SUPPLEMENTAL INFORMATION REQUIRED FOR RENEWAL OF INDIVIDUAL
NPDES STORM WATER PERMIT
Two copies of each of the following shall accompany this submittal in order for the application
to be considered complete:
(Do not submit the site Stormwater Pollution Prevention Plan)
Initials
1. A current Site Map from the Stormwater Pollution Prevention Plan. The location of
industrial activities (including storage of materials, disposal areas, process areas and
loading and unloading areas), drainage structures, drainage areas for each outfall,
building locations and impervious surfaces should be clearly noted.
2. A summary of Analytical Monitoring results during the term of the existing permit
(if your permit required analytical sampling). Do not submit individual lab reports.
The summary can consist of a table including such items as outfall number,
parameters sampled, lab results, date sampled, and storm event data.
X73. A summary of the Visual Monitoring results. Do not submit individual monitoring
reports. The summary can consist of a table including such items as outfall number,
parameters surveyed, observations, and date monitoring conducted.
4. A summary of the Best Management Practices utilized at the permitted facility.
Summary should consist of a short narrative description of each BMP's in place at
the facility. If the implementation of any BMP's is planned, please include
information on these BMP's.

5. A short narrative describing any significant changes in industrial activities at the
permitted facility. Significant changes could include the addition or deletion of
work processes, changes in material handling practices, changes in material storage
practices, and/or changes in the raw materials used by the facility.
_ 6. Certification of the development and implementation of a Stormwater Pollution
Prevention Plan for the permitted facility (Sign and return attached form).
If the final year analytical monitoring of the existing permit term has not been completed
prior to filing the renewal submittal, then the last years monitoring results should be submitted
within 30 days of receipt of the laboratory reports. (i.e. do not withhold renewal submittal
waiting on lab results)

Best Management Practices
January 2019
Best Management Practices (BMPs)
Best management (BMPs) includes proper equipment design and operation,
manufacturing and material handling procedures, structural controls and landscaping
practices.
• Spill Containment Structures: Containment structures are constructed of concrete
and concrete block. All containment structures are sized to hold the contents of
the largest tank in addition to accumulated rainwater. Each containment structure
has the maximum level for rainwater clearly marked such that the structure will still
hold the contents of the largest storage tank.
• Drainage from Containment Areas: All containment areas have manually operated
valves or pumps, which remain closed and locked when not in operation. Drain
valves are on/off type — no flapper type valves are used. Accumulated stormwater
is inspected and inspection results are documented before stormwater is released.
Only trained personnel may inspect and release stormwater.
• Storage Tank Design and Installation: All storage tanks are constructed of
materials compatible with material stored and operating pressure and temperature.
All storage tanks have concrete secondary containment structures. All bulk
storage tanks have high level interlocks that will stop material transfer when
tripped.
• Material Loading/Unloading: All bulk material handling is performed in spill -
contained areas. Standard Operating Procedures are required for all bulk material
handling operations and checklists are used for each transfer. Only trained
personnel are permitted to load or unload material.
• Preventative Maintenance: The Site's preventative maintenance program includes
inspections of bulk storage tanks, loading stations, transfer piping, spill
containment structures, high level alarms, critical process equipment and
instrumentation.

• Inspections: I he site's Environmental, Health and Safety team conduct monthly
inspections of the stormwater outfall as well as semi-annual inspections for the
qualitative monitoring requirement.
• Personnel Training and Instruction: All process operators receive basic
environmental awareness and safety training before beginning specific process
training. A qualified operator must train all process operators before they are
allowed to operate any process alone. Corporate policy requires standard
operating procedures (SOPs) for process operations.
• Production Areas: All production operations are conducted in enclosed buildings.
• Particulate Emission Control: All processes handling particulates have dust
collectors to control particulate emissions. All dust collectors are part of the Site's
preventative maintenance program
• Housekeeping: Housekeeping inspections are conducted monthly by each
production building. This helps to ensure the areas are maintained properly and
to prevent safety hazards.
• Security: All buildings are locked and there is limited access to the production
buildings due to established badge accessible fenced in areas.
• Erosion Prevention and Control: Parking areas are concrete and asphalt. The
remaining ground is covered with grass, shrubs, and trees. Landscaping and
contouring of the site grounds have been utilized to minimize run-off of stormwater
from non-production/storage areas into active chemical usage and storage areas.

Significant Changes in Industrial Activities
January 2019
Significant Changes:
Our parent company is in the process of closing this facility. In the process of the
shutdown, the following significant changes have been implemented or are in process:
Closure of Building#5: All production activities have ceased in Building #5 on the
site. All production equipment has been moved out and transferred to other sites
within the company. The three above -ground storage tanks (AST's) that were
located at this building were professionally cleaned and removed from the site.
The secondary containment structure was then cleaned and decommissioned.
The building is now idle.
• Reduction of headCOUnt and production (sitewide): Due to the impending closure
of the site, headcount has been dramatically reduced, thereby reducing the
production output of the two remaining production buildings. This reduction is due
to transfer of equipment and processes to other facilities within the company.
• Reduction of inventory: Inventories of both raw materials and finished goods have
been greatly reduced, thereby significantly reducing shipping, receiving and
storage of chemicals and the associated risks of handling these products.
• Discontinuation of the use of Xylene in manufacturing: The use of Xylene in
manufacturing was discontinued late in 2018 when the process in which it was
used was transferred to another site within the company.
• Decommissioning of Above -Ground Storage Tanks (AST's)- Two (2) AST's were
decommissioned, cleaned and removed from Building #1 and the secondary
containment was cleaned and decommissioned. Five (5) AST's were
decommissioned, cleaned and removed from the north tank farm at Building #3.
More tanks are still in use in this tank farm, but will be decommissioned in the
coming months. Two (2) 6,000-gallon AST's and one 8,000-gallon AST were
decommissioned and cleaned at the tank farm to the west of Building 3. The tanks
are still in place, waiting for disposition, but are not in use. All associated piping
and utilities have been removed.
