On September 27, 2023, a site inspection was conducted for the Cascades Moulded Pulp facility located
at 112 Cascades Way, Rockingham, Richmond County, North Carolina. A copy of the Compliance
Inspection Report is enclosed for your review. Your time and assistance was greatly appreciated. The site
visit and file review revealed that the subject facility is covered by NPDES Stormwater General Permit
NCGO50000 under Certificate of Coverage NCG050286. Permit coverage authorizes the discharge of
stormwater from the facility to receiving waters designated as Falling Creek(Hinson Lake,Great Falls Pond),
a class C waters in the Yadkin- Pee Dee River Basin.
As a result of the inspection, the facility was found to be compliant with the conditions of the NCG050000
permit. Please refer to the enclosed Compliance Inspection Report for additional comments and
observations made during the inspection.
Please be advised that violations of the NPDES Stormwater General Permit are subject to a civil
penalty assessment of up to$25,000 per day for each violation. If you or your staff have any questions,

Inspection Summary:
Melissa Joyner and Kelly Jonas met with Jamie Mendoza, Plant Manager of the Cascades Moulded Pulp facility. The
Stormwater Pollution Prevention Plan (SWPPP)was reviewed and appeared to be mostly in accordance with the conditions
of NPDES General Permit NCG050000. Since Mr. Mendoza is the new Plant Manager a Permit Contact Update Request
Form will need to be completed and submitted on-line. The Site Map should be updated by including a north arrow,
topography, labeling for the drains, a delineation of each drainage area of the outfalls and a table of impervious percentage
for each drainage area.The Spill Response Team data should be updated. The facility did not have solvents, therefore this
information must be certified in the SWPPP. Documentation of annual employee training will need to be included in the
SWPPP. The facility inspections will need to be conducted on a quarterly basis per conditions of the new General Permit
NCG050000 (effective June 1, 2023). Analytical and Qualitativie Monitoring will also need to be conducted on a quarterly
basis. Discharge Monitoring Reports (DMR's)will need to uploaded electronically within 30 days of conducting Analytical

The facility grounds were inspected and appeared orderly.All Above Ground Storage Tanks are located inside of the facility.
An on-site rain gauge will need to be installed. Ph will need to be recorded with a calibrated Ph meter within 15 minutes of
collecting the water sample.
