o The person listed as the applicant does not match the signature and the title and position
information provided for CMH Homes. Please either revise so Hugh T. Statum is listed as
the applicant or provide the original signature and title and position information for Amie
Hacker. RESPONSE: The application page has been revised to reflect Mr. Statum's
title and position information.
• 1x hard copy of official documentation showing the title and position information for the signing
official, John C. Barnes [Barnes Land Corporation]. RESPONSE: Enclosed is a copy of the
Secretary of State report listing Mr. Barnes.
• 1x original, signed and notarized applicable O&M agreement(s)from the proposed pennittee,as
required for the permit.NOTE: This is still required,there is an option on the O&M agreement
for low density. RESPONSE: Original signed and notarized O&M agreement enclosed.
