Lowery was not approved for the following reason(s):
Even if the project never started, you'll still need to close your E&SC plan with the Fayetteville Regional
Office. They will send someone out to confirm that the site is not under construction and they will
provide you with an inspection report, which you'll use as your close out document.
On the NOT Certification Form, the legally responsible organizational entity should be SR Diem and
Lowery, LLC and the legally responsible person should be Shawn Rainey. Either have Shawn Rainey sign
the NOT Certification Form and submit the eNOT, or a delegated individual from the SR Diem and
Lowery, LLC to print their name and title on the "Print Name and Title of Signed if Authorized" line of the
NOT Certification Form, sign the form using an original wet signature, and upload the updated NOT
Certification form.
