Mr. Bulow,
Thank you for calling me yesterday in response to my email. Since the application for an individual permit was
submitted, the company should continue with that process, but currently the submittal is incomplete. Because the plant
has been in operation, the application requires that a storm water sample be taken and "the results of at least one
analysis for every pollutant" be shown in Table VII Part A. Below is a list of the missing items needed in order to
complete the application and move forward in the application process.
1. Section V is missing a signature and date.
2. Section VII Part A is missing analysis for pollutants listed for each outfall.
3. "Sampling results for pollutants that you know or have reason to believe is present" is missing from table C.
I have enclosed a copy of the pages with the missing items. The results from the storm water samples will enable us to
craft an appropriate permit for the site.
