The Division of Energy, Mineral, and Land Resources' Stormwater Program received your
request for a minor modification to permit no. NCS000580 on December 20, 2018. The request
was to add a new stormwater discharge outfall, Outfall 002 to Bowes Branch, to the permit. The
Division has reviewed and approved your request. This modification is issued pursuant to the
requirements of North Carolina General Statute 143-215 .1 and the Memorandum of Agreement
between North Carolina and the U.S. Environmental Protection Agency dated October 15, 2007
(or as subsequently amended).
This modification includes the following changes from your current permit:
Modification to the permit in Part II on Page 6 of 15, to include stormwater Outfall 002
in the monitoring requirements.
Revisions to the permit cover page to reflect the additional receiving waters and latest
amendment date.
Please replace these two pages in the current permit with the revised pages and discard
the older pages.
If any parts, measurement frequencies, or sampling requirements contained in this modification
are unacceptable to you, then you have the right to file a petition for a contested case with the
Office of Administrative Hearings within thirty (30) days following receipt of this letter. This
petition must conform to the requirements of Chapter 150B of the North Carolina General
Statutes. Petitions must be timely filed with the Office of Administrative Hearings, Post Office
Drawer 11666, Raleigh, North Carolina 27604. If you do not file a petition within the required
time, the agency decision shall be final and binding.

This permit is not transferable except after notice to the Division. Part III, B.2. addresses the
requirements to be followed in case of change in ownership or control of this discharge. This
permit modification does not affect the legal requirements to obtain other permits which may
be required by this Division, or permits required by the Division of Water Resources, Coastal
Area Management Act, or any other federal or local governmental permit that may be required.

TO DISCHARGE STORMWATER UNDER THE
NATIONAL POLLUTANT DISCHARGE ELIMINATION SYSTEM
In compliance with the provisions of North Carolina General Statute 143-215.1, other
lawful standards and regulations promulgated and adopted by the North Carolina
Environmental Management Commission, and the Federal Water Pollution Control Act, as
amended,

is hereby authorized to discharge stormwater from a facility located at
Mayo Electric Generating Plant

Roxboro, NC 27574
Person County
to receiving waters designated as Mayo Reservoir and Bowes Branch, class WS-V and class
C waters, respectively, in the Roanoke River Basin, in accordance with the discharge
limitations, monitoring requirements, and other conditions set forth in Parts I, II, III, and IV
hereof.
This permit shall become effective January 27, 2017
This permit and the authorization to discharge shall expire at midnight on December 31, 2021.

Outfalls 004, 005 006c, 006d, and 006e are included in wastewater NPDES permit
NCO038377 due to potential non-stormwater discharge.
Outfall 006b does not have a defined point source discharge. Without a point source this
outfall is not covered by the NPDES stormwater program and is not included in this permit.
The following parameters shall be monitored during a measurable storm event from
Outfall 006a and Outfall 010 to the Mayo Reservoir, and Outfall 002 to Bowes Branch.
