System (NPDES) permit number NCS000370. The permit requires semi-annual
quantitative sampling of storm water at the facility's two outfalls. The outfalls are named
by their geographic locations, South and East. Please find the analytical results and
Storm Water Outfall Discharge Monitoring Reports for the Sampling Year 9 Period for
the South and East outfalls in the Attachment. The samples were collected on
November 12, 2018. The analytical results were received from the laboratory via
electronic mail at BATO Wilson on November 26, 2018. As required, the original and
one copy are enclosed.

System (NPDES) permit number NCS000370. The permit requires semi-annual
quantitative sampling of storm water at the facility's two outfalls. The outfalls are named
by their geographic locations, South and East. Please find the analytical results and
Storm water Outfall Discharge Monitoring Reports for the Sampling Year 7 Period 2 for
the South outfall in the Attachment. The sample for the East Outfall was collected on
July 23, 2017. The analytical results were received from the laboratory via electronic
mail at BATO Wilson on August 1, 2017. Included for completeness, are the results
from the East outfall, which was sampled on June 5, with results already submitted to
the agency. As required, the original and one copy are enclosed.

The BATO-Wilson facility operates under National Pollutant Discharge Elimination
System (NPDES) permit number NCS000370. The permit requires semi-annual
quantitative sampling of stormwater at the facility's two outfalls. The outfails are named
by their geographic locations, South and East. Please find the analytical results and
Stormwater Outfall Discharge Monitoring Reports for the second half of 2014 in the
Attachment. The samples were collected on December 16, 2014 and the analytical
results were received from the laboratory via electronic mail at BATO Wilson on
January 9, 2015. As required, the original and one copy are enclosed.
According to the NCDENR letter dated June 13, 2014 (attached), regarding the zinc
benchmark in NPDES Permit NCS00370, the Wilson plant will continue the required
semi-annual monitoring for zinc.

In response to your discussions with NCDENR nand Quality Section staff, concerning multiple
exceedances of the benchmark value for zinc, [lave Parnell of the Land Quality Section -
Stormwater staff, conducted a compliance evaluation inspection (CEI) on May 22, 2014. The
inspection was conducted to determine compliance with the conditions of your NPDES
NCS00370 Stormwater Permit and to discuss any actions that have been or could be taken to
identify and eliminate potential sources of Zinc related to facility operations.
Keep in mind that benchmark exceedances are not limitviolations or violations of permit
conditions; however, you are obligated to follow the tiered response actions outlined in your
permit At the time of the inspection, your facility was found to be in compliance with the
tiered response actions and other conditions of the permit. During the inspection, you are
reported to have relayed to Mr. Parnell that you have been unable to determine the source of
the higher levels of zinc, found at the South outfall. It was reported that zinc oxide is used in
the process of producing your tires, but the addition of zinc oxide into the production process
occurs within the drainage area of the East outfall. Zinc oxide is not used in the tires
production within the drainage area of the South outfall. No exceedance of zinc is found
within the East outfall. Also, it is notable in this case, that numerous best management
practices (BMP) implemented during the last several months, appears to have had an effect on
the reduction on zinc at the South outfall. Your diligence in attempting to determine and

eliminate the zinc source and your continued communication with Raleigh Regional Office
staff is commendable and Is documented back to September 2013.
Based on the aforementioned items, DENR Land Quality Section is granting regulatory relief in
the form of a collapsed Tier 2-Tier 3 response. Upon receipt of this letter, Bridgestone
Americas "Tire Operation - Wtison may resume the permit specified semi-annual analytical
monitoring for the remainder of the current permit term - which ends August 31, 2015. This
decision applies only to the zinc benchmark. A benchmark excceedance of any other
parameter listed In your permit will trigger the tiered response actions as described in
your permit
You must`notify this office, in writing, within five business days if you become aware of any
significant source of zinc at your facility that has the potential to be exposed to stormwater.
The relief granted in this letter is contingent upon the current industrial practices at

