Explain the purpose of the proposed project (continue on back): To replace a 60+ year old log bridge with an aluminized corrugated arched culvert. Although the bridge is still fairly solid, some delivery truck drivers refuse to cross it with their trucks. 2b. Describe the overall project in detail, including the type of equipment to be used: The project will include the removal and disposal of the old bridge using an excavator (John Deere 60g) and a rubber -tired tractor backhoe (Kubota L35). As much as possible, all removal of bridge materials will be made from the existing roadbed on each side of the bridge. Every attempt will be made not to enter the water of the stream during demolition. Preparation for placing the arched culvert into the stream will include digging down 12 inches deep below the stream bed to fit the size of the width and length dimensions of the culvert. The culvert will then be lowered into place. Proposed culvert dimensions are 25 feet long and 11.5 feet wide in the flat bottom. Height dimension is 89 inches. Once the culvert is placed in the stream, backfill will begin with a layer of washed stone (gravel) at the base of each side, then a layer of fill dirt, then topped off with another layer of washed stone to cover. At the head (water entrance) and foot (water exit) of the culvert, reinforced prefabricated concrete barrier blocks (18"x24"x4' or 6') will be placed to serve as culvert wing headwalls. Then riprap rock will be used to reinforce the culvert wing headwalls and key into the stream banks. C. Stream Impacts: Please list all proposed impacts to the stream 1 a.
