WORK SHALL COMPLY WITH ALL APPLICABLE LAWS, CODES, JURISDICTION REQUIREMENTS AND ORDINANCES. 2) MATERIALS: PAVING/PATCHING SHALL MEET DOT STANDARDS FOR S 9.5 B ASPHALT AND CABC, AND BE MEASURED AFTER COMPACTION. CABC WILL BE COMPACTED TO 100%+ MINIMUM. CONCRETE SHALL BE 3000 PSI UNLESS NOTED OTHERWISE. 3) GRADING AND TYPICAL SECTION: GRADE INTERSECTIONS FROM 1-5 % DO NOT EXCEED 5%. FOR THE FIRST 100 FEET. OTHER STREET GRADES NOT TO EXCEED 10 %. GRADE ROADS TO TYPICAL SECTION. EXTEND BEYOND THE R/W TO MAINTAIN THE BACK SLOPE OR FILL SECTION. WITHIN THE PROPERTY THE CROSS SECTION HAS PRIORITY OVER CONSTRUCTION LIMITS EXCEPT IN AREAS OF WETLANDS OR OTHER PROTECTED AREAS. ADJUST SILT FENCE, TEMPORARY SWALES AND OTHER SOIL EROSION MEASURES AS NEEDED. WHERE A DITCH IS CALLED TO BE EXTENDED TO DAY LIGHT THE CONTRACTOR WILL SLOPE THE SIDES TO A MAXIMUM OF 3:1 OR AS NOTED OTHERWISE ON THE PLANS. IN C & G SECTIONS, THE MINIMUM ASPHALT SLOPE DRAINING ACROSS THE INTERSECTION WILL BE A MIN OF 1% FROM GUTTER TO GUTTER OR USE 4' x 6" CONCRETE VALLEY SECTION. 4) CLEARING: CLEAR 5 FT BEYOND CUT OR FILL LINE MINIMUM OR TO THE R/W - WHICHEVER IS GREATER. 5) EARTHWORK AND/OR UNSUITABLE BACK FILL: WHEN UNSUITABLE MATERIALS ARE FOUND THE CONTRACTOR WILL NOTIFY THE ENGINEER IN WRITING. CONTRACTOR TO REMOVE UNSUITABLE MATERIAL TO 12" MIN. UNSUITABLE SOILS WILL BE USED IN EMBANKMENT CONSTRUCTION. FILL MATERIAL WILL BE FREE OF ROOTS, DEBRIS, ROCKS LARGER THAN 2" AND ICE. FILL MATERIAL TO BE PLACED IN 6" TO 9" LIFTS AND COMPACTED TO 95% STANDARD PROCTOR MINIMUM OR AS OTHERWISE NOTED. NOTE: THE CONTRACTOR WILL GRADE AND ADJUST ELEVATIONS AS NEEDED TO BALANCE THE SITE. BALANCING SHALL BE INCLUDED IN THE BASE BID. EXCAVATION IS UNCLASSIFIED UNLESS AGREED TO BY OWNER IN WRITING. APPROPRIATE DOCUMENTS COVERING CHANGES MUST BE EXECUTED BY BOTH PARTIES TO BE VALID. 6) MINIMUM COVER AND CLEARANCE: A MINIMUM COVER OF 36" IS REQUIRED ON ALL PVC PIPE. A 24" CLEARANCE WITH ANY STORM DRAIN IS REQUIRED FOR CITY AND DOT STREETS. USE DIP IF THESE REQUIREMENTS CANNOT BE MET. 7) EXISTING UTILITIES: THE CONTRACTOR IS RESPONSIBLE FOR LOCATING ALL UTILITIES IN THE PROJECT AREA. AT A MINIMUM THE CONTRACTOR SHALL CONTACT THE NORTH CAROLINA ONE CALL CENTER (NCOCC) AT 1-800-632-4949 FOR UTILITY LOCATION PRIOR TO CONSTRUCTION. IN ADDITION, SHOULD OTHER UTILITIES PUBLIC OR PRIVATE BE OBSERVED IN THE AREA, THE CONTRACTOR WILL CONTACT THE APPROPRIATE UTILITY COMPANY OR OWNER FOR LOCATION. THE CONTRACTOR SHALL NOTIFY THE INSPECTOR OR ENGINEER OF ANY OBVIOUS CONFLICTS PRIOR TO CONSTRUCTION AND ANY NOT FOR SEEN DURING CONSTRUCTION. 