In response to your discussions with NCDENR Land Quality Section staff, concerning multiple
exceedances of the benchmark value for zinc, Dave Parnell of the Land Quality Section -
Stormwater staff, conducted a compliance evaluation inspection (CEI) on May 22, 2014. The
inspection was conducted to determine compliance with the conditions of your NPDES
NCS00370 Stormwater Permit and to discuss any actions that have been or could be taken to
identify and eliminate potential sources of Zinc related to facility operations.
Keep in mind that benchmark exceedances are not limit violations or violations of permit
conditions; however, you are obligated to follow the tiered response actions outlined in your
permit: At the time of the inspection, your facility was found to be in compliance with the
tiered response actions and other conditions of the permit. During the inspection, you are
reported to have relayed to Mr. Parnell that you have been unable to determine the source of
the higher levels of zinc, found at the South outfall. It was reported that zinc oxide is used in
the process of producing your tires, but the addition of zinc oxide into the production process
occurs within the drainage area of the East outfall. Zinc oxide is not used in the tires
production within the drainage area of the South outfall. No exceedance of zinc is found
within the East outfall. Also, it is notable in this case, that numerous best management
practices (BMP) implemented during the last several months, appears to have had an effect on
the reduction on zinc at the South outfall. Your diligence in attempting to determine and

eliminate the zinc source and your continued communication with Raleigh Regional Office
staff is commendable and is documented back to September 2013.
Based on the aforementioned items, DENR Land Quality Section is granting regulatory relief in
the form of a collapsed Tier 2-Tier 3 response. Upon receipt of this letter, Bridgestone
Americas Tire Operation - Wilson may resume the permit specified semi-annual analytical
monitoring for the remainder of the current permit term - which ends August 31, 2015. This
decision applies only to the zinc benchmark. A benchmark exceedance of any other
parameter listed in your permit will trigger the tiered response actions as described in
your permit.
You must notify this office, in writing, within five business days if you become aware of any
significant source of zinc at your facility that has the potential to be exposed to stormwater.
The relief granted in this letter is contingent upon the current industrial practices at
Bridgestone. If industrial practices change and zinc does become a significant stormwater
exposure risk then this office reserves the right to withdraw this decision and reinstate the
permit specified tiered response or other actions that may be warranted by the new set of
circumstances.

Bridgestone Americas Tire Operations, LLC ("Bridgestone") Wilson Plant operates
under National Pollutant Discharge Elimination System (NPDES) permit number
,�NCS000370. The permit requires semi-annual quantitative sampling of stormwater at
the facility's two outfalls. The outfalls are named by their geographic locations, South
and East. The 41h year, 2nd period (Sample 8) stormwater sample was due to be taken
between March 1, 2014 and August 31, 2014. Due to emergency events at our facility,
we did not collect Sample 8 during the sampling period. Per the instructions you gave
the Bridgestone Wilson Plant during our November 5, 2014 phone conference, we
proceeded to collect the sample on November 17, 2014 and are submitting the attached
analytical results by way of this letter.
The Bridgestone Wilson Plant regrets the delay in collecting samples during the

The Bridgestone Americas Tire Operations, LLC (BATO) Wilson Plant operates under National
Pollutant Discharge Elimination System (NPDES) stormwater permit No. NCS000370. The plant is
permitted to discharge stormwater run-off through two outfalls, which are referred to by their
geographic location at the plant, East and South. The permit requires semi-annual sampling of
each outfall in accordance with Section B Table 2 of the permit.
Both the East and South Outfall Year 4 Period 1 sampling analytical results for zinc triggered Tier
Two requirements which include monthly analytical and qualitative monitoring.
In March, 2014 there was no qualifying rain event during time when we could successfully get a
representative sample thus Bridgestone was not able to collect a representative sample as defined
in the permit. As such, we are submitting this March, 2014 monitoring report indicating "No
sample" to comply with the reporting requirements

