Dear Permittee:
Your facility is currently covered for stormwater discharge under NPDES Permit NCS000076. This permit expires
on November 30. 1999. In order to assure your continued coverage under your permit. you must apply to the
Division of Water Quality (DWQ) for renewal of your permit. To make this renewal process easier, we are
informing you in advance that your permit will be expiring. Enclosed you will find an individual permit renewal
application form, supplemental information request, and Stormwater Pollution Prevention Plan certification. Filing
the application form along with the requested supplimental information will constitute your application for renewal
of your permit. The application form must be completed and returned along with all requested information by June
14, 1999 in order to constitute a timely renewal filing.
Recent legislation modified the fee structure for DWQ permits. Renewal fees have been eliminated and annual fees
have been changed. The new annual fee for your permit is now $715.00 (you will be invoiced later this year for
your annual fee_) A copy of the new fee schedule is enclosed in this package.
Failure to request renewal by June 14, 1999 may result in a civil assessment of at least $250.00. Larger penalties
may be assessed depending on the delinquency of the request. Discharge of stormwater from your facility without
coverage under a valid stormwater NPDES permit would constitute a violation of NCGS 143-215.1 and could
result in assessments of civil penalties of up to $ 10.000 per day.

SUPPLEMENTAL INFORMATION REQUIRED FOR RENEWAL OF INDIVIDUAL NPDES
STORM WATER PERMIT
Two copies of each of the following shall accompany this submittal in order for the application to be
considered complete:

(Do not submit the site Stormwater Pollution Prevention Plan)
1. A current Site Map from the Stormwater Pollution Prevention Plan. The location of
industrial activities (including storage of materials, disposal areas, process areas and
loading and unloading areas), drainage structures, drainage areas for each outfall, building
locations and impervious surfaces should be clearly noted.
2. A summary of Analytical Monitoring results during the term of the existing permit (if your
permit required analytical sampling). Do not submit individual lab reports. The summary
can consist of a table including such items as outfall number, parameters sampled, lab
results, date sampled, and storm event data. *
3. A summary of the Visual Monitoring results. Do not submit individual monitoring reports.
The summary can consist of a table including such items as outfall number, parameters
surveyed, observations, and date monitoring conducted.
4. A summary of the Best Management Practices utilized at the permitted facility. Summary
should consist of a short narrative description of each BMP's in place at the facility. If the
implementation of any BMP's is planned, please include information on these BMP's.

facility. Significant changes could include the addition or deletion of work processes,
changes in material handling practices, changes in material storage practices, and/or
changes in the raw materials used by the facility.
6. Certification of the development and implementation of a Stormwater Pollution Prevention
Plan for the permitted facility (Sign and return attached form).
* if the final year analytical monitoring of the existing permit term has not been completed
prior to filing the renewal submittal, then the last years monitoring results should be
submitted within 30 days of receipt of the laboratory reports. (i.e_ do not withhold renewal
submittal waiting on lab results)
Representative storm sampling may now be conducted anytime during the year (the April to
November window has been eliminated) and the representative rainfall event is now
defined as a storm event that measures greater than 0.1 inches and is preceded by at least 72
hours in which no storm event measuring greater than 0.1 inches has occurred.

the outline of the facility, the location of each of its existing and proposed intake and discharge structures, each of Its hazardous waste
treatment, storage, or disposal facilities, and each well where it injects fluids underground. Include all springs, rivers and other surface

XII. NATURE OF BUSINESS(provide a brief description)
The Coming Incorporated - Wilmington Facility - manufactures optical waveguide fibers (SIC Code 3229). The manufacturing process occurs completely
Within enclosed building structures under stringent quality control conditions. By product HCI acid Is also manufactured at the facility. Both manufacturing and
process activities are not exposed to stormwater. All outtalls identified in this application (With the exception of 001 & 002) do not include drainage areas where
loading, unloading, storage or disposal of bulk materials or finished products occurs, Stormwater from these areas goes to outfalls 001 and 002 (also permitted
under NC0003794). Transportation into and out of the facility occurs within the drainage areas described in this application.

