Attached please find the new MS4 Permit and Approved Final SWMP. These documents are also
available in the Stormwater Program Laserfiche Repository. The new permit is effective January 1,
2022 through December 31, 2026.

The required Annual Self-Assessment Template associated with this new permit will be provided
under separate cover at a later date. The reporting period for this permit is the calendar year. The
first annual report utilizing the new template will be due no later than February 28, 2023 and will
cover the 2022 calendar year activities (permit year 1).

The issuance of this permit concludes all required actions under Notice of Violation number NOV-
2020-PC-0116, and this action has been officially closed.
