Interim Director Subject: Request for Additional Information Stormwater Permit No. SWI 180402 Jo Ellen Ammons Fieldhouse for Mars Hill University — Mars Hill, NC Madison County Dear Mr. Johnson: The Division of Energy, Mineral and Land Resources (DEMLR), received a Stormwater Management Permit Application for the subject project on April 30, 2018. A review of that information has determined that the application is not complete. The following information is needed to continue the stormwater review: 1. Please submit an SWU-101 —State Stormwater Permit Application Form. This form can be found at: https:Hdeq.nc.gov/about/divisions/energy-mineral-land-resources/energ_y- mineral-land-rules/stormwater-pro rg am/post-construction. 2. Please use the Discrete NRCS Curve Number Method or Simple Method for calculating the water quality volume. Using a composite CN yields an artificially lower volume. General MDC 1 requires the SCM to be sufficiently sized to handle the treatment volume. 3. Please provide a plan view of the bioretention system on the stormwater plan that labels inlets, outlets, erosion protection, underdrain pipes, cleanouts, and other significant features of the system. 4. The drainage area maps should identify pervious and impervious areas in both the proposed and existing conditions. 5. Please specify on the plans that the vegetation in the bioretention cell should consist of non -clumping, deep-rooted species. 6. Please specify on the plans that the P -index of the media must be less than 50. 7. General MDC 6 requires that SCMs have a method to drawn down any standing water to facilitate maintenance and inspection. 8. Revise the plans to show a maintenance access and easement that extend to the nearest right-of-way. The maintenance access must be at least 10' wide and have side slopes no steeper than 3:1 (horizontal to vertical). The easement must contain the entire maintenance access and SCM. 9. Provide pdfs of all revisions, 2 hardcopies of revised plan sheets, and l hardcopy of other documents. Pdfs must be uploaded using the form at: hqps:Hedocs.deg.nc.gov/Forms/SW ProjectSubmittal Please note that this request for additional information is in response to a preliminary review. The requested information should be received by this Office prior to June 10, 2018, or the application will be returned as incomplete. The return of a project will necessitate resubmittal of all required items, including the application fee. Please reference the State assigned project number SW1180402 on all correspondence. If you need additional time to submit the information, please submit your request for a time extension to the Division at the contact below. The request must indicate the date by which you expect to submit the required information. The Division is allowed 90 days from the receipt of a completed application to issue the permit. The construction of any impervious surfaces, other than a construction entrance under an approved Sedimentation Erosion Control Plan, is a violation of NCGS 143-215.1 and is subject to enforcement action pursuant to NCGS 143-215.6A.
