A review has been made of the plans, specifications and design data submitted under cover letter
dated February 23, 2017 and received in this office on February 23, 2017 for the water line
replacement and realignment of the referenced high hazard dam. Prior to issuance of an approval
to repair, additional information or revision is requested for the following items:
1. Please provide stability analyses for the referenced dam along with soft copies.
2. Please provide detail H&H analyses along with soft copies.
3. Installation of atleast 3 survey monuments are required.
4. Please provide clarification for standards of grout mixture of being non -shrink product.
5. Drawing No :"ASV_C904.001.006" for typical trench detail (W.21) does not reflect/ show
dimensioning for depth of excavation.
It would greatly aid further application review if a response to each of the above comments were
made in a cover letter submitted with two sets of the revised plans, specifications and design
data
