stream Origin/Buffer Applicability Determination form should be used when a landowner or other affected party believes that the maps applicable to the relevant buffer rule have inaccurately depicted surface water on a property of interest. The form may be used to request an on-site determination by the Division.

  • Shoreline Stabilization

    The Shoreline Stabilization form should be used to apply for a 401 Water Quality Certification for activities eligible for coverage under U.S. Army Corps of Engineers Regional General Permits 30, 79, or 80. This includes, but is not limited to, shoreline stabilization on Federal Energy Regulatory Commission (FERC) regulated lakes, Tennessee Valley Authority (TVA) regulated lakes, and certain private lakes. This form does not apply to property within the twenty coastal counties that are affected by the Division of Coastal Management (CAMA) rules.

Groundwater and Wells Forms

  • DWR has developed an on-line portal to submit the well construction form (“GW-1”) required by 15A NCAC 02C ,0114(b)(1).  Well construction forms submitted through this on-line system will also be immediately available to the local county Health Departments.

    DWR will continue to accept paper copies of the GW-1 form, however the use of the paper form will eventually be terminated in order to improve accuracy and decrease costs.  A link to the current paper copy of the GW-1 form is available here: PDF copy of GW-1 Well Construction Form.  Please note that if you use the paper copy of the GW-1 form, you must submit the completed form to the addresses shown at the bottom of the form and provide a copy to the local county Health Department.

  • EDMR Supplemental Material

    The Division of Water Resources (DWR) is providing this site to allow for the upload of required supplemental information associated with discharge monitoring reports (DMR).  All DMR data is required to be submitted electronically through the eDMR system and should not be submitted here.  Division staff will review all uploaded documents and will only maintain required information.  Documents such as laboratory reports are not required to be submitted and will not be maintained in the system.
