Dear Mr. McGee:
In reference to the phone conversation held on Thursday, August 25, 2022 I'm
submitting the modified pages of IUP Number 000022. These changes were
made due to a letter I received on December 12, 2019 from Ms. Vivien Zhong of
the PERCS unit. As you stated, there were no records showing the submittal of
this permit with these modifications. Wyeth, LLC is operating under this modified
IUP currently.
These modifications were submitted prior to the deadline specified in Ms.
Zhong's letter. During our conversation, you deemed that with this duplicate sub-
mission with the modifications completed as requested in the afore mentioned
letter would be acceptable. This letter will serve as an approval as such. I have
included a copy of the letter from Ms. Zhong, the industry transmittal letter, and
the modified permit pages.

The Pretreatment, Emergency Response and Collection System (PERCS) Unit of the Division of Water
Resources has reviewed the Industrial User Pretreatment Permit(IUP) Renewals submitted by the City of
Sanford for the following Significant Industrial Users (SIU). This IUPs were received by the Division on
September 26,2019.Additional information was requested and was received on November 4 2019 and
December 9,2019.

The review indicates that with the following comments,the IUPs are adequate and meet the minimum
requirement of 15A NCAC 2H.0905 and.0916 and 40 CFR 403.8(f)(1)(iii). The allocation table,maintained
in the Division's database,has been updated accordingly.
General:
1. Application form: It was noted that the new application form (2012)was used during the 2014
renewal but the old form(1997)was used this time. It is recommended to have applicants to use the
attached new form in the future.

2. General condition#10:Please update the Department reference to"Department of Environmental
Quality"the next time the IUPs are modified or renewed_ It was left unaddressed from previous
review letters.
3. Special condition certified laboratory analysis: Please update the Division reference to"Division of
Water Resources"the next time the following IUPs are modified or renewed:ASC/Trion, Hydrotube,
Tyson.
4. IUP synopsis:Please submi
t a copyof all the IUP synopsis to PERCS file.
5. Allocation table:The submission used the headworks analysis that was approved in 2014, please see
attached latest allocation table with corrected IUP limits highlighted and start using this version for
future modification and renewals.Silver is over allocated.2019 DMR data has shown that the city's
silver influent and effluent has always been below the practical quantitative level of 1 ug/L,thus
over allocation on paper is acceptable. it is recommended to lower the IUP silver limits and keep a
close look of the city's influent silver levels.
Caterpillar Inc.:
1. Please submit a copy of the inspection form to PERCS file once it is available.
2. Pretreatment Units: Filtration listed in the IUP is not checked on the page 7 of the application. It is
assumed the IUP listing is correct. Please have the SlU revise the application and advise the PERCS
unit with any needed IUP modification.
3. Sampling Point Diagram: The diagram in the current IUP was not clear as where the sampling points
are. Please modify the IUP to add labels to the diagram to specify the locations of the sampling
points.
Caterpillar Paint&Plating Operations
1. Pretreatment Units: Filtration,sedimentation,clarifier,filter press,grease trap and oil water
separator listed in the IUP were not checked on the page 7 of the application. Please see Caterpillar
Inc. comment#2.
2. Description of Discharge:Steam boiler blow down and air compressor/dryer condensate included in
pipe#01 in the IUP were not indicated in the wastewater survey on the page 5 of the application.
please clarify this with either a modified IUP or a corrected application.
Charah:
1. Sampling Point Diagram: Please include a diagram specifying the sampling point.
2. Pollutant Checklist:The wastewater pollutant checklist in the permit application was not filled out
by the SIU to indicate whether the pollutants of concern are present Or absent at the facility or in
the discharge. Please have the S1U complete the Wastewater Pollutant checklist based on their
historical data and submit a copy for our files. For any POCs indicated to be present at the facility or
in the discharge, please address them as needed in the IUP and/or the IUP synopsis.
3. Startup conditions:Please remove the startup conditions the next time the IUP is modified or
renewed if they do not apply any longer.
Coty:
1. Pretreatment Units:Grease trap checked on page 8 of the application was not listed in the IUP.
Clarifier,mixing tank,aerated storage tank,sludge storage and unit sludge filter listed in the IUP
were not checked on page 8 of the application. Please see Caterpillar inc.comment#2.
Megneti Marelli:
1. Categorical Industrial User: It was indicated on the cover page and IUP history that the SIU has been
permitted under 40 CFR 433.However,on page 2,7 and 8,433,463 and 464 was listed as the
categorical information. Please clarify and revise those three pages or modify the permit limits
before January 31,2020.
2. Pretreatment Units:Filtration,grease trap,clarifier and filter press listed in the IUP were not
included on the page 9 of the application.Please see Caterpillar Inc.comment#2.
Moen:
1. Pretreatment Units:Cyanide destruction listed in the IUP was not included on the page 8 and 9 of
the application. Non-aerated equalization tank was listed to be 0.05 million gallons while 0.02
million gallons in the application_ Please see Caterpillar Inc.comment#2.
2. Sample collection method: Phenol should be grab sample. Please revise and submit the permit
modification before January 31,2020.
STI Polymer:
1. Pretreatment Units: Equalization,flocculation,screening and spill protection listed in the IUP were
not included on page 7 of the application.Please see Caterpillar Inc.comment#2.
2. Wastewater survey:On page 6 of the application, it was indicated that domestic water goes through
the pretreatment unit. It is assumed that the SIU had a typo in the application. Please have the SW
revise the application or modify the permit with a combined wastestream formula.
3. Pollutant Checklist: Barium was checked present in the discharge with an unknown amount. Please
contact the SIU about the level of Barium and determine whether Barium would be a concern for
the WWTP.
Tyson:
1. Inspection form: Please submit a copy of the inspection form to PERCS file once it is available.
2. Pretreatment Units: pH adjustment listed in the IUP was not included on page 7 of the application.
Please see Caterpillar Inc.comment#2.
3. Expiration date:The expiration date on page 5 of the IUP was incorrect.Please revise and submit the
modified permit before January 31,2019.
Wyeth:
1. Pretreatment Units: Biological treatment and spill prevention listed in the IUP were checked as no on
page 20 of the application. Please see Caterpillar Inc.comment#2.
2. Sampling location diagram: Please mark the sampling points on the diagram and resubmit the next
time the IUP is modified or renewed.
3. Categorical limits:Acetone has a monthly average limit of 8.2 mg/L but was listed as monitoring only
on the limit page. Listing all the required parameters on the limit page instead of stating "40 CFR
439.16 PPES" would prevent such conflict from happening. It was also noted from page 15 of the
application that chloroform was detected thus not eligible for the monitoring waiver. Please make
sure to include such language in the special conditions. Special conditions were not included in the
submission,please incorporate any other applicable changes and resubmit it before January 31,2019.
Federal and State pretreatment regulations require the local delegated pretreatment program to
effectively control and document the discharge of wastewater from Significant/Categorical Industrial
Users to the POTW. It is the POTW's responsibility to ensure that these objectives are consistently met.

the effluent limitations, monitoring requirements, and all other conditions set
forth in this Industrial User Pretreatment Permit(IUP)into the sewer collection
system and wastewater treatment facility of the Control Authority and/or
Municipality.
2.) The Pennittee is hereby authorized to continue operation of and discharge
