We want to explain the need for the temporary gravel parking. The existing hospital facility will stay in operation during the construction of the new hospital. There is a new parking lot being constructed with the new hospital. There is also some "re -work" of the bigger existing parking lot behind the existing smaller Medical Office Buildings. This situation created a need for temporary parking to be available for the patients, family and employees to use during the construction of the new hospital. This gravel parking lot shares the same physical location on the site as the new stormwater wetland BMP's. After the new parking lot is operational, the gravel parking lot will be removed and the stormwater wetlands can be installed. This would be the final phase of construction. A preliminary review.of your project indicates that before a State Stormwater permit can be issued the followino additional information is needed: 1. As stated on Site plan sheet (C2), the temporary gravel parking lot (64 spaces), located at the southwestern corner of the hospital's property, is to be abandoned and removed in the final phase of construction. Please be reminded that the stormwater rules and the stormwater permit require the stormwater management system to be constructed in its entirety, vegetated and operational for its intended use prior to the construction of any built -upon surface. Considering the cost of planting material and labor, it would be acceptable to have the Stormwater Wetland constructed prior to installation of any built -upon area, but vegetated at the time when the construction is finalized and the site is stabile. Please revise the construction sequence concerning the temporary parking area or relocate the said temporary parking. 2. Provide two copies of signed and sealed revised sheets and requested document. The above requested information must be received in this office prior December 1, 2010 or your application will be returned as incomplete. The return of this project will necessitate resubmittal of all required items including the application fee. If you need additional time to submit the required information, please mail or fax your request for time extension to me at the Letterhead address and include the application number on the request. The request must include justification for the amount of additional time needed. The request will only be granted for justifiable delays that are unforeseen and beyond the owner or consultant's control. You should also be aware that the Stormwater Rules require that the permit be issued prior to any development activity. Construction without a permit is a violation of 15A NCAC 2H.1000 and North Carolina General Statute 143-215.1 and may result in civil penalties of up to $25,000 per day. Limited Soil Exploration Washington County Hospital Plymouth, North Carolina Sunny 72085078 PERSONNEL: Lei Gu As requested by Mr. Jeffrey D. Lundy, representing Henson Design, Inc., a representative of Terracon Consultants, Inc. was on site on December 15, 2008 to provide visual soil classification and to determine the depth to the seasonal high water the areas selected by the client. The hand auger borings included HA-1 and HA-2 as shown on the attached drawing. Each of the borings encountered approximately 6 inches of grass and topsoil at the ground surface. The soils encountered in HA -I consist of yellowish brown (IOYR-5-6), light yellowish brown (IOYR-6-4) and pale brown (IOYR-6-3) clayey sand to approximately 4 feet below the existing grade. An indication of a seasonal high water table was observed at a depth of 4 feet below the existing grade. Light brownish gray (I OYR-6-2) sandy clay sand silty sand was encountered to about 7-5-feet below the existing grade. Groundwater at the time of our hand auger boring was observed at a depth of 7.5 feet below the existing grade. The soils encountered in HA-2 consist of yellowish brown (10YR-5-6) and orange brown silty sand to approximately 4 feet below the existing grade. An indication of a seasonal high water table was observed at a depth of 4 feet below the existin rade. Light gray (10YR-7-2) and orange brown sity sand was encountered to about 7 feet below the existing grade. Groundwater at the time of our hand auger boring was observed at a depth of 7 feet at HA-2. Samples for flexible wall permeability testing were obtained from both borings at a depth of 4.5 to 5 feet below the existing grades. Upon return to the laboratory, the permeability samples were remolded to the conditions indicated on the attached permeability test data sheets. The permeability test results obtained are attached.
