The first sample period of your permit begins August 1, 2017. Tier 3 actions in Part IV of your permit are triggered by benchmark exceedances on four occasions beginning on the effective date of this permit and do not count prior exceedances. Likewise, Tier 2 actions are triggered by two consecutive benchmark exceedances beginning on the effective date of this permit and do not count prior exceedances. Howeverif your facility is already in Tier 2 (monitoring stormwater discharges monthly) or Tier 3, you must continue monthly monitoring until relieved through conditions of the permit or by DEMLR staff approval. When does electronic DMR reporting start? We are setting up our database with final permit parameters and the outfalls that permittees submitted on-line. All NCG14 Permittees will receive notification when our eDMR system is ready for these permittees to register and begin reporting monitoring data electronically. In Hamlet, Airport Road is located 0.6 mile north on HWY 177 from the intersection of HWY 177 and HWY 74 (Exit 316). Concrete Supply is 0.5 nines West on Airport Rd. Facility is on the right. The door to the office is up the stairs and around the walkway. 2) Safety Considerations Standard DAQ safety equipment. Watch for trucks entering and exiting facility. 3) Facility and Process Description Concrete Supply - Hamlet is a truck mix concrete batch plant. The concrete batch plant consists of a cement silo, a fly ash silo, aggregate storage and handling system, an aggregate weigh -hopper, an aggregate mixing conveyor, a fly ash/cement weigh -hopper, and a truck load -out operation. The silos, fly ash/cement weigh -hopper and truck loadout are all controlled by a central bagfilter. Fine (sand) and coarse (gravel) aggregates are front-end loaded into a pit, belt conveyed to one of three storage bins, the sand and aggregate weigh hopper weighs out material, and it's transported by belt conveyor to the truck mix loadout. Cement and fly ash are stored in separate silos, gravity fed into the fly ash/cement weigh hopper, and added to the truck mix. Water is added throughout this entire process. A shroud curtain surrounds this area. This facility has an insignificant boiler/water heater, which has no federal or state requirements. Facility is permitted under Air Permit No. 08064R12, effective from 2 July 2014 until 30 June 2022. Mike Thomas conducted the last compliance inspection on 11 June 2015. We met with Johnie Alexander, Environmental Manager, and Bubba Metts, Area Manager, Mr. Alexander reviewed the FACFINDER and relayed that no changes were needed. Mr. Metts provided the facility's bagfilter I(M logbook for review. The facility performs and documents monthly inspections (Filter Kleen Environmental Service performs all necessary maintenance). An annual maintenance check was documented for 16 March 2016 and 9 April 2015, There are weekly maintenance entries noted within the logbook, documenting inspections of the baghouse, ductwork, cement hose, fly ash hose, dust shroud, and line pressure. Mr. Metts relayed that the facility has not received any complaints from the surrounding area and that there have been no excess emissions. A discussion was held about the recent permit exemption rules. The facility was not operating during the inspection. I verified that the equipment listed on the permit matched what is at the facility. Silos are filled on an as needed basis, which varies from week to week. The haul roads and area around the plant are a mix of gravel'and concrete. The potential for dusting during dry periods is high. The facility addresses this issue by using a water truck to wet the roads. 6) Pernut Stipulations a. A.2 2D .0304 and .0202 PERMIT RENEWAL AND EMISSION INVENTORY REQUIREMENT —The permittee shall submit an application for permit renewal no later than 90 days prior to expiration, including an emission inventory report for calendar year 2021. APPEARED IN COMPLIANCE — Last permit renewal application and emission inventory were due on 2 August 2014 and were received on 9 June 2014. The next permit renewal and emission inventory will be due in April 2022. b. A.3 2D .0515 PARTICULATE CONTROL REQUIREMENT — Particulate matter emissions from the emission sources shall not exceed allowable emission rates. APPEARED IN COMPLIANCE — Latest permit review indicates compliance when the facility is operated as permitted. There have been no changes to the facility since the last permit review. c. A.4 2D .0521 VISIBLE EMISSIONS CONTROL REQUIREMENT -- Visible emissions shall not be more than 20%.in opacity. APPEARED IN COMPLIANCE — This facility was not operating during the inspection. Facility personnel stated that there have been no complaints and there were no indications that the facility would exceed the opacity limit. d. A.5 2D .0S3S NOTIFICATION REQUIREMENT — Facility is required to notify the DAQ of excess emissions that last for more than four hours. APPERARED IN COMPLIANCE — This requirement was discussed and Mr. Alexander seemed to have a thorough understanding of the requirement. Mr. Metts stated there have been no periods of excess enussions. e. A.6 2D .0540 FUGITIVE DUST CONTROL REQUIREMENT— The Permittee shall not cause or allow fugitive dust emissions to cause or contribute to substantive complaints or excess visible emissions beyond the property boundary. APPEARED IN COMPLIANCE — Since the facility was not operating, I observed no excess visible emissions beyond the property boundary. Mr. Metts reported that there have been no complaints. DAQ has not received any complaints regarding this facility either. f. A.7 2D .0611 FABRIC FILTER REQUIREMENTS — Particulate matter emissions shall be controlled as described in the permitted equipment list; an annual internal inspection, as well as periodic inspections and maintenance shall be conducted. APPEARED IN COMPLIANCE — Maintenance'is completed monthly on the bagfilter and the last annual inspection was completed on 16 March 2016. 