The purpose of this project is for the construction of a new 19,548 of Education Center for the First Baptist Church of Swainsboro. Building design is by Smithson, Inc., (252.977.3055). 2. Location and topographical information was taken from an actual field survey performed by Appian Consulting Engineers, PA dated August 2007. Reference Datum is MSL. 3. Boundary information is shown for reference only. Consult Professional Land Surveyor for boundary data. 4. This church property is currently comprised of four separate tracts owned by the church that will be recombined in the future. The tracts are: DO 350, PG 349; DO 204 PG 329; DB 577 PG 213; DO 1759 PG 770. 5. All materials, construction, workmanship shall meet the Town of Swansboro Standards and Onslow Water and Sewer Authority (ONWASA) specifications. 6. Contractor shall comply with all pertinent provisions of the 'Manual of Accident Prevention in Construction' issued by AGC of America, Inc., and the Safely FIRST BAPTIST CHUR( and Health Regulations for Construction issued by the U.S. Department of Labor. 7. Existing utility locations shown are approximate and shall be verified in the field prior to beginning work. Contractor shall contact NC One Call Center at 1-800-632-4949 for locations of existing utilities prior to beginning construction. 8. Owner is responsible for determination or discovery of existing underground storage tanks (UST), septic tanks, abandoned utility Lines, graves or cemeteries that may exist on the site. 9. Storm drainage pipe is to be Class III reinforced concrete meeting ASTM C-76, latest revision. 10. Contractor is to bring building pad to within 0.1 foot of the subgrade level. See bidding notes, specs, or sods report for clarification of potential surcharge area. 11. Maintain a minimum exterior grade drop of 8 inches below the finished floor elevation. Do not place landscaping fill against the foundation in such a way that the outside grade elevation is raised higher than 8 inches below the finished floor elevation. 12. Building Contractor shall be responsible to verify the minimum building setback requirements with the Town building inspections office before construction. 13. No additional signage is proposed at this time. EDUCATION CENTER Environmental 14. This Stormwater Management Plan has been approved by NCDENR DWQ, Stormwater Permit No. SW84)80615, dated 6.27,08. This high density permit is good for 10 years and Men must be renewed. A permit renewal simply require the stormwater to be treated in the same manner as the original permit meeting all the same criteria. Owner has agreed to an Operation and Mainte nance Plan, including inspections and records of maintenance. 15. Upon completion of construction of storm drainage features lie, sand filter), prior to issuance of a Certificate of Occupancy and prior to operation of this facility, the Engineer to issue a written certification that this system has been installed according to these plans. Any deviations from the approved plans may require repair or modification of Stormwater Permit 16. This Erosion Control Plan was approved by NCDENR Land Quality, (Onsio-2008-340), dated 6.23.06. It is imperative Chat Contractor follow construction sequence as shown. 17. All disturbed areas shall be landscaped and seeded within 15 days of completion of oonstnx;tion. 18. Any additional erosion control items that are installed that are not shown on the plans, shall be approved by the local agency or the Engineer before installing in order to approve for payment. items not approved will be installed at the contractors expense. 19. Contractor to remove sedimentation from stormwater system and to restore system to design condition prior to operation as a slonrnvater treatment device and prior to occupancy of the facility. 20. Property is not located in a FEMA mapped flood area or Flood Hazard zone, per FEMA Flood Panel 3720.5364.00K, dated November 3, 2005. 21. The existing ditch on the eastern property line does not appear on the USGS or soils maps and is not protected by riparian buffers. However, there is a 10' buffer strip per the Town of Swansboro along the eastern property line. Do riot disturb existing vegetation in this area except as noted. 22. When clearing right-of-way or easements, watch for tall young slender unbraced pine trees that oc; d fall into the rightof-way after construction is complete. Bring to the atentlon of Engineer for resolution during clearing operation. 23. Unless noted otherwise, all trees not marked to be removed are not to be disturbed. They are to be ,xotected from damage by equipment during construction. 24. All construction debris shall be disposed of oft -site in a state permitted landfill. 25. The location of the topsoil stockpile shall be coordinated with Owner. ADA 26. Sitework handicap signage, surfaces, grades, pavement markings, etc., to comply with ADA requirements. 27. All handicap ramps are to meet 'ADA Accessibility Guidelines for Buildings and Facilities' as detailed in Federal Register, Vol 56, No. 144 dated July 26, 1991, rules and regulations activated January 26, 1992. also refer to North Carolina State Building Code Volume 1-C, 'Making Buildings and Facilities Accessible To and Useable by the Physically Handicapped", latest edition. Unities 28. Proposed 6' fmreline to be private and maintained by the Owner within the propertyline. Within the public right-of-way, proposed watermain to be maintained by ONWASA. 29. Watermain to be constructed according to ONWASA Water System Specifications. 30. Domestic water to be 2' PE 3408, SDR 9, 160 psi working pressure. 