I have included the permit application as an attachment (originally received on January 2, 2014). DEQ is responsible for the time that has lapsed between now and the receipt of the permit; however, 1 am unable to draft the permit with just the information provided: • EPA Fonn I, Section XII, Nature of Business: A brief description has not been provided to the specifications of the directions found on page 1-5 of the instructions provided with the Form.
