required by Title 15A NCAC 4B .0129.
As of April 1. 2019. all new construction AWNitics are required to Mete and submit an electronic
Notice of Intent teNoll form requesting s Certificate of Coverage (COC) under the NC601(K)40
Construction General Permit. After the foam is reviewed and found to be cornplete, you will receive a
link with p trmrmt instructions for the S 100 annual perrnit fee. Aber the fee is processed. you %& ill
receive the COC via email. As tie Financially Responsible Pasty shown on the FRO form submitted for
this project, you MUST obtain the COC prior to cm=ence-ment (if any land disturbing