6' fireline to be PVC C-900, Class 150. A piping at baddlow prevention boxes and vaults to be Ductile Iron Pipe (DIP). Minimum bury is 36' of cover. 31. Tracer tapes: 2-Inch wide blue warning tape marked 'Caution Water Main Below' shad be installed in the ditch 2 feet over all PVC watermains. 32. Tracer wire: 10-guage bare copper wire shall be installed along the top of all nonmetallic pipe. The wire shell be continuous and urintertupled. A sufficient excess length of wire shall be left in each valve box to provide at least 2 feet of length above finished yiade. 33. 2' PE water service joints to be compression joints with solid stainless steel inserts. 6' C-900 firelin• joints to be push on oT but fused. Al DIP joints to be mechanical joint. PVC glued or threaded pipe or fittings are not permitted. 34. Contractor to contact ONWASA (Onslow Water and Sewer Authority) at least 48 hrs prior to commencing construction on the water or sewedines, 910.455.0722. 35. On waterlines, 'Megalug' joint restraints may be used in lieu of concrete blocking at fittings normall;, ,equired to have concrete blocking. Verify acceptability with Municipality before employing for approval. 36. Sewer service to be PVC SDR-35 smoothwall, confirming to ASTM D-3034 Type PSM, SDR-35, or asspecified by Engineer. 37. Contractor to notify Engineer, the Town and ONWASA at least 72 hours in advance of water and sewer lasts and have tests witnessed. 38. Site contractor is to assume responsibility for all water and sewer utilities from a point 5' outside of building to point of public connection. 39. AN ske plumbing is to meet the NC State Building Code Volume 11, Plumbing. 40. All fire hydrants shall be set to height per local municipality. Adjusting fire hydrants to grade is considered incidental to the cost of the fire hydrant. 41. Existing or new operable Manholes or Wetwells qualify as 'confined' and require compliance with OSHA 'Confined Access Entry' requirements. Certified equipment, proper notification and other applicable equipment and/or devices may be necessary to protect workers, after system is operational from hydrogen -sulfide gas build-up or an otherwise oxygen -less environment. 42. Contractor is required to provide'as-built drawings for Engineer upon completion of construction. Note any deviations from the original place. Plans are to reflect the actual location, materials and depth of facility along with valve tie -downs. Failure to provide as -built drawings may delay project and require at contractor's expense, a filed survey by a PLS. 43. The cost for power pole relocation is to be bome by the Developer. Contractor is to coordinate relocation with the Department of Utilities. 44. Minimum bury on underground electric is to be 48'. Minimum bury on Cable TV is to be 24'. Minimum bury on Telephone is to be 36'. 45. The contractor shall be responsible for the repair of any pavement or existing utilities that may be damaged due to construction activity. Exercise caution. 46. Any relocation of existing electrical facilities will be at the developers expense. 47. Contractor shall exercise extreme caution when working under, around, and/or adjacent to existing power lines. 48. Electric power is to be placed underground, and shall be installed, if possible, before curb, pavement, and seeding of adjacent area. Coordinate with utility agency. Do not seed and mulch behind curb until underground electric has been installed. Coordination of electrical plaoartheM shall be the sale responsibility of the Contractor. Concrete 49. All concrete is to be minimum 3000 psi at 28 days, air entrained except dumpster pad to be 4500 psi. unless otherwise rated on the plans. 50. Do not pour any concrete before forms are inspected and approved by the project engineer. Failure to do so may constitute removal of work performed at Contractors cost until made satisfactory by the Engineer. 51. All curb and gutter returns shall have a 20' radius, unless shown otherwise. Provide handicap ramps at ail returns. 52. Contractor shall watch for any potential upgrade depressions adjacent to and behind proposed curb. Should any be detected, fill to provide positive surface drainage over top of curb. 53. All curb and gutter within NCDOT right-of-way shall be 30' standard curb and gutter. Traffic 54. All signs, pavement markings, and other traffic control devices shall conform to the Manual on Uniform Traffic Control Devices, latest edition as amended, and are the Contractors responsibility. 55. The contractor shall be responsible for all work zone traffic control in or adjacent to NCDOT or City right-of-way. All methods shall contour to the Manual on Uniform Traffic Control Devices (MUTCD) and NCDOT standards. 56. Work within the NCDOT right-of-way shall conform to NCDOT standards and specifications. 57. Contractor shall saw -cut to provide smooth transition at tie-in to existing edge of pavement where applicable. 58. Contractor will be responsible for providing all pavement markings in his bid. GENERAL NOTES 1. Location and topographical information was taken from an actual field survey performed by Appian Consulting Engineers, PA dated August 2007. Reference Datum is MSL. 2. Storm drainage pipe if shown, is to be Class III reinforced concrete meeting ASTM C-76, latest revision. 3. All materials, construction, workmanship shall meet the Town of Swansboro Manual of Specifications, Standards and Design as well as Onslow County Water/Sewer Authority. 4. All signs, pavement markings, and other traffic control devices shall conform to the Manual on Uniform Traffic Control Devices, latest edition as amended, and are the Contractor's responsibility. 