Wilson and Chuck Cranford, from the Asheville Regional Office of the Division of Water Quality (DWQ), conducted a site inspection for the stormwater controls at Mills River Elementary School. Some issues were noted during the inspection and file review and are outlined in the attached inspection report. Please contact this office in writing after the issues have been reviewed and/or resolved (this should be done by December 10, 2010). The original Operation and Maintenance agreement was signed by Mr. Bo Caldwell and has been attached for your review. DWQ reminds you that the stormwater controls are to be inspected, at minimum, once a month and within 24 hours after every storm event greater than 1.0 inches. Records for inspection and maintenance should be kept in a known/set location and must be available upon request. DWQ recommends that the eroded inlet to the forebay be fixed as soon as possible. The School's original consulting engineer/consulting firm has also been copied with this letter. The Engineer's Certification for the subject permit has not been received and should be submitted to this office. As noted in the inspection report, a level spreader appears on the approved plans but could not be located during the inspection. A level spreader was shown on the approved plans - but was not found on the site. Please review this situation with your design engineers. The inlet to the forebay of the basin appears severely eroded (and since it is outside of the fence, may cause a safety hazard). At this time, there does not appear to be any resultant water quality problems at the site. However, if the stormwater BMP is not properly ma�tained, problems may occur in the future. SW Measures Yes No NA NE Are the SW measures constructed as per the approved plans? ❑ ■ ❑ ❑ Are the inlets located per the approved plans? ❑ ❑ ❑ Are the outlet structures located per the approved plans? ❑ ❑ ❑ Comment: Although the approved plans indicate a level spreader at the outlet - it was not found during inspection. Comment: The inlet (near the fence) is significantly eroded. Since part Of it is outside of the fence - it could pose a significant safety hazard. It is also likely causing more sediment to be contributed to the forebay area, which may compromise the efficiency of the forebay. The school should consider trimming the vegetation (possibly 2/year) so that the inlet and outlet can be observed during inspection. Some areas outside of the fenced area (between the parking area and the basin) should be re -seeded to prevent erosion in the future. If cattails are not removed periodically - they will take over the basin and compromise efficiency. Other Permit Conditions Is the site compliant with other conditions of the permit? Vee M. Although minor issues were noted with the stormwater BMP, it appeared to be working adequately. Page:4
