Good afternoon,
Unfortunately, I processed the check yesterday and since this was a physical check, it is being processed by the
bank. You should know that payments made out to DEMLR do not offer refunds. If your annual fee for NCC215547 was
not already paid, there could have been a possibility to have the check cover the annual fee. Currently, I do not know of
any other options at this time.

Mr. Moctezuma (Tony),
Hi. I understand. It sounds as though you need to submit an eNOT request to rescind the incorrect COC. Since this will
not be a typical termination request, please put a note in the comments box that explains that the COC was issued in
error and so there is no erosion/sedimentation closeout inspection report. Please still download, complete, wet sign,
scan and upload the NOT Certification Form. If the check has already been processed, it is difficult to issue refunds, so
the funds may be able to be applied to other fees.
I'll let either Tev or Bethany comment further on the billing situation. Thx.

Mr. Clark,
The error is that there are now two COC numbers for the same project.

Mr. Moctezuma (Tony),
Hi. Thanks for the message. If the error does not involve an ownership change, you could submit a
modification request. Please see the following and contact us with questions:
How to Modify a Construction Permit File
Complete the Modification form. Notes:
1. Use this form to add updated E&SC Plan approval and close-out letters to the file
for projects that already have an approved COC.
2. The Permittee or appropriately authorized individual within the Permittee
organization must sign and submit the modification request. A modification
request by a consultant or someone outside of the Permittee organization will not
be accepted.
3. This form may also be used for changes to Legally Responsible Individual, but
not Entity.
4. This form may NOT be used to change project ownership. The new owner
must submit an e-NOI form and the previous owner must submit an e-NOT form.

Good afternoon,
Lasherfiche just issued the certificate of coverage for this project, but it is still wrong. Would you be able
to cancel this and send the check back?

Good morning,
I had just mailed it in on Friday. I haven't received any confirmation that it has been
logged in yet. So it might not have been processed yet.

Good morning,
I recently sent in an NOI for Peytons Ridge, Section IX and got the COC
number NCC223884, but I made a mistake with this permit. There was
already a COC number, NCC215547, and I was supposed to send in a
modification instead of applying for a new permit.
What is the best way to resolve this problem?
Thanks,
