Please see the attached .PDF. My supervisor finally signed the forms and he is sending them in via snail
mail. There was also a link you sent me to an electronic form that I needed to fill out as well. Can you
send that link again, please? I believe there was an area where I was supposed to attach the scanned

forms that we are snail mailing. Please let me know if there is a step I am missing or something I forgot to
do. I will happily start in on it. Thanks!

Thanks for speaking with me on the phone a few days ago. I talked everything over with my supervisor
and relayed to him the information you gave me. He wants to be the "owner affiliation", instead of Steve
Heckle. I am re -doing the forms as such. We are going to snail mail all the forms in at the same time to
avoid confusion. It will be a few days before they come, but wanted you to know that we are working on
it. Hope you had a Happy Thanksgiving! Thanks again for your time and guidance.

Marcus Payton was my predecessor and no longer works for LKQ, therefore he cannot sign any forms
anymore. Steve Heckel is the boss of my boss. I am pretty sure he can sign that form. There aren't many
people higher in the company than him (only a couple). Please call me if you need to. We need to find a
rational path forward to getting me signed up as a certifier for the DMR reports without Marcus Payton.

Some people who received this message don't often get email from [e-mail]. Learn why this is important
Were recently received a Stormwater eReporting Registration Form from LKQ Corp. Steve Heckle was
Listed on the form as and signed as the Responsible Official. Mr. Heckle does not appear in our system
for any LKQ permits. Mr. Marcus Payton is listed as the Owner/ Responsible Official in our system. We
can not process the submittal since a responsible official (owner or person with delegated signature
authority) has not signed in Section E. To address this issue, you can submit anew Stormwater
eReporting Registration Form completed with and signed by Mr. Payton or, you can have Mr. Payton
complete and submit our Delegation of Signature Authority (DOSA) form to grant Mr. Heckle this
status. The DOSA process can be completed with an online submittal with our Permit Contact Update
Request Form.

Your submittal also included, in Section B, a listing of 7 permits with 7 different users listed in the
table. If you want these users to be granted Submitter rights, then you will have to also provide
completed details in Section C of the form for each of these users and include signed Section F for each
person. If you simply want to add them as users to the system without Submitter rights then you can
have one of your staff members with Facility Administrator rights add them to the eDMR system. We can
also help with this if needed but we will need more inforamtion.