Stormwater outfalls DSN003, DSN004, and DSN005 have not received drainage in the past three (3) years from significant materials
treatment, storage, and disposal areas operated in such a manner to allow exposure to storm water. Material management practices
at the facility provide for the proper and/or permitted treatment, storage, and/or disposal of materials w1ftn these and other permitted drainage
areas (see NPOES NC0003794). Pesticide treatment occurs in the grassed areas at an applications rate averaging 10 Ibs, per 4,000 sq.11.

The Wilmington Regional Office received a modification to the Stormwater Management Permit Application for Corning
Glass Tower 5 Expansion on May 19, 1997, with final information on July 10, 1997. Staff review of the plans and
specifications has determined that the project, as proposed, will comply with the Stormwater Regulations set forth in Title
15A NCAC 2H.1000. We are forwarding Permit No. SW8 950318 Modification dated July 14, 1997, for the construction
of Corning Glass Tower 5 Expansion.
This permit shall be effective from the date of issuance until July 14, 2007, and shall be subject to the conditions and
limitations as specified therein. Please pay special attention to the Operation and Maintenance requirements in this permit.
Failure to establish an adequate system for operation and maintenance of the stormwater management system will result
in future compliance problems.
if any parts, requirements, or limitations contained in this permit are unacceptable, you have the right to request an
adjudicatory hearing upon written request within thirty (30) days following receipt of this permit. This request must be
in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with the
Office of Administrative Hearings, P.O. Drawer 27447, Raleigh, NC 27611-7447. Unless such demands are made this
permit shall be final and binding.

4. The tract will be limited to the amount of built -upon area indicated on page 3 of this permit, and per approved
Is plans.
5. The permittee is responsible for verifying that the proposed built -upon area does not exceed the allowable built -
upon area.
6. The following items will require a modification to the permit:
a_ Any revision to the approved plans, regardless of size.
b. Project name change.
C. Transfer of ownership.
d. Redesign or addition to the approved amount of built -upon area.
e. Further subdivision of the project area.
f. Filling in, altering, or piping of any vegetative conveyance shown on the approved plan.
In addition, the Director may determine that other revisions to the project should require a modification to the
permit.
7. The Director may notify the permittee when the permitted site does not meet one or more of the minimum
requirements of the permit. Within the time frame specified in the notice, the permittee shall submit a written time
schedule to the Director for modifying the site to meet minimum requirements. The permittee shall provide copies
of revised plans and certification in writing to the Director that the changes have been made.
H. SCHEDULE OF COMPLIANCE
01. The permittee will comply with the following schedule for construction and maintenance of the stormwater
management system:
a. The stormwater management system shall be constructed in it's entirety, vegetated and operational for it's
intended use prior to the construction of any built -upon surface except roads.
b. During construction, erosion shall be kept to a minimum and any eroded areas of the system will be
repaired immediately.
2. The permittee shall at all times provide the operation and maintenance necessary to assure the permitted stormwater
system functions at optimum efficiency. The approved Operation and Maintenance Plan must be followed in its
entirety and maintenance must occur at the scheduled intervals including, but not limited to:
a. Semiannual scheduled inspections (every 6 months).
b. Sediment removal.
C. Mowing and revegetation of side slopes.
d. Immediate repair of eroded areas.
e. Maintenance of side slopes in accordance with approved plans and specifications.
f. Debris removal and unclogging of outlet structure, orifice device and catch basins and piping.
g. Access to the outlet structure must be available at all times.
3. Records of maintenance activities must be kept and made available upon request to authorized personnel of DWQ.
The records will indicate the date, activity, name of person performing the work and what actions were taken.
4. This permit shall become voidable unless the facilities are constructed in accordance with the conditions of this
permit, the approved plans and specifications, and other supporting data.