8 WORK PLACE PROCEDURES AND SAFETY: THE ENGINEER SHALL HAVE NO CONTROL OVER NOR RESPONSIBILITY FOR THE CONTRACTOR'S MEANS METHODS SEQUENCE, TECHNIQUES OR PROCEDURES IN PERFORMING THE WORK SITE SAFETY OR SAFETY PROGRAMS IN CONNECTION WITH THE WORK. THESE ARE SOLELY THE RESPONSIBILITY OF THE CONTRACTOR, WHO IS ALSO RESPONSIBLE FOR COMPLYING WITH ALL HEALTH AND SAFETY PRECAUTIONS AS REQUIRED BY REGULATORY AGENCIES. WHEN NEEDED, TRENCH SHORING OR BRACING WILL BE DESIGNED BY A PROFESSIONAL ENGINEER. 9) DRIVEWAY AND ROAD CROSSINGS: THE CONTRACTOR SHALL SAW CUT ALL ASPHALT AND CONCRETE CROSSING. REPLACEMENT MATERIALS ARE TO MATCH. REPLACEMENT THICKNESS ARE EXISTING DEPTH +_ 1". CABC BASE MATERIAL SHALL BE INCLUDED IN THE COST OF THE ASPHALT CONCRETE. 10) GRASSING AND SEEDING: THE CONTRACTOR SHALL BE RESPONSIBLE FOR CLEANUP AND SEEDING AS PER THE APPROVED PLAN. THE CONTRACTOR SHALL FURNISH ALL LABOR, EQUIPMENT LIME, FERTILIZER AND SEED NECESSARY TO COMPLETE IN ACCORDANCE WITH THE SEEDING SCHEDULE. ALL ROOTS, STONE, AND DEBRIS SHALL BE REMOVED AND OVERALL SHAPING OF THE AREA TO LEVEL OUT HIGH AND LOW SPOTS SHALL BE DONE BY THE CONTRACTOR. THE CONTRACTOR SHALL MAINTAIN THE GRASSED AREA FOR A PERIOD OF THREE (3) MONTHS AFTER THE GRASS GROWTH APPEARS AND SHALL RE -SEED BARE SPOTS AND REPAIR ANY AND ALL ERODED AREAS DURING THIS TIME. REPAIRED AREAS SHALL BE MAINTAINED UNTIL GRASS IS ESTABLISHED. 11) SOIL EROSION CONTROL: ONCE THE CONTRACTOR STARTS WORK ALL PROVISIONS OF RULES AND REGULATIONS FOR EROSION CONTROL AND SEDIMENT CONTROL" OF THE STATE OF NORTH CAROLINA DEPARTMENT OF NATURAL RESOURCES AND COMMUNITY DEVELOPMENT SHALL BE COMPLIED WITH WHEN APPLICABLE. THE CONTRACTOR SHALL CONSTRUCT TEMPORARY EROSION CONTROL MEASURES CONSISTING OF BERMS, SILT FENCES, SWALES, AND BASINS AS NECESSARY TO CONTROL EROSION. PHASING OF THE CLEARING, GRADING, TRENCHING AND PIPING OPERATIONS SHALL BE CLOSELY COORDINATED WITH THESE TEMPORARY CONTROL MEASURES. INSTALL STORM DRAIN PIPES PRIOR TO FILTER BASINS WHEN FILTER BASINS DISCHARGE