7) 112R Status This facility neither uses nor stores chemical compounds that require a written risk management plan under the Clean Air Act, Section 112R. 8) Non-compliance History Since 2010 There are no 'records of non-compliance. 9) Comments and Compliance Statement Concrete Supply - Hamlet appeared to be in compliance on 24 June 2016. We met with Johnie Alexander, Environmental Manager, and Bubba Metts, Area Manager. Mr. Alexander reviewed the FACFINDER and relayed that no changes were needed. Mr. Metts provided the facility's bag€ilter UM logbook for review. The facility performs and documents monthly inspections (Filter Kleen Environmental Service performs all necessary maintenance). An annual maintenance check was documented for 16 March 2016 and 9 April 2015. There are weekly maintenance entries noted within the logbook, documenting inspections of the baghouse, ductwork, cement hose, fly ash hose, dust shroud, and line pressure. Mr. Metts relayed that the facility has not received any complaints from the surrounding area and that there have been no excess emissions. A discussion was held about the recent permit exemption rules. The facility was not operating during the inspection. I verified that the equipment listed on the permit matched what is at the facility. Silos are filled on an as needed basis, which varies from week to week. The haul roads and area around the plant are a mix of gravel'and concrete. The potential for dusting during dry periods is high. The facility addresses this issue by using a water truck to wet the roads. 6) Permit Stipulations a. A.2 2D .0304 and .0202 PERMIT RENEWAL AND EMISSION INVENTORY REQUIREMENT — The permittee shall submit an application for permit renewal no later than 90 days prior to expiration, including an emission inventory report for calendar year 2021. APPEARED IN COMPLIANCE -- Last permit renewal application and emission inventory were due on 2 August 2014 and were received on 9 June 2014. The next permit renewal and emission inventory will be due in April 2022. b. A.3 2D .0515 PARTICULATE CONTROL REQUIREMENT— Particulate matter emissions from the emission sources shall not exceed allowable emission rates. APPEARED IN COMPLIANCE — Latest permit review indicates compliance when the facility is operated as permitted. There have been no changes to the facility since the last permit review. c. A.4 2D .0521 VISIBLE EMISSIONS CONTROL REQUIREMENT— Visible emissions shall not be more than 20%.in opacity. APPEARED IN COMPLIANCE -- This facility was not operating during the inspection. Facility personnel stated that there have been no complaints and there were no indications that the facility would exceed the opacity limit. d. A.5 2D .0535 NOTIFICATION REQUIREMENT —Facility is required to notify the DAQ of excess emissions that last for more than four hours. APPERARED IN COMPLIANCE — This requirement was discussed and Mr. Alexander seemed to have a thorough understanding of the requirement. Mr. Kent and I discussed the following: a) Verified the FACFINDER information. No changes are needed. b) Truck mix batch plant #2 (portable plant) which contains sources ES-1 through ES-6 was never brought to the facility and set up. Plant #2 remains on the permit in case the facility decides to bring the plant in. c) There have been no changes at the facility since the last compliance inspection. d) The facility performs semiannual inspections (Filter Kleen Environmental Service actually comes in and performs the maintenance). There are weekly maintenance entries noted within the logbook, documenting inspections of the baghouse, ductwork, cement hose, fly ash hose, dust shroud, and line pressure. Patrick Smith (Assistant Plant Manager) makes weekly entries. They tell all the drivers to keep the pressure b/t 1045 psi. They also record the pressure drop readings, which are normally 3.5 — 4.0. 4) Inspection Summary The facility was operating during the inspection. I verified that the equipment listed on the permit matched what is at the facility. The capacity of the cement silo is 82 tons and the capacity of the fly ash silo is 50 tons. Silos are filled on a demand basis that varies from week to week. The facility tries to maintain at least 62 tons in the cement silo. The haul roads and area around the plant are a mix of gravel and concrete. The potential for dusting during dry periods is high. The facility addresses this issue by using a water truck to wet the roads. NOTIFICATION REQUIREMENT - As required by 15A NCAC 2D .0535, the Permittee of a source of excess emissions that last for more than four hours and that results from a malfunction, a breakdown of process or control equipment or any other abnormal conditions, shall: a. Notify the Director or his designee of any such occurrence by 9:00 a.m. Eastern time of the Division's next business day of becoming aware of the occurrence and describe: the name and location of the facility, ii. the nature and cause of the malfunction or breakdown, iii. the time when the malfunction or breakdown is first observed, iv. the expected duration, and V. an estimated rate of emissions. b. Notify the Director or his designee immediately when the corrective measures have been accomplished. This reporting requirement does not allow the operation of the facility in excess of Environmental Management Commission Regulations. 