5. Upon completion of construction, prior to issuance of a Certificate of Occupancy, and prior to operation of this
permitted facility, a certification must be received from an appropriate designer for the system installed certifying
that the permitted facility has been installed in accordance with this permit, the approved plans and specifications,
and other supporting documentation. Any deviations from the approved plans and specifications must be noted on
the Certification.
6. A copy of the recorded deed restrictions must be submitted to the DWQ within 30 days of the date of the recording.
The recorded copy must be signed by the permittee, dated, stamped with the deed book number and page, and bear
the stamp of the Register of Deeds.
7. A copy of the approved plans and specifications shall be maintained on file by the Perminee for a minimum of ten
years from the date of the completion of construction.
M. GENERAL CONDITIONS
1. This permit is not transferable. In the event there is a desire for the facilities to change ownership, or there is a
name change of the Permittee, a formal permit request must be submitted to the Division of Water Quality
accompanied by an application fee, documentation from the parties involved, and other supporting materials as may
be appropriate. The approval of this request will be considered on its merits and may or may not be approved.
2. Failure to abide by the conditions and limitations contained in this permit may subject the Permittee to enforcement
action by the Division of Water Quality, in accordance with North Carolina General Statute 143-215.6A to 143-
215.6C.
3. The issuance of this permit does not preclude the Permittee from complying with any and all statutes, rules,
regulations, or ordinances which may be imposed by other government agencies (local, state, and federal) which
have jurisdiction.
4. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions, the
Permittee shall take immediate corrective action, including those as may be required by this Division, such as the
construction of additional or replacement stormwater management systems.
5. The perminee grants DEHNR Staff permission to enter the property for the purpose of inspecting all components
of the permitted stormwater management facility.
6. The permit may be modified, revolted and reissued or terminated for cause. The filing of a request for a permit
modification, revocation and reissuance or termination does not stay any permit condition.

L Monthly, or after every runoff producing rainfall event, whichever comes first:
A. Inspect the trash rack; remove accumulated debris, repair/replace if it is not functioning.
B. inspect and clear the orifice of any obstructions. If a pump is used as the drawdown
mechanism, pump operation will be checked. A log of test runs of the pump will be kept
on site and made available to DEM personnel upon request.
C. Inspect the pond side slopes and grassed inlet swales; remove trash, and repair eroded
areas before the next rainfall event.
D. If the pond is operated with a vegetated filter, the filter will be checked for sediment
accumulation, trash accumulation, erosion and proper operation of the flow spreader
mechanism. Repairs/cleaning will be done as necessary.
H. Quarterly:
A. Inspect the collection system (ie. catch basins, piping, grassed swales) for proper
functioning. Accumulated trash will be cleared from basin grates, basin bottoms, and
piping will be checked for obstructions and cleared as required.
B_ Pond inlet pipes will be checked for undercutting, riprap or other energy dissipation
structures will be replaced, and broken pipes will be repaired.
M. Semi-annually:
A. Accumulated sediment from the bottom of the outlet structure will be removed.
B_ The pond depth will be checked at various points. If depth is reduced to 75% of original
design depth or 3 feet whichever is greater, sediment will be removed to at least the
original design depth.

IV. General:
A. Mowing of the side slopes will be accomplished according to the season. Maximum grass
height will be 6" .
B. Cattails are encouraged along the pond perimeter, however they will be removed when they
cover more than 1/2 the surface area of the pond.
C_ The orifice/pump is designed to draw down the pond in 2-5 days. If drawdown is not
accomplished in that time, the system will be checked for clogging. The source of the
clogging will be found and eliminated.
D. All components of the detention pond system will be kept in good working order. Repair
or replacement components will meet the original design specifications as per the approved
sormwater plan.