INTO THE PIPE INLET. IMMEDIATELY FOLLOWING CONSTRUCTION OF FINAL SLOPES, FILLS, BACK FILLS AND GRADING, THE CONTRACTOR SHALL BEGIN PERMANENT TYPE EROSION CONTROL MEASURES. THESE MEASURES PRIMARILY CONSIST OF SEEDING, FERTILIZING AND MULCHING. UPON SATISFACTORY STABILIZATION OF ALL AREAS, THE CONTRACTOR SHALL REMOVE THE TEMPORARY EROSION CONTROL MEASURES. ONCE THE PROJECT BEGINS, THE CONTRACTOR IS RESPONSIBLE FOR COMPLIANCE UNTIL THE PROJECT IS COMPLETED UNLESS THE PROBLEM ITEMS FALL UNDER THE ONE YEAR WARRANTY, 12) REPLACEMENT: THE CONTRACTOR SHALL REPLACE CULVERTS, AND LANDSCAPING INCLUDING SHRUBS DAMAGED DURING CONSTRUCTION. 13) SIGNAGE AND TRAFFIC CONTROL: THE CONTRACTOR SHALL PROVIDE SIGNAGE AND TRAFFIC CONTROL FOR PROJECT AS NEEDED. 14) LOCATION: EXISTING WATER MAINS SHOWN ON THE PLANS ARE APPROXIMATE. THE CONTRACTOR WILL SHIFT THE WATER AND/OR SEWER LINE LOCATION TO AVOID EXISTING UTILITIES OR PROPOSED INFRASTRUCTURE. 15) QUANTITIES: THE CONTRACTOR WILL BE PAID FOR ACTUAL QUANTITIES AT THE UNIT PRICES OR LUMP SUM PRICE STATED IN THE PROPOSAL. PRICES SHALL BE FOR ALL WORK COMPLETE IN PLACE THE CONTRACTOR SHALL MAKE NO ADDITIONAL CLAIMS FOR EXTRA COMPENSATION AS THE RESULT IN THE CHANGE OF LOCATION OF THE PROPOSED WATER AND/OR SEWER MAINS. 16) OBSERVATIONS: FIELD OBSERVATIONS OF WORK AT A PARTICULAR TIME AND PLACE ARE JUST OBSERVATIONS AND DO NOT RE LEAVE THE CONTRACTOR OF HIS SOLE RESPONSIBILITY TO PERFORM ALL WORK AS CALLED FOR IN THE PLANS AND SPECIFICATIONS. 17) COMPLIANCE WITH JURISDICTION: PRIOR TO STARTING ANY WORK, THE CONTRACTOR SHALL CONTACT THE APPROPRIATE JURISDICTIONS AND COMPLY WITH THEIR REQUIREMENTS, SPECIFICATIONS AND DETAILS DURING THE PERFORMANCE OF THE WORK. 18) CHANGES: THE CONTRACTOR SHALL NOTIFY THE ENGINEER WRITING OF ANY CHANGES REGARDING DEPTH OR LOCATION PRIOR TO MAKING CHANGES. 19) WARRANTY: THE CONTRACTOR SHALL WARRANT THE MATERIALS AND WORK FOR ONE YEAR AFTER FINAL ACCEPTANCE. 20) STAKING: STAKING TO BE PROVIDED BY CONTRACTOR. 