Their activities included removal of excessive woody vegetation from around the perimeter of the pond, cutting of the vegetation along the pond slopes, removal of excessive floating vegetation on the ponds surface, and removal of trash and debris from the pond. On September 16, 2019 Mid -Atlantic conducted an inspection of the site to confirm these activities had been conducted. Our inspection indicated that the maintenance activities had been completed and that routine trash and vegetation growth control is necessary to properly maintain the wet detention pond. Janowski Engineering PO Box 2792 Winterville, NC 28590 252-714-3002 MAR 3 2011 STORMWATER MANAGEMENT NARRATIVE TRADE WILCO 4 1845 STORMWATER MODIFICATION — SW 7970519 Renewal HIGH DENSITY March 2, 2011 WASH INGTON, NORTH CAROLINA Narrative This project site and drainage area consists of 1.382 acres of land consisting of 0.929 acres of project within the boundary, 0.192 acres of land between the boundary and the back of curb on US 264 and 0.261 acres of land for the BMP. The impervious area is 0.902 acres total and consists of 0.781 acres (34,004 sf) of impervious area within the site and 0.121 acres (5,258 st) of impervious area off site. The total percent impervious for the watershed is 65.22%. The BMP water surface area is 3,276 sf or 0.075 acres with a depth of 4 feet and is designed to handle the first inch of runoff and the 2 year storm without discharge. The stormwater for this development is also designed for 85% removal of TSS and has a rip rap level spreader to convey treated discharge into the natural drainage system. The design constraints for this project utilize the 1995 BMP manual for this project that was originally permitted in August of 1997. The surface area to drainage area utilizing current Coastal Area runoff charts is computed for 4.165%. This establishes a surface area of 2,507 sf which is less than the 3,276 sf provided. Runoff from this will not be discharged into the adjacent waters of Runyon Creek and is in the Tar -Pamlico Watershed. The original design calculations were for a drainage area of 1.43 acres and a total impervious area of 1.01 acres. The pond utilized a Piedmont Area runoff chart for a 3 foot deep pond with 70% impervious and computed a SA/DA of 2.75% and a surface area of 1,712 sf. SCHEDULE OF COMPLIANCE The stormwater management system shall be constructed in its entirety, vegetated and operational for its intended use prior to the construction of any built -upon surface. 2. During construction, erosion shall be kept to a minimum and any eroded areas of the system will be repaired immediately. 3. The permittee shall at all times provide the operation and maintenance necessary to assure the permitted stormwater system functions at optimum efficiency. The approved Operation and Maintenance Plan must be followed in its entirety and maintenance must occur at the scheduled intervals including, but not limited to: a. Semiannual scheduled inspections (every 6 months). b. Sediment removal. C. Mowing and revegetation of slopes and the vegetated filter. d. Immediate repair of eroded areas. e. Maintenance of all slopes in accordance with approved plans and specifications. f. Debris removal and unclogging of bypass structure, infiltration media, flow spreader, catch basins, piping and vegetated filter. g. A clear access path to the bypass structure must be available at all times. 4. Records of maintenance activities must be kept and made available upon request to authorized personnel of DWQ. This permit shall become voidable unless the facilities are constructed in accordance with the conditions of this permit, the approved plans and specifications, and other supporting data. Upon completion of construction, prior to issuance of a Certificate of Occupancy, and prior to operation of this permitted facility, a certification must be received from an appropriate designer for the system installed certifying that the permitted facility has been installed in accordance with this permit, the approved plans and specifications, and other supporting documentation. Any deviations from the approved plans and specifications must be noted on the Certification. A modification may be required for those deviations. 7. If the stormwater system was used as an Erosion Control device, it must be restored to design condition prior to operation as a stormwater treatment device, and priorto occupancy of the facility. 8. Access to the stormwater facilities shall be maintained via appropriate easements at all times. 9. The permittee shall submit to the Director and shall have received approval for revised plans, specifications, and calculations prior to construction, for any modification to the approved plans, including, but not limited to, those listed below: a. Any revision to any item shown on the approved plans, including the stormwater management measures, built -upon area, details, etc. b. Project name change. C. Transfer of ownership. d. Redesign or addition to the approved amount of built -upon area or to the drainage area. e. Further subdivision, acquisition, lease or sale of all or part of the project area. The project area is defined as all property owned by the permittee, for which Sedimentation and Erosion Control Plan approval or a CAMA Major permit was sought. f. Filling in, altering, or piping of any vegetative conveyance shown on the approved plan. 10. The permittee shall submit final site layout and grading plans for any permitted future areas shown on the approved plans, prior to construction. 11. A copy of the approved plans and specifications shall be maintained on file by the Permittee at all times. 12. The Director may notify the permittee when the permitted site does not meet one or more of the minimum requirements of the permit. Within the time frame specified in the notice, the permittee shall submit a written time schedule to the Director for modifying the site to meet minimum requirements. The permittee shall provide copies of revised plans and certification in writing to the Director that the changes have been made. 13. The reissuance of this stormwater permit does not imply that the site is currently in compliance with the terms and conditions of this state stormwater permit. A plan of action to correct the deficiencies noted in the December 10, 2008 Notice of Violation will need to be submitted to the Division of Water Quality. Failure to provide the requested plan of action may initiate enforcement action. 