The Wilmington Regional Office received the modification to the Stormwater Management Permit Application for Corning
Parking Lot Expansion on September 12, 1997, with final information on October 28, 1947. Staff review of the plans and
specifications has determined that the project, as proposed, will comply with the Stormwater Regulations set forth in Title 15A
NCAC 2H.1000. We are forwarding Permit No. SW8 950826 Modification dated December 2, 1997, for the construction of
Parking Lot Expansion.
This permit shall be effective from the date of issuance until December 2. 2007, and shall be subject to the conditions and
limitations as specified therein. Please pay special attention to the Operation and Maintenance requirements in this permit.
Failure to establish an adequate system for operation and maintenance of the stormwater management system will result in
future compliance problems.
If any parts, requirements, or limitations contained in this permit are unacceptable, you have the right to request an adjudicatory
hearing upon written request within thirty (30) days following receipt of this permit. This request must be in the form of a
written petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative
Hearings, P.O. Drawer 27447, Raleigh, NC 27611-7447. Unless such demands are made this permit shall be final and binding.

5. The permittee is responsible for verifying that the proposed built -upon area does not exceed the allowable built -upon
area.
6. The following items will require a modification to the permit:
a. Any revision to the approved plans, regardless of size.
b. Project name change.
C. Transfer of ownership.
d. Redesign or addition to the approved amount of built -upon area.
e. Further subdivision of the project area.
f. Filling in, altering, or piping of any vegetative conveyance shown on the approved plan.
In addition, the Director may determine that other revisions to the project should require a modification to the permit.
7. The Director may notify the permittee when the permitted site does not meet one or more of the minimum requirements
of the permit. Within the time frame specified in the notice, the permittee shall submit a written time schedule to the
Director for modifying the site to meet minimum requirements. The permittee shall provide copies of revised plans
and certification in writing to the Director that the changes have been made.
H. SCHEDULE OF COr%fPLLA.NCE
l . The pertnittee will comply with the following schedule for construction and maintenance of the stormwater management
system:
a. The stormwater management system shall be constructed in it's entirety, vegetated and operational for it's
intended use prior to the construction of any built -upon surface except roads.
b. During construction, erosion shall be kept to a minimum and any eroded areas of the system will be repaired
immediately.
All connections into the stormwater system from future areas/outparcels shall be made such that short-
circuiting of the system does not occur.
2. The permittee shall at all times provide the operation and maintenance necessary to assure the permitted stormwater
system functions at optimum efficiency. The approved Operation and Maintenance Plan must be followed in its entirety
and maintenance must occur at the scheduled intervals including, but not limited to:
a. Semiannual scheduled inspections (every 6 months).
b. Sediment removal.
C. Mowing and revegetation of side slopes.
d. Immediate repair of eroded areas.
e. Maintenance of side slopes in accordance with approved plans and specifications.
f. Debris removal and unclogging of outlet structure, orifice device and catch basins and piping.
g. Access to the outlet structure must be available at all times.
3. Records of maintenance activities must be kept and made available upon request to authorized personnel of DWQ.
The records will indicate the date, activity, name of person performing the work and what actions were taken.
4. This permit shall become voidable unless the facilities are constructed in accordance with the conditions of this permit,
the approved plans and specifications, and other supporting data.

5. Upon completion of construction, prior to issuance of a Certificate of Occupancy, and prior to operation of this
permitted facility, a certification must be received from an appropriate designer for the system installed certifying that
the permitted facility has been installed in accordance with this permit, the approved plans and specifications, and other
supporting documentation. Any deviations from the approved plans and specifications must be noted on .the
Certification.
6. A copy of the approved plans and specifications shall be maintained on file by the Permittee for a minimum of ten years
from the date of the completion of construction.
7. Prior to the sale of any portion of the property, an access/maintenance easement to the stormwater facilities shall be
granted in favor of the permittee if access to the stormwater facilities will be restricted by the sale of any portion of
the property.
M. GENERAL CONDITIONS
This permit is not transferable. In the event there is a desire for the facilities to change ownership, or there is a name
change of the Permittee, a formal permit request must be submitted to the Division of Water Quality accompanied by
an application fee, documentation from the parties involved, and other supporting materials as may be appropriate.
The approval of this request will be considered on its merits and may or may not be approved.
2. Failure to abide by the conditions and limitations contained in this permit may subject the Permittee to enforcement
action by the Division of Water Quality, in accordance with North Carolina General Statute 143-215.6A to 143-
215.6C.
3. The issuance of this permit does not preclude the Permittee from complying with any and all statutes, rules,
regulations, or ordinances which may be imposed by other government agencies (local, state, and federal) which have
jurisdiction.
4. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions, the Perrnittee
shall take immediate corrective action, including those as may be required by this Division, such as the construction
of additional or replacement stormwater management systems.
5 _ The permittee grants DENR Staff permission to enter the property for the purpose of inspecting all components of the
permitted stormwater management facility.
6. The permit may be modified, revoked and reissued or terminated for cause. The filing of a request for a permit
modification, revocation and reissuance or termination does not stay any permit condition.