In response to your discussions with NCDENR Land Quality Section staff, concerning multiple
exceedances of the benchmark value for zinc, Dave Parnell of the Land Quality Section -
Stormwater staff, conducted a compliance evaluation inspection (CEI) on May 22, 2014. The
inspection was conducted to determine compliance with the conditions of your NPDES
NCS00370 Stormwater Permit and to discuss any actions that have been or could be taken to
identify and eliminate potential sources of Zinc related to facility operations.
Keep in mind that benchmark exceedances are not limit violations or violations of permit
conditions; however, you are obligated to follow the tiered response actions outlined in your
permit. At the time of the inspection, your facility was found to be in compliance with the
tiered response actions and other conditions of the permit. During the inspection, you are
reported to have relayed to Mr. Parnell that you have been unable to determine the source of
the higher levels of zinc, found at the South outfall. It was reported that zinc oxide is used in
the process of producing your tires, but the addition of zinc oxide into the production process
occurs within the drainage area of the East outfall. Zinc oxide is not used in the tires
production within the drainage area of the South outfall. No exceedance of zinc is found
within the East outfall. Also, it is notable in this case, that numerous best management
practices (BMP) implemented during the last several months, appears to have had an effect on
the reduction on zinc at the South outfall. Your diligence in attempting to determine and

eliminate the zinc source and your continued communication with Raleigh Regional Office
staff is commendable and is documented back to September 2013.
Based on the aforementioned items, DENR Land Quality Section is granting regulatory relief in
the form of a collapsed Tier 2-Tier 3'response. Upon receipt of this letter, Bridgestone
Americas Tire Operation - Wilson may resume the permit specified semi-annual analytical
monitoring for the remainder of the current permit term -which ends August 31, 2015. This
decision applies only to the zinc benchmark. A benchmark exceedance of any other
parameter listed in your permit will trigger the tiered response actions as described in
your permit.
You must notify this office, in writing, within five business days if you become aware of any
significant source of zinc at your facility that has the potential to be exposed to stormwater.
The relief granted in this letter is contingent upon the current industrial practices at
Bridgestone. If industrial practices change and zinc does become a significant stormwater
exposure risk then this office reserves the right to withdraw this decision and reinstate the
permit specified tiered response or other actions that may be warranted by the new set of
circumstances.

The BATO-Wilson facility operates under National Pollutant Discharge Elimination
System (NPDES) permit number NCS000370. The permit requires semi-annual
quantitative sampling of stormwater at the facility's two outfalls. The outfalls are named
by their geographic locations, South and East. The year 3, period 2 (sample 6)
stormwater sample is due between March 1, 2013 and August 31, 2013, Please find the
analytical results for the facility's two outfalls in the Attachment. As required, the original
and one copy are enclosed. The samples were collected on October 7, 2013 and the
analytical results were received from the laboratory via electronic mail at BATO Wilson
on October 30, 2013. It is important to note that the facility received an extension for
performing this sampling event in order to complete Tier I BMP action items. Upon
completion of the Tier I action items identified in the memo dated September 24, 2013,
the facility performed sampling at the next representative storm event (October 7,
2013).

Source of the Upset/Spill/Bypass (Location or Treatment Unit): The Permittee's facility is a tire
manufacturing plant located at 3001 Firestone Parkway, Wilson, NC (Latitude: 35045'34"N and
Longitude: 77°52'8"W). The source of the spill was a tote from drain platform at an oil water
separator. The tote contained a mixture of oil and water.
Cause or Reason for the Upset/Spill/Bypass; A forklift operator was removing a 75% full tote from
the drain platform for the oil 1 water separator. The pallet portion of the tote was broken and the tote
fell from the forks. The tote fell onto a driveway and graveled area next to a concrete railcar dock
wall.
Describe the Repairs Made or Actions Taken: Immediate response actions consistent with the
facility's SPCC plan and employee training were taken when the tote fell. The forklift operator, a