6. FUGITIVE DUST CONTROL REQUIREMENT - As required by 15A NCAC 2D .0540 "Particulates from Fugitive Dust Emission Sources," the Permittee shall not cause or allow fugitive dust emissions to cause or contribute to substantive complaints or excess visible emissions beyond the property boundary. If substantive complaints or excessive fugitive dust emissions from the facility are observed beyond the property boundaries for six minutes in any one hour (using Reference Method 22 in 40 CFR, Appendix A), the owner or operator may be required to submit a fugitive dust plan as described in 2D .0540(f). "Fugitive dust emissions" means particulate matter that does not pass through a process stack or vent and that is generated within plant property boundaries from activities such as: unloading and loading areas, process areas stockpiles, stock pile working, plant parking lots, and plant roads (including access roads and haul roads). FABRIC FILTER REQUIREMENTS including artridge filters, baahouses, and other dry filter particulate collection devices - As required by 15A NCAC 2D .0611, particulate matter emissions shall be controlled as described in the permitted equipment list. a. Inspection and Maintenance Requirements - To comply with the provisions of this permit and ensure that emissions do not exceed the regulatory limits, the Permittee shall perform, at a minimum, an annual (for each 12 month period following the initial inspection) internal inspection of each bagfilter system. In addition, the Permittee shall perform periodic inspections and maintenance as recommended by the equipment manufacturer. b. RecordkeepinQ Requirements - The results of all inspections and any variance from manufacturer's recommendations or from those given in this permit (when applicable) shall be investigated with corrections made and dates of actions recorded in a logbook. Records of all maintenance activities shall be recorded in the logbook. The logbook (in written or electronic format) shall be kept on -site and made available to DAQ personnel upon request. These records must be kept on site for a minimum of 2 years, unless another time period is otherwise specified. 3. ANNUAL FEE PAYMENT - Pursuant to 15A NCAC 2Q .0203(a), the Permittee shall pay the annual permit fee within 30 days of being billed by the DAQ. Failure to pay the fee in a timely manner will cause the DAQ to initiate action to revoke the permit. 4. EQUIPMENT RELOCATION - A new air permit shall be obtained by the Permittee prior to establishing, building, erecting, using, or operating the emission sources or air cleaning equipment at a site or location not specified in this permit. 5. REPORTING REQUIREMENT - Any of the following that would result in previously unpermitted, new, or increased emissions must be reported to the Regional Supervisor, DAQ: a. changes in the information submitted in the application regarding facility emissions; b. changes that modify equipment or processes of existing permitted facilities; or c. changes in the quantity or quality of materials processed. If appropriate, modifications to the permit may then be made by the DAQ to reflect any necessary changes in the permit conditions. In no case are any new or increased emissions allowed that will cause a violation of the emission limitations specified herein. Permit No. 08064R 12 Page 6 6. This permit is subject to revocation or modification by the DAQ upon a determination that information contained in the application or presented in the support thereof is incorrect, conditions under which this permit was granted have changed, or violations of conditions contained in this permit have occurred. The facility shall be properly operated and maintained at all times in a manner that will effect an overall reduction in air pollution. Unless otherwise specified by this permit, no emission source may be operated without the concurrent operation of its associated air cleaning device(s) and appurtenances. 7. This permit is nontransferable by the Permittee. Future owners and operators must obtain a new air permit from the DAQ. 8. This issuance of this permit in no way absolves the Permittee of liability for any potential civil penalties which may be assessed for violations of State law which have occurred prior to the effective date of this permit. 9. This permit does not relieve the Permittee of the responsibility of complying with all applicable requirements of any Federal, State, or Local water quality or land quality control authority. 10. Reports on the operation and maintenance of the facility shall be submitted by the Permittee to the Regional Supervisor, DAQ at such intervals and in such form and detail as may be required by the DAQ. Information required in such reports may include, but is not limited to, process weight rates, firing rates, hours of operation, and preventive maintenance schedules. 11. A violation of any term or condition of this permit shall subject the Permittee to enforcement pursuant to G.S. 143-215.114A, t43-215.11413, and 143-215.114C, including assessment of civil and/or criminal penalties. 12. Pursuant to North Carolina General Statute 143-215.3(a)(2), no person shall. refuse entry or access to any authorized representative of the DAQ who requests entry or access for purposes of inspection, and who presents appropriate credentials, nor shall any person obstruct, hamper, or interfere with any such representative while in the process of carrying out his official duties. Refusal of entry or access may constitute grounds for permit revocation and assessment of civil penalties. 