I. Monthly, or after every runoff producing rainfall event, whichever comes first:
A. Inspect the trash rack; remove accumulated debris, repair/replace if it is not functioning.
B. inspect and clear the orifice of any obstructions. If a pump is used as the dmwdown
mechanism, pump operation will be checked. A log of test runs of the pump will be kept
on site and made available to DEM personnel upon request.
C. Inspect the pond side slopes and grassed inlet swales; remove trash, and repair eroded
areas before the next rainfall event.
D. If the pond is operated with a vegetated filter, the filter will be checked for sediment
accumulation, trash accumulation, erosion and proper operation of the flow spreader
mechanism. Repairs/cleaning will be done as necessary.
II. Quarterly:
A. Inspect the collection system (ie. catch basins, piping, grassed swales) for proper
functioning. Accumulated trash will be cleared from basin grates, basin bottoms, and
piping will be checked for obstructions and cleared as required.
B. Pond inlet pipes will be checked for undercutting, riprap or other energy dissipation
structures will be replaced, and broken pipes will be repaired.
M. Semi-annually:
A. Accumulated sediment from the bottom of the outlet structure will be removed.
B. The pond depth will be checked at various points. if depth is reduced to 75 % of original
design depth or 3 feet whichever p greater, sediment will be removed to at least the
original design depth.
C. Grassed swales, including the vegetated filter if applicablellre, year
as necessary.

IV. General:
A. Mowing of the side slopes will be accomplished according to the season Maximum grass
height will be 6" .
B. Cattails are encouraged along the pond perimeter, however they will be removed when they
cover more than 1 /2 the surface area of the pond-
C. The orifice/pump is designed to draw down the pond in 2-5 days. If drawdown is not
accomplished in that time, the system will be checked for clogging. The source of the
clogging will be found and eliminated.
D. All components of the detention pond system will be kept in good working order. Repair
or replacement components will meet the original design specifications as per the approved
stormwater plan. If previously approved components are determined to be ineffective, the
component must be redesigned and/or replaced.

Parking Lot on April 29. 1998. with final information on June 24. 1998- Staff review of the plans and specifications has
determined that the project, as proposed, will comply with the Stormwater Regulations set forth in Title 15A NCAC
2H.1000. We are forwarding Permit No. SW8 980441 dated August 14, 1998. for the construction of Corning
Construction Parking Lot.
This permit shall be effective from the date of issuance until August 14. 2008, and shall be subject to the conditions and
limitations as specified therein. Please pay special attention to the Operation and Maintenance requirements in this permit.
Failure to establish an adequate system for operation and maintenance of the stormwater management system will result
in future compliance problems -
If any parts, requirements. or limitations contained in this permit are unacceptable. you have the right to request an
adjudicatory hearing upon written request within thirty (30) days following receipt of this permit. This request must be

