original permit was issued to LECHASE CONSTRUCTION SERVICES, LLC, not Progress Carolina, LLC.
Instead, Progress Carolina, LLC must obtain a new COC for this project, and then LeChase should
submit a Notice of Termination form (and provide the new COC number).
