Step 1: Open the ONLINE FORM

Step 2: Click on the boxes next to "Type of Mitigation Project" that applies to the document you are
uploading. Example #1: If the document being uploaded needs to be reviewed primarily for a Buffer or
Nutrient Offset Mitigation Site, you will click the boxes for "Buffer" and "Nutrient Offset" (ex. BPDP).
Example #2: If the document being uploaded needs to be reviewed primarily for a Stream or Wetland
Mitigation Site, you will click the boxes for "Stream" and "Wetland" (ex. Stream & Wetland Restoration
Designs). Example #3: If the document being uploaded needs to be reviewed for both a Buffer &
Nutrient Offset Mitigation Site as well as a Stream Mitigation Site, you will click the boxes for "Stream",
"Buffer" and "Nutrient Offset" (ex. Conservation Easements).
Step 3: Click "Yes" if this is a New Site/Project. Click "No" if this is an Existing Project. Example: If the
document being uploaded is a Prospectus you will click "Yes". If the document you are uploading is a
Monitoring Report, you will click "No". By clicking "No", this automatically prompts you to enter your
DWR ID #, which can be obtained by looking at the project's credit ledgers, Banking Instrument,
Conservation Easement, etc. If you do not know the number, please contact Katie Merritt or Mac Haupt.
Step 4 (a): If you Clicked "Yes" in Step 3, this implies that DWR has not received anything on this
project to review in the past and likely does not have a DWR ID#. Therefore, you will see the screen
below and you will be asked to enter enough information so that we can assign a DWR ID # to your
document/Project. When complete, skip down to Step 7.

Step 4 (b): If you Clicked "No" in Step 3, this implies that DWR has received something on this proiect
to review in the past and likely has a DWR ID#. Therefore, you will see the screen below and be
required to know and enter your DWR ID# and corresponding Version. The ID # is your 8 -digit DWR
Project # and is only numbers. Example #1: If DWR ID# is 2018-0001, you will enter all 8 digits into the
DWR ID# field without the dash. The Version # cannot be "0". Therefore, for this example, the Version
# is "1". Example #2: If the DWR ID# is 2018-0001v2, you will enter all 8 digits into the DWR ID# field
without the dash, but your Version# will be "2".

Step 5: Enter the Proiect Information associated with the document you are uploading.

Step 6: Enter the Document Information you are uploading. Using the Draw Down Menu arrow,
choose the Mitigation Document Type for the document being uploaded. Example: If you are uploading
a "Bank Parcel Development Package" or "Mitigation Plan", pick the item on the draw down menu for
"Mitigation Plan", they are one in the same.

Step 7: Upload the document. The document must be complete and include all attachments and
appendices. Only 1 PDF per form can be uploaded. Example: When uploading the document,
remember to include all supporting information (attachments, figures, photos, appendices, etc ) as one
document. Do not upload incomplete documents.
Step 8: Add your Electronic Signature

Step 9: Enter the Code Script located on the form to verify you are not a robot.

Step 10: Click Submit. If uploaded successfully, a message will pop up that the upload was successful. If
uploaded unsuccessfully, an error message will pop up and the form will need to be filled out again.
