(S+G) is pleased to submit this first semiannual Discharge Monitoring Report (DMR1 for stormwater monitoring at the Brownfield Road C&D Landfill. The site is located in Raleigh, North Carolina. The facility is permitted to discharge stormwater related to landfitling activities under the NPDES program by the General Permit No. 120000 (permit), issued by the North Carolina Department of Environmental Quality (NCDEQI, Division of Water Quality. Please note that samples were analyzed from two (21 of three (31 outfalts during this event. Samples were collected at SDO-001 (Pond) and SDO-002. Both samples exhibited results below all benchmark thresholds. No discharge occurred at SDO-003. As required by the permit, analytical and qualitative monitoring of the discharge will continue on a semi-annual basis. (S+G) is pleased to submit this second semiannual Discharge Monitoring Report (DMR1 for stormwater monitoring at the Brownfield Road C&D Landfill. The site is located in Raleigh, North Carolina. The facility is permitted to discharge stormwater related to landfilling activities under the NPDES program by the General Permit No. 120000 (permitl, issued by the North Carolina Department of Environmental Quality (NCDEQI, Division of Water Quality. Please note that samples were analyzed from one (1) of three (3) outfalls during this event. Samples were collected from Outfall 003 on November 511; however, there was no stormwater discharge at Outfalls 001 and 002 during this event. S+G personnel returned to the site on November 1511 to collect samples from Outfalls 001 and 002. The samples were submitted that day to Pace Analytical Laboratory; however, they were not logged by the lab for the analyses listed in the NPDES Permit which invalidated the samples. Although rain was realized at the site during the remained of the reporting period, no rain event was substantial enough to generate discharge at the outfalls. A tetter from Pace Analytical documenting the event is attached. As required by the permit, analytical and qualitative monitoring of the discharge will continue on a semi-annuat basis. Root Cause: The samples for the Brownfield B-1 project were received by the Pace Analytical Services - Raleigh on 11/15/18 at 13:10. The samples were logged into the L1MS system for Total Settleable Solids and sent to the Pace Analytical Services — Eden location for analysis. The Project Manager reviewed the log -in of the project on 11/16/18, but the analysis had already started on the samples and no sample volume remained to perform the Total Suspended Solids analysis. Corrective Action Plan: Pace Analytical Services, LLC has retrained the Project Coordinators to carefully follow the chain -of -custody for logging -in samples. The laboratory has worked with the Project Manager to reduce the time to review the log -in of projects. Additionally the laboratory analysts have been retrained to inspect and compare client sample labels to laboratory labels before conducting analysis. Monitoring and Follow-up: The Quality department will follow up in 60-90 days to ensure all that the correct processes are being followed by the laboratory staff. We apologize for any inconvenience this has caused you. Please be assured that Pace Analytical Services, LLC is committed of meeting the business needs of our customers with integrity and high quality analytical results.