13. The Permittee must comply with any applicable Federal, State, or Local requirements governing the handling, disposal, or incineration of hazardous, solid, or medical wastes, including the Resource Conservation and Recovery Act (RCRA) administered by the Division of Waste Management. 14. PERMIT RETENTION REQUIREMENT - The Permittee shall retain a current copy of the air permit at the site. The more significant changes in the General Permit since your last Certificate of Coverage include the following (please note the names and numbers of sections have been changed from the previous permit revision): Part II: • Authorization to Construct (ATC) language was added to the permit and adapted to fit NCG140000. Part III: • The Stormwater Pollution Prevention Plan (SPPP) section has been updated to the most current language of our permits. Some additional conditions specific to this industry have been added to the SPPP language. Part IV: • Sections A & B: Revised provisions require stormwater discharges be sampled twice per year (instead of annually), and sampling results shall be compared to new benchmark values. (The previous cut-off concentrations have been removed). • Sections A & B: Revised provisions require the permittee to execute Tier One, Two & Three response actions: Tier One requirements for a first benchmark exceedence, and/or Tier Two response for a second consecutive benchmark exceedence, if applicable. Tier Two requires that the permittee institute monthly monitoring (reported to the appropriate Regional Offices) instead of twice -per -year monitoring. Monthly monitoring shall be done until three consecutive monitoring events show no benchmark exceedences. • Sections A & B: This permit now contains a provision that four exceedences of any benchmark will trigger increased DWQ involvement in the permittee's stormwater management and control actions (Tier Three). DWQ may direct the,permittee to apply for an individual permit, institute specific moritoring, or may direct the implementation or installation of specific stormwater control measures. • Sections A & B: Failure to perform analytical stormwater monitoring requires that the permittee begin a monthly sampling scheme for both industrial stormwater discharges and/or stormwater discharges from VMA areas (where applicable). After 6 months of monthly sampling, the permittee may return to semi- annual sampling, unless otherwise specified by DWQ. • Sections A & B: A lower TSS benchmark of 50 mg/I for HQW, ORW, PNA and Tr Waters was added to this permit for these more sensitive waters. • Section B: The monitoring parameter Oil & Grease has been replaced by the parameter Total Petroleum Hydrocarbons. • Sections A, B & C: Inability to sample due to adverse weather must be reported in the SPPP. • Sections A, B & C: The terminology "Representative Storm Event" has been replaced by "Measurable Storm Event" in response to comments from industry groups. A measurable storm event is a storm event that results in an actual discharge from the permitted site outfall. The previous measurable storm event must have been at least 72 hours prior, unless the permittee is able to document that a shorter interval is representative for local storm events during the sampling period, and obtains written approval from the local DWQ Regional Office. • Section C: Qualitative monitoring must now be conducted semi-annually and is tied to the same measurable storm event as the analytical monitoring. • Section C: If the permittee fails to respond effectively to problems identified by qualitative monitoring, the permittee may be required by DWQ to perform corrective action. • Section D: This section has now been consolidated to one section with one set of combined tables for all types of authorized wastewater discharges. • Section D: The wastewater analytical monitoring schedule has been set to quarterly. • Section D: TSS limits have been revised for PNA, Trout and HQW waters per rule requirements in 15A NCAC 02B .0224. • Section D: 7010-based flow limits have been set for PNA waters per regulations in 15A NCAC 02B. • Section D: TPH monitoring requirements have been added to wastewater monitoring when stormwater runoff from VMA areas commingles with wastewater. • Section D: This wording in the permit has been removed: "For facilities which route stormwater and all process wastewater to a recycle system, no analytical monitoring of process wastewater discharges is required if that recycle system discharges as a result of a 10-year 24-hour (or larger) precipitation event." Part V: • Section A: For existing facilities previously permitted and applying for renewal under this General Permit: New elements of the Stormwater Pollution Prevention Plan for this permit renewal shall be developed and implemented within 6 months of the effective date of the initial Certificate of Coverage issued pursuant to this General Permit and updated thereafter on an annual basis. Your coverage under the General Permit is transferable only through the specific action of DWQ. This permit does not affect the legal requirements to obtain other permits which may be required by DENR, nor does it relieve the permittee from responsibility for compliance with any other applicable federal, state, or local law, rule, standard, ordinance, order, judgment, or decree.