6. The contractor shall be responsible for all work zone traffic control in or adjacent to NCDOT or City right-of-way. All methods shall conform to the Manual on Uniform Traffic Control Devices (MUTCD) and NCDOT standards. 7. Work within the NCDOT right-of-way shall conform to NCDOT standards and specifications. 8. Contractor shall saw -cut to provide smooth transition at tie-in to existing edge of pavement where applicable. 9. Contractor shall comply with all pertinent provisions of the "Manual of Accident Prevention in Construction" issued by AGC of America, Inc., and the Safety and Health Regulations for Construction issued by the U.S. Department of Labor. 10. Existing utility locations shown, if any, are approximate and shall be verified in the field prior to beginning work. Contractor shall contact NC One Cali Center of 1-800-632-4949 for locations of existing utilities prior to beginning construction. It. All disturbed areas shall be landscaped and seeded within 15 days of completion of construction. 12. On waterlines, "Megolug" joint restraints may be used in lieu of concrete blocking at fittings normally required to have concrete blocking. Verify acceptability with Municipality before employing for approval. 13. Regulatory signs (i.e. STOP, etc.) and street name signs shall be manufactured from high intensity reflective materials. Street name markers shall be provided per local municipality or NCDOT requirements. 14. Lot dimensions shown are approximate. 15. When clearing right-of-way or easements, watch for tall young slender unbraced pine trees that could fall Into the right-of-way after construction is complete. Bring to the attention of Engineer for resolution during clearing operation. 16. Any relocation of existing electrical facilities will be at the developer's expense. 17. All fin hydrants shall be set to height per local municipality. Adjusting fire hydrants to grade is considered incidental to the cost of the fire hydrant. 18. Property is not located in a FEMA mapped flood area or Flood Hazard zone. 19. Owner Is responsible for determination or discovery of existing underground storage tanks (UST), septic tanks, abandoned utility Imes, groves or cemeteries that may exist on the site. 20. Contractor shall exercise extreme caution when working under, around, and/or adjacent to existing power lines. 21. Sitework handicap signage, surfaces, grades, pavement markings, etc., to comply with ADA requirements. 22. Contractor will be responsible for providing all pavement markings in his bid. All lines are to be 4" white unless otherwise noted. 23. Site contractor is to assume responsibility for all water and sewer utilities from a point 5' outside of building to point of public connection. 24. All site plumbing is to moot the NC State Building Code Volume II, Plumbing. 25. Do not pour any concrete before forms are inspected and approved by the project engineer. Failure to do so may constitute removal of work performed at Contractor's cost until mode satisfactory by the Engineer. 26. Site disturbance is more than 1 acre therefore an erosion control permit has been applied for. No work shall begin on site until an approved copy of the erosion control permit has been obtained from the review agency. All additional erosion control Items that are Installed that are not shown on the plans, shall be approved by the local agency or the Engineer before installing in order to approve for payment. Items not approved will be installed at the contractor's expense. 27. All curb and gutter returns shall have a 20' radius, unless shown otherwise. Provide handicap ramps at all returns. 28. Contractor shall watch for any potential upgrade depressions adjacent to and behind proposed curb. Should any be detected, fill to provide positive surface drainage over top of curb. 29. All curb and gutter within NCDOT right-of-way shall be 30" standard curb and gutter. 30. Electric power is to be placed underground, and shall be installed, If possible, before curb, pavement, and seeding of adjacent area. Coordinate with utility agency. Do not seed and mulch behind curb until underground electric has been installed. Coordination of electrical placement shall be the sole responsibility of the Contractor. 31. Existing or new operable Manholes or Wetwells qualify as "confined" and require compliance with OSHA "Confined Access Entry" requirements. Certified equipment, proper notification and other applicable equipment and/or devices may be necessary to protect workers, after system is operational from hydrogen -sulfide gas build-up or an otherwise oxygen -less environment. 32. All handicap romps are to most "ADA Accessibility Guidelines for Buildings and Facilities" as detailed in Federal Register, Vol 56, No. 144 dated July 26, 1991, rules and regulations activated January 26, 1992. also refer to North Carolina State Building Code Volume 1-C, "Making Buildings and Facilities Acces-ib!e T9 and Useable by the Physically Handicapped", latest edition. 33. The contractor shall be responsible for remediating and correcting all conflicts created by him, at his own expense, which may be at variance with the Approved Construction Drawings. 38. Building Contractor shall be responsible to verify the minimum building setback requirements with the building inspections office before construction. 39. All construction debris shall be disposed of off -site in a state permitted landfill. 40. Owner to be responsible for sign permit and sign placement for new building as required or needed. 41. The cost for power pole relocation is to be borne by the Developer. Contractor Is to coordinate relocation with the Department of Utilities. 42.
