We hope that you and your families are doing well.
We were updating the Stormwater Pollution Prevention Plan (SWPPP) for our client Salem Leasing Corp.-
Durham (NCG080987) and determined that the lat/long location for the stormwater discharge outfall location
needs to be updated.
We also noticed that the COC for NCG080987 needs updating as the current 2021 COC has an incorrect
receiving stream.
When we were updating information around this time last year for eDMR we used information in the SWPPP
completed by a previous consultant.
During our current SWPPP update and after sampling this location for the past year we wanted to make sure
we have the most up to date information in the SWPPP and NCDEQ records.
We have two update requests:
1. Update the Lat/Long for Outfall 1:
Facility: Salem Leasing Corp. — Durham Permit COC: NCG080987 Stormwater
Discharge Outfall Location: Outfall 1
The correct stormwater discharge outfall location for Outfall 1 latitude and longitude should be as
follows: We have attached a map as well.
Latitude: 35.930056° (35° 55' 48.2")
Longitude: -78.8058330 (-780 48' 21.0")
The receiving stream information as updated last year and as noted in the email stream below is
correct.

2. Updated COC with correct receiving stream listed.
The COC update form online says it cannot be used until August 2025. See attached current COC from
Laserfiche with correct receiving stream noted at bottom.
We apologize for any inconvenience, and hope this is not an issue with the eDMR system, but we are doing
our best to have the best information available in the updated SWPPP and NCDEQ Stormwater records.