21) CONFLICTS: THIS PROJECT INCLUDES SEWER AND DRAINAGE PIPE WITH SPECIFIC INVERTS AND RIM ELEVATIONS THAT ARE CRITICAL AND MAY LEAVE LITTLE ROOM FOR ADJUSTMENT. AFTER LOCATES AND BEFORE LAYING ANY PIPE OR CLEARING, THE CONTRACTOR SHALL LOCATE AND VERIFY DEPTHS OF EXISTING UTILITIES AND PIPING. SHOULD A CONFLICT BE FOUND, THE CONTRACTOR SHALL NOTIFY THE ENGINEER IN WRITING OF ANY SUCH CONFLICTS FOR REVIEW AND ADJUSTMENTS OF DESIGN. FAILURE OF THE CONTRACTOR TO VERIFY DEPTHS AND LOCATE ANY AND ALL UTILITIES AND STORM DRAINAGE THAT COULD AND MAY CAUSE ANY CONFLICTS WITH NEW OR PROPOSE UTILITIES OR DRAINAGE PIPE SHALL BE THE SOLE RESPONSIBILITY OF THE CONTRACT. PLEASE NOTE THAT ONLY SUPERPAVE ASPHALT MIXES ARE ACCEPTABLE. 3. PRIOR TO THE PLACEMENT OF ANY ASPHALT, THE DEVELOPER IS REQUIRED TO SUBMIT THE QMS-MV L FORM FOR ALL PROPOSED ASPHALT TYPES. 4. IF AGGREGATE BASE COURSE IS USED IN LIEU OF SOIL TYPE BASE COURSE, THE DEVELOPER IS REQUIRED TO SUBMIT THE TEST RESULTS FOR THE DEPTH AND DENSITY OF THE AGGREGATE BASE COURSE. 5. THE DEVELOPER IS ALSO REQUIRED TO SUBMIT THE VERIFICATION OF COMPLIANCE WITH ENVIRONMENTAL REGULATION S FORM. IT IS NECESSARY FOR THE DEVELOPER TO CONTACT CHRISTOPHER JONES AT: (910) 364-060 IN THE DISTRICT ENGINEER'S OFFICE DURING EACH PHASE OF CONSTRUCTION. 6. THE DEPARTMENT OF TRANSPORTATION RESERVES THE RIGHT TO MAKE ANY INSPECTIONS AND TESTS THAT IT DEEMS NECESSARY TO ASSURE THAT CONSTRUCTION MEETS THE MINIMUM REQUIREMENTS. WHEN CONSTRUCTION IS COMPLETED, THE DEVELOPER IS REQUIRED TO PROVIDE TESTING REPORTS AND A CONTRACTOR CERTIFICATION MEMO. THESE ITEMS SHOULD INDICATE THAT THE CONSTRUCTION ASSOCIATED WITH THE DRIVEWAY PERMIT, SUBDIVISION DESIGN APPROVAL, AND/OR ENCROACHMENT AGREEMENT IS IN COMPLIANCE WITH THE APPROVED PLANS AND THE MINIMUM STANDARDS AND SPECIFICATIONS OF THE DEPARTMENT. 7. UPON COMPLETION OF CONSTRUCTION, IT IS NECESSARY THAT ALL EXPOSED GRADED AREAS BE SEEDED AND MULCHED IN ACCORDANCE WITH THE DEPARTMENT OF TRANSPORTATION'S STANDARD SPECIFICATIONS, WHERE AN OPEN DITCH SECTION IS USED, CONCRETE PAVED DITCH MAY BE REQUIRED TO CONTROL EROSION, IF DEEMED NECESSARY BY THE DISTRICT ENGINEER. 8. THE DEVELOPER SHALL BE RESPONSIBLE FOR ALL TRAFFIC CONTROL MEASURES. THIS INCLUDES, BUT IS NOT LIMITED TO, THE PLACEMENT OF STOP SIGNS AT ALL INTERSCTIONS. ALL TRAFFIC CONTROL MEASURES SHALL MEET THE STANDARDS OUTLINED IN THE MANUAL ON UNIFORM TRAFFIC CONTROL DEVICES (MUTCD) PUBLISHED BY THE FEDERAL HIGHWAY ADMINISTRATION, NORTH CAROLINA DEPARTMENT OF TRANSPORTATION SUPPLEMENT TO THE MUTCD, AND THE CURRENT EDITION OF THE NORTH CAROLINA DEPARTMENT OF TRANSPORTATION ROADWAY STANDARD DRAWINGS.