5. The runoff from all built -upon area on the project (including future areas) must be directed into the permitted
• stormwater control system.
6. The permittee is responsible for verifi ing that the proposed built -upon area does not exceed the allowable built -
upon area.
7. The following items will require a modification to the permit:
a. Any revision to the approved plans, regardless of size.
b. Project name change-

d. Redesign or addition to the approved amount of built -upon area.
e. Further subdivision of the project area.
f. Filling in, altering, or piping of any vegetative conveyance shown on the approved plan.
In addition, the Director may determine that other revisions to the project should require a modification to the
permit_
8. The Director may notify the permiaee when the permitted site does not meet one or more of the minimum
requirements of the permit. Within the time frame specified in the notice, the permittee shall submit a written time
schedule to the Director for modifying the site to meet minimum requirements. The permittee shall provide copies
of revised plans and certification in writing to the Director that the changes have been made_
II. SCHEDULE OF COMPLIANCE
. 1. The permittee will comply with the following schedule for construction and maintenance of the stormwater
management system:
a. The stormwater management system shall be constructed in it's entirety, vegetated and operational for its
intended use prior to the construction of any built -upon surface except roads.
b. During construction. erosion shall be kept to a minimum and any eroded areas of the system will be
repaired immediately.
2. The permittee shall at all times provide the operation and maintenance necessary to assure the permitted stormwater
system functions at optimum efficiency. The approved Operation and Maintenance Plan must be followed in its
entirety and maintenance must occur at the scheduled intervals including, but not limited to:
a. Semiannual scheduled inspections (even: 6 months).
b. Sediment removal.
C. Mowing and revegeration of side slopes.
d. Immediate repair of eroded areas.
e. Maintenance of side slopes in accordance with approved plans and specifications.
f. Debris removal and unclogging of outlet structure, orifice device and catch basins and piping.
Access to the outlet structure must be available at all times.
3. Records of maintenance activities must be kept and made available upon request to authorized personnel of DWQ.
The records will indicate the date. activity, name of person performing the work and what actions were taken_
4. Decorative spray fountains will not be allowed in the ponds-

. 5. This permit shall become voidable unless the facilities are constructed in accordance with the conditions of this
permit, the approved plans and specifications. and other supporting data.
Upon completion of construction. prior to issuance of a Certificate of Occupancy, and prior to operation of this
pennined facility, a certification must be received from an appropriate designer for the system installed certifying
that the permitted facility has been installed in accordance with this permit, the approved plans and specifications,
and other supporting documentation. Any deviations from the approved plans and specifications must be noted on
the Certification.
A copy of the approved plans and specifications shall be maintained on file by the Permittee for a minimum of ten
years from the date of the completion of construction.
Prior to the sale of any portion of the property, an access/maintenance easement to the stormwater facilities shall
be granted in favor of the permittee if access to the stormwater facilities will be restricted by the sale of any portion
of the property.
III. GENERAL CONDITIONS
This permit is not transferable. In the event there is a desire for the facilities to change ownership, or there is a
name change of the Permittee, a formal permit request must be submitted to the Division of Water Quality
accompanied by an application fee, documentation from the parties involved, and other supporting materials as may
be appropriate. The approval of this request will be considered on its merits and may or may not be approved.
2. Failure to abide by the conditions and limitations contained in this permit may subject the Permittee to enforcement
action by the Division of Water Quality, in accordance with North Carolina General Statute 143-215.6A to 143-
8 215.6C.
The issuance of this permit does not preclude the Permittee from complying with any and all statutes, rules,
regulations, or ordinances which may be imposed by other government agencies (local, state, and federal) which
have jurisdiction.
In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions. the
Permittee shall take immediate corrective action, including those as may be required by this Division, such as the
construction of additional or replacement stormwater management systems.
The permittee grants DENR Staff permission to enter the property for the purpose of inspecting all components
of the permitted stornwater management facility.
The permit may be modified. revoked and reissued or terminated for cause. The filing of a request for a permit
Inodlflcation, revocation and relssuance or termination does not stay any permit condition.

