For a name change request, the signed Permittee's certification is sufficient. This completed application is required for both name change andlor ownership change requests. Legal documentation of transfer of ownership (such as relevant pages of a contract deed, or a bill of sale) is required for an ownership change request. Articles of incorporation are not sufficient for an ownership change. During this time we will be able to address any comments or concerns you have about your permit draft. • During the same 30-day period, the draft will be sent to a Raleigh Regional Office staff member for review and the permit draft will be sent for Public Notice. Once all comments and concerns are addressed, you will be issued the final permit. A cover letter submitted with the application states the primary function of the facility is the transfill and ultrasonic testing of compressed gas cylinders: "The primary materials at the facility are gasses. Liquid materials onsite which may be exposed to stormwater include caustic (25% solution) which is stored outside in 55-gallon drums on spill containment pallets. Solid materials stored at the facility which are exposed to stormwater include scrap metals and compressed gas cylinders. Scrap metals are stored in open dumpsters which are located in the employee parking area and in the area located behind Building 184. Compressed gas cylinders are stored throughout the facility. Small amounts of hazardous waste are also produced by the facility through various processes, however; once it is produced it is collected by the facility's dust collector. The dust collector is located outdoors and in ground -mounted. Dust is collected in 55-gallon drums which are placed on spill containment pallets. Waste contained within the drums have the potential to contain arsenic particulates." Is this information still correct? Also, there is not a deadline for this information, so take your time.
