Tier 3 actions in Part IV of your permit are triggered by benchmark exceedances on four occasions beginning on the effective date of this permit and do not count prior exceedances. likewise, Tier 2 actions are triggered by two consecutive benchmark exceedances beginning on the effective date of this permit and do not count prior exceedances. However, if your facility is already in Tier 2 (monitoring stormwater discharges monthly) or Tier 3, you must continue monthly monitoring until relieved through conditions of the permit or by DEMLR staff approval. When does electronic DMR reporting start? We are setting up our database with final permit parameters and the outfalls that permittees submitted on-line. All NCG14 Permittees will receive notification when our eDMR system is ready for these permittees to register and begin reporting monitoring data electronically. This facility is currently owned and operated by Argos Ready Mix. A Permit Name/Ownership Change Form for this facility or letter from the current owner updating ownership of all facilities in the state needs to be submitted to staff with the Stormwater Permitting Program at the Central Office in Raleigh in order to update our files. The Stormwater Pollution Prevention Plan was reviewed and contained the required components as specified in the permit. No spills have taken place at the facility since the previous inspection of the facility in October 2012. There is one outfall on the site. Since both stormwater and processed wastewater would be discharged from this outfall when the facility is operational, the outfall is monitored on a quarterly basis. The monitoring records indicated no flow/no discharge for the monitoring periods since the previous inspection of the facility. The facility has one stormwater-only outfall, therefore the annual non-stormwater discharge certification is not applicable. A tour was conducted of the facility's grounds where the outfall, secondary containment area and concrete washout pit was observed. The outfall was at a low point on the property and received sheet flow. There was no visual evidencs of recent discharge through the outfall. The facility appeared well -maintained and in compliance with the conditions of General Permit NCG140000. These and other findings are listed below: -The site was found to be neat and well maintained, - Located on the site is a second ready mixed plant # 121. The second plant appears to generates wastewater and stormwater in the same manner as plant # 118. Ready Mixed Concrete Company is asked to make sure your Stormwater Pollution Prevention Plan and other required components of your permit reflect the presence of the plant # 121. Further, you are also asked to make sure that onsite BMPs are designed and operated in manner to treat the additional loading. - Considerable time was taken going over the permit. When discussing analytical and qualitative monitoring and during the site tour it became apparent that site personnel have been sampling a point that is NOT representative of the actual discharge from the site should a discharge occur. The stormwater sampling point was noted to be a point in which stormwater would pool near the rear of the property. Based on discussions with you and Mr. Underwood and based on a rather lengthily tour of the property it is unclear if the site ever actually discharges. Noted during the tour was a berm of soil and a old silt fence at the rear of the property. Also observed was a depression or old ditch that had no point of discharge. Per our discussion you should not feel compelled to sample any point that is not a true Stormwater Discharge Outfall (SDO). You should never sample anything that is not a true discharge and representative of the stormwater/process wastewater leaving the site. In keeping with our discussion it is understood that you are going to clear (mow) the rear of the property in order to properly determine the stormwater flow path(s) and where the true SDO point(s) are located. Also discussed was the possibility of determining site contours or elevations to determine the "low spot". In the event that you do not feel the site routinely or ever discharges you are asked to establish and clearly mark the point a discharge would occur. You are asked to contact the writer within 30 days of completion of the noted work to schedule a second site visit. This second visit will be, at a minimum, for the verification of SDOs should they exist. Discharge Monitoring Reports. As noted above please do not feel compelled to sample if your site(s) do not discharge. Please refer to Section E: Reporting Requirements, item 1 of your permit. The permit states, in part "When no discharge has occurred from the facility during the report period, the permittee is required to submit a discharge monitoring report, within 30 days of the end of the sampling period, giving all required information and indicating "NO FLOW" as per 15A NCAC 028 .0506." please in the future submit the required discharge monitoring report with "NO FLOW " indicated if called for. The follow-up visit was to observe site clearing (mowing) that had been performed at the rear of the property to determine the exact location of the stormwater discharge outfall (SDO). Based on your comments and the site visit it was determined that there is only one SDO for the site. Further, it is believed that the site will only discharge during extreme weather events and/or during periods of extended wet weather. You are urged to keep the vegetation at this location managed in such a manner that observations of the SDO can be performed easily and safely. Also please take samples of stormwater discharges only as outlined in your permit and submit the data as required. Fly ash used as a specialty aggregate. The plant stores, conveys, measures and discharge these constituents into a truck for mixing and transport to the job site. A dump pit is used to transfer sand and rock to one of three storage silos via an open belt system. The sand and rock is then transferred to the three -bin hopper via an open belt system. Sand and aggregate is then weighted for use by the weigh -hopper under the three -bin hopper. The material is then transferred to the mixing hopper via a conveyor belt and is mixed with water, cement and sometimes fly ash. Mr. Honecker said that he is the plant manager for the facility located in Spring Lake and that he was at this plant only to assist the new plant manager, Ronnie Butner. He also said that Mr. Dennis Underwood was moved to the plant in Laurinburg. I explained the permit conditions and what is expected during the compliance inspection. Mr. Honecker provided the I/M logbook that contained the air permit and the bagfilter maintenance records along with production records. It seemed that Mr. Butner was not as knowledgeable as Mr. Honecker concerning the permit and records required by the permit. Mr. Honecker indicated that he would be working with Mr. Butner to get him up to speed with the air permit requirements. Mr. Honecker reviewed the FACFINDER and indicated that all was correct with exception to Mr. Underwood replaced with Mr. Butner. My review of the log indicated an annual inspection occurred on 8 July 2015 by the contractors Filter Kleen Environmental Service for both plants. The log also indicated daily general inspections are done by Ronnie Butner. Mr. Butner stated that Plant #2 was presently not needed and Plant # 1 will produced about 9,500 cubic yards for cy 2015. 8) Walk Through: Mr. Honecker and I went outside to walk the surrounding grounds. A truck was loaded with concrete to be delivered and no dust appeared. The plant appeared to be well maintained and operated. According to records, this facility produces a relatively small amount of concrete annually, around 9500 cubic yard. This request must be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings Post Office Drawer 27447, Raleigh, North Carolina 27611-7447. Unless such demand is made, this decision shall be final and binding. Please take notice that this certificate of coverage is not transferable. Part H, B.2, addresses the requirements to be followed in case of change in ownership or control of this discharge.