member of the maintenance team that responds to spills, immediately righted the tote to stop the spill
and notified his supervisor by radio. A maintenance crew quickly assembled at the spill and used
absorbent socks and pads to contain and absorb the spill and all runoff. No oillwater reached any
drains or surface waters. The maintenance crew shoveled the oillwater mixture, as well as used
absorbent materials, into drums for proper disposal. The maintenance crew also removed the top
layer of gravel and dirt and placed it into drums for proper disposal. Maintenance and environmental
personnel conducted multiple inspections of the area and the pertinent stormwater outfall after the
immediate response actions to ensure that the spill had been properly addressed and that it did not
reach surface waters.
Action Taken to.Contain SpillClean Up and Rmediate the Site (if applicable): Immediate response
actions consistent with the facility's SPCC plan and employee training were taken when the tote fell.

immediate response actions, no additional remedial or corrective actions were necessary. The spill
did not reach any drains or surface waters.

Enclosed with this letter is a copy of the draft stormwater permit for your facility. Please review the draft
very carefully to ensure thorough understanding of the conditions and. requirements it contains.
The draft permit contains the following significant chanties from -this facility's current permit:
Analytical monitoring changes:
1.' Analytical monitoring parameters, Total Zinc, TSS, and pH, have been maintained in this permit. Add
COD and TPH. have been added.
2. -All analytical monitoring remains at semi_annually during a representative storm event as defined in Part
'II Section B. The permittee must also document the total precipitation for each event. If no discharge
occurs during the sampling period, the permittee must submit a monitoring report indicating "No Flow"
within 30 days of the end of the six-month sampling period. Additionally, samples must be taken a
minimum of 60 days apart, as specified in Table 2.-
3. Benchmarks for analytical monitoring have been maintained in this draft permit. Exceedances of
benchmark values require the permittee to increase monitoring, increase management actions, increase
record keeping, and/or install stormwater Best Management Practices (BMPs) in a tiered program. If the
sampling results are above a benchmark value, or outside of the benchmark range, for any parameter at
any outfall then the facility shall follow the Tier 1 guidelines which require a facility inspection within
two weeks and implementation of a mitigation plan within two months. If during the term of this permit,
the -sampling results are above the benchmark values, or outside_ of the benchmark range, for any specific
:__parameter at a specific discharge outfall two times in a row (consecutive), then the facility shall follow the %`
Tier 2 guidelines which require a repetition of the steps listed for Tier 1 and also immediately institute
monthly monitoring for all parameters at every outfall where a sampling result exceeded the benchmark
vahxe for two consecutive samples.
4. Yo;r,are required to collect all of the analytical and qualitative monitoring samples during representative
storm events as defined in Part Il Section B. Qualitative monitoring is required regardless of
representative outfall status.
5. You are responsible for all monitoring until the renewal permit is issued. See Footnote 1 of Tables 1, 4,
and 5.
6. Vehicle maintenance monitoring remainsat semi-annuaIIy hi order to coincide with analytical and
qualitative monitoring. Monitoring for TPH has replaced Oil & Grease. This requirement appears in all
Wetlands and Stormwater Branch One

Individual Stormwater permits, however it only applies to facilities that do vehicle maintenance.' If the
facility begins vehicle maintenance during the permitted timeframe then the requirements shall apply.
Other permit changes:
1. Per the requirements of the Neuse Riparian Buffer Rule and Tar -Pamlico Riparian Buffer Rule, all
stormwater drainage from portions of this site that have been constructed after July 22,1997 must
be discharged through a.correctly designed level spreader or another device that meets diffuse flow
requirements per 15A NCAC 213.0233. Diffuse flow requirements are described in Chapter 8 of the
North Carolina Stole-mwater BMP Manual, available at:

2. In addition to submitting,two signed copies of DMRs to Central Files, a separate signed Annual Summary-