ATTACHMENT A
I. Deed Restriction Language
The following statements must be recorded for all low density projects and for all projects that involve the
subdivision and selling of lots or outparcels:
1. No more than square feet of any Iot, including that portion of the right-of-way between the
edge of pavement and the front lot line, shall be covered by impervious structures including
asphalt, gravel, concrete, brick, stone, slate or similar material but not including wood decking or
the surface of swimming pools. This covenant is intended to insure continued compliance with the
stormwater permit issued by the State of North Carolina. The covenant may not be changed or
deleted without the consent of the State.
2. No one may fill in, pipe, or alter any roadside swale except as necessary to provide a minimum
driveway crossing.
NOTE: if lot sizes vary significantly, the owner, developer must provide an attachment listing each lot
number, size and the allowable built -upon area for each lot.
For commercial projects that have outparcels or future development the following statements must also be
recorded:

I. Monthly, or after every runoff -producing rainfall event, whichever comes first:
A. Inspect the trash rack; remove accumulated debris, repair/replace if it is not functioning.
B. Inspect and clear the orifice of any obstructions. If a pump is used as the drawdown
mechanism, pump operation will be checked. A log of test runs of the pump will be kept
on site and made available to DWQ personnel upon request.
C. Inspect the pond side slopes and inlet swales; remove trash, and repair eroded areas before the
next rainfall event.
D. If the pond is operated with a vegetated filter, the filter will be checked for sediment
accumulation, trash accumulation, erosion and proper operation of the flow spreader
mechanism. Repairs/cleaning will be done as necessary.
H. Quarterly (Four times a year):
A. Inspect the collection system (i.e., catch basins, piping, swales) for proper functioning.
Accumulated trash will be cleared from basin grates, basin bottoms, and piping will be
checked for obstructions and cleared as required.
B. Pond inlet pipes will be checked for undercutting. Riprap or other energy dissipation
structures will be replaced, and broken pipes will be repaired.
III. Semi-annually (twice a year):
A. Accumulated sediment from the bottom of the outlet structure will be removed.
B. The forebay depth will be checked at various points. Sediment removal is required when the
depth is reduced to 75 % of original design depth of 4 feet-
C. Swales, including the vegetated filter if applicable, will be regraveled as necessary. Repair
eroded areas immediately.

IV. Annually (Once a year):
A. The pond depth will be checked at various points. Sediment removal is required when the
depth is reduced to 75 % of the original design depth or 3' , whichever is greater. Design
depth is 7-1/2 feet, measured vertically from the orifice down to the pond's bottom. Sediment
must be removed to at least the original design depth.
V . General:
A. Maintenance of the side slopes is required. Maximum grass height will be 6".
B. Cattails are encouraged along the pond perimeter; however, they will be removed when they
cover more than 1/2 the surface area of the pond. The best time to cut them is at the end of
the growing season, in November. Environmentally sensitive chemicals for use in killing
cattails can also be used. Contact your nearest Agricultural Extension Agent.
C. The orifice/pump is designed to draw down the pond in 2-5 days. If drawdown is not
accomplished in that time, the orifice will be replaced with a larger or smaller orifice. Slow
drawdown may be attributed to a clogged system. The source of the clogging will be found
and eliminated.
D. All components of the detention pond system will be kept in good working order. Repair or
replacement components will meet the original design specifications as per the approved
stormwater plan. If previously approved components are determined to be ineffective, the
component will be redesigned and/or replaced.