14. The plans previously approved on August 15, 1997 remain in full force and effect. In the event of a change of ownership, or a name change, the permittee must submit a formal permit transfer request to the Division of Water Quality, accompanied by a completed name/ownership change form, documentation from the parties involved, and other supporting materials as may be appropriate. The approval of this request will be considered on its merits and may or may not be approved. The permittee is responsible for compliance with all permit conditions until such time as the Division approves the transfer request. 2. Failure to abide by the conditions and limitations contained in this permit may subject the Permittee to enforcement action by the Division of Water Quality, in accordance with North Carolina General Statute 143-215.6A to 143-215.6C. The issuance of this permit does not preclude the Permittee from complying with any and all statutes, rules, regulations, or ordinances, which may be imposed by other government agencies (local, state, and federal) having jurisdiction. 4. The permittee will comply with the following schedule for construction and maintenance of the stormwater management system. a. The stormwater management system shall be constructed in its entirety, vegetated and operational for its intended use prior to the construction of any built -upon surfaces except roads. b. During construction, erosion shall be kept to a minimum and any eroded areas of the system will be repaired immediately. 2. The facilities must be properly maintained and operated at all times. The approved Operation and Maintenance Plan must be followed in its entirety and maintenance must occur at the scheduled intervals. 3. The permittee shall at all times provide the operation and maintenance necessary to assure the permitted stormwater system functions at optimum efficiency including, but not limited to: 5 a. Semiannual scheduled inspections (every 6 months) b. Sediment removal C. Mowing and revegetation of side slopes d. Immediate repair of eroded areas e. Maintenance of side slopes in accordance with approved plans and specifications f. Debris removal and unclogging of outlet structure, orifice device and catch basins and piping. 4. Records of maintenance activities must be kept and made available upon request to authorized personnel of DWQ. The records will indicate the date, activity, name of person performing the work and what actions were taken. 5. The approval of this request will be considered on its merits and may or may not be approved. 2. Failure to abide by the conditions and limitations contained in this permit may subject the Permittee to enforcement action by the Division of Water Quality, in accordance with North Carolina General Statute 143-215.6(a) to 143-215.6(c). 3. Repairs/cleaning will be done as necessary. Quarterly (Four times a year): A. Inspect the collection system lie. Catch basins, piping, grassed swales) for proper functioning. Accumulated trash will be cleared from basin grates, basin bottoms, and piping will be checked for obstructions and cleared as required. B. Pond inlet pipes will be checked for undercutting. Riprap or other energy dissipation structures will be replaced, and broken pipes will be repaired. Ill. Semi-annually (twice a year): A. Accumulated sediment from the bottom of the outlet structure will be removed. B. The forebay depth will be checked at various points. Sediment removal is required when the dept is reduced to 75% of the original forebay design depth of 3 ft. C. Grassed swoles, including the vegetated filter if applicable, will be reseeded twice a year as necessary. IV. Annually (Once a year): A. The pond dept will be checked at various points. Sediment removal is required when the dept is reduced to 75% of the original design dept or 3', whichever is greater. Design dept is 3', measured vertically from the orifice down to the pond's bottom. Sediment must be removed to at least the original design depth. V. General: A. Mowing of the side slopes will be accomplished according to the season. Maximum grass height will be 6". B. Cattails are encouraged along the pond, perimeter, however:they will be removed when they cover more than'/s the surface area of the pond. The best time to cut them is at the end of the growing season, in November. Environmentally sensitive chemicals for use in killing cattails can also be used. Contact your nearest Agricultural Extension Agent. C. The orifice/pump is designed to draw down the pond in 2-5 days. If drawdown is not accomplished in that time, the orifice will be replaced with a larger or smaller orifice, Slow drawdown may be attributed to a clogged system. The source of the clogging will be found and eliminated. . D. All components of the detention pond system will be kept in good working order. Repair or replacement components will meet the original design specifications as per the approved stormwater plan. It previously approved components are determined to be ineffective, the component must be redesigned and/or replaced. VI. Special Requirements: Steve Williams hereby acknowledge that I am the financially responsible party for maintenance of this detention pond. Section .1003(h) of 15 A NCAC 2H .1000 (the stormwater rules) requires that applications for permit renewals shall be submitted 180 days prior to the expiration of a permit and must be accompanied by a processing fee, which is currently set at $505.00. After a thorough review of our filing system we found your permit information inadvertently omitted from our database. Due to our error, the lateness of the renewal of your permit is certainly excused. We do, however, ask that you please complete and submit the enclosed renewal application as soon as possible. If you have sold the project and wish to transfer the permit, the renewal application should also include a completed name change form that is enclosed.
