The invoice number is 2021PR009920 for NPDES Stormwater Permit NCS000325 for Myers Industries — Patch Rubber. This invoice is now overdue. The billing address on file with the Stormwater Permitting Program is: Myers Industries — Patch Rubber PO Box H Roanoke Rapids, NC 27870 If this billing address is out of date, please respond to this email with the current billing and mailing addresses. If this facility is no longer operational, please complete a Rescission Request Forma and mail it to us so that the permit can be properly closed out.
