inspection conducted at Pack's Garage on April 7,2026. As a result of the inspection,the site was
found to be noncompliant with the conditions outlined in the NC G 100000 permit.

Inspection Summary:
At the time of inspection, I met with the owner, Donald Pack. We began in the office on site,where Mr. Pack explained the
current nature of the business. He is essentially retired and only uses the shop to maintain his own vehicles. He
occasionally sells vehicles and parts from the stock of older vehicles he has outdoors. He claimed that he did not have the
paperwork for the permit available, but he has paid the permit fees.
The site was maintained with good grass cover around the perimeter,and all areas were stable with either grass or gravel.
There was very little erosion. There were a few empty drums and a couple of double-walled used oil containers outside the
shop. The outfall and the stream below had no visible signs of pollution at the time of inspection.
It is recommended Mr. Pack should apply for dormant status or rescind the permit since large scale industrial activities are
no longer performed on the property.All materials related to industrial activity would need to be removed prior to dormant
status or recission to be approved.Alternatively, if he eventually sells the business to someone else,they can have the
permit transferred to their name. Links for the dormant status application, rescission request,and transfer forms can be

If the permittee does not apply for dormant status, rescission or a transfer,a SWPPP would be required to be created and
analytical and qualitative monitoring should be completed on a quarterly basis.The outfall coordinates would also need to be
updated by contacting Brittany Cook at [e-mail], as the coordinates shown in the permit summary report
are incorrect. This facility will be reinspected on or after June 7,2026 to reassess compliance. If you have any questions,