Site Drainage Controls
Introduction
The Coming Incorporated facility in Wilmington has two (2) NPDES outfalls for non -
contact wastewater and stormwater, three (3) NPDES outfolls for stormwater only, and
eight (8) stormwater outfalls serving areas exempt from NPDES permitting. Attachment B
contains a table listing the total drainage area acreage for the outfolls discussed in the
next sections.
NPDES Permitted Outfalls
Outfalls 001 & 002
NPDES Permit No. NCO003794 covers wastewater discharges from the facility via NPDES
Outfalls 001 and 002 (see attachment F Drawing labeled 'Overall Sheet"). These two
outfolls also discharge stormwater runoff from portions of the facility where chemical
storage, loading and unloading occur. Outfall 001 also receives stormwater runoff from
❑ portion of the parking area labeled Drainage Area 14. This drainage area is
considered to be an exempt area since it is only a parking lot (see Attachment F,
Drawing Sheet labeled "Quad 3"). Outfall 001 has a total drainage area of 2.24 acres.
This total does not include the exempt Drainage Area 74. Outfall 002 has a total
. drainage area of 9.24 acres with 6.87 acres being impervious.
Outfails 003,004 & 005
The NPDES outfalls at the Coming Wilmington facility discharging only stormwater receive
runoff from 12.46 acres, 7.28 acres being impervious, and discharges through three (3)
separate outfalls identified as Ouffall 003, Ouffoll 004 and Outfall 005 (see Attachment F,
Drawing labeled "Overall Sheet).
Drainage Area 3 is bounded on the south by the property line (see Attachment F,
Drawing Sheets labeled 'Quad 1 ' and "Quad 4). The total drainage area is 7.87 acres
with 5.15 acres being impervious. Drainage Area 3 discharges through Outfall 003. A
stormwater retention pond was constructed in Drainage Area 3 in 7998 as required by
the terms and condition of 15A NCAC 2H .1000 (see Attachment E, 'State Stormwater
Management Systems Permit No. SW8 980441 ).
Drainage Area 4 receives stormwater drainage from a portion of the Manufacturing
Building roof (see Attachment F Drawing Sheet labeled "Quad 4). All2.12 acres of
Area 4 are impervious. Drainage Area 4 discharges through Outfall 004. Also, exempt
Drainage Area 5D discharges through Outf611004. Drainage Area 5D is O.64 acres of
impervious surface.
Drainage Areas 5A and 5B discharge to a stormwater retention pond (see Attachment
C, 'State Stormwoter Management Systems Permit No. SW8 950318) located prior to
Outfall 005. The retention pond was constructed in 7996. Drainage Area 5A contains 2.54
. total acres including 0.61 impervious acres. Drainage Area 5B is 7.07 acres with 5.70 acres
impervious. Two exempt drainage areas discharge through Outfall 005. Exempt
• Drainage Area 5C is comprised of a manicured front lawn, with 2.21 total acres and 0.33
acres impervious. Exempt Drainage Area 5D receives stormwater drainage from an
administrative office building roof (see Attachment F, Drawing Sheets labeled "Quad 3"
and "Quad 4 ").
Outfails Exempt From NPDES Permitting
Eight outfalls are identified on the storm drain piping drawings in Attachment F as
"Exempt Outfolls (EO) These ore outfalls receive water from areas containing no
industrial activity and no potential for stormwater contamination; therefore these outfalls
and corresponding drainage area are exempt under NPDES regulations.
Drainage Areas Not Being Discharged Through Ouffalls
The remainder of the Corning Incorporated property, Drainage Area 6 and Drainage
Area 9, is completely pervious with no direct discharge points from the site. Drainage
Area 6 is 2.54 acres and Drainage Area 9 is 10.22 acres.
Site Drainage Controls
The Wilmington plant system design standard requires storm drain systems to function
during a 50-year storm event. All three (3) stormwater retention ponds meet the
requirements of NCDWQ State Stormwater Management Systems permit program.
AI123 secondary containment systems located at the facility have been upgraded to
. retain the capacity of the largest vessel plus a 24-hour/25-year storm event (defined as 8
inches of rainfall for the Wilmington area). The Stormwater Pollution Prevention Plan (SP3)
Secondary Containment Vessels Compliance Program includes keyed lockouts of the
drain systems. The Area SP3 team identified 'appropriate testing" and record keeping
provisions during Best Management Practices (BMP) development. Collected stormwater
from all 23 secondary containment systems is evaluated prior to making a determination
to discharge any collected stormwater to the outfalls. There is an alternate system
available for the discharge of contaminated stormwater. Collected stormwater can be
redirected, if required due to contamination, to the Pollution Abatement Process for
pretreatment prior to discharge to the City of Wilmington WWTP.
