Our preliminary review has determined that the application is not complete. Our office requires that a professional engineer stamp design calculations and drawings. The overall site map (Ll) is not stamped. Furthermore there are no deed restrictions in the submitted package, nor is section VI of the permit application completed. We need the following information in order to conduct a complete review: 1. We require additional definitive information pertaining to deed restrictions for your project. The plans show an outparcel for the property and a future access to the outparcel. However, no deed restrictions are proposed. Please see Section IV of the application which states that Form DRPC-2 is the sample deed restriction for High Density Developments with Outparcels. 2. Section VI of the application is not filled out. The DWQ requires that all items in Section VI be initialed with original signature. 3. The submitted plans do not show all required elements, as contained in Section VI. In particular the "Overall Site Master Plan", number Ll (the Plan) is not sealed signed or dated by a professional engineer. Furthermore there is only one copy of the Plan. DWQ requires one original and one copy of all items listed in Section VI. 4. The Plan does not contain the following required elements; a. Original contours, b. Proposed contours, c. Drainage areas delineated, d. Drainage easements, e. Existing drainage. The Plan states under Property Notes that there is a 0.64 acre tract, and a 1.53 acre tract. Right below that under Project Data the acreage is listed as 1.51 acres and 0.66 acres. Please reconcile these numbers. 6. The Plan states that there is 28, 907 square feet of vehicle use area. However, in the application the on site built upon area (not including the buildings) is 26,017 square feet. Please reconcile these numbers. 7. The Plan shows parts of the treatment unit located in the right of way for Burns Street. If the treatment unit is located in the right of way we need a statement from either DOT or the owner of the public road stating that they will grant an permanent drainage easement for the treatment unit such that the unit will not be disturbed by future activities. 8. The Plan does not show all of the symbols used in the legend. Specifically, the solid line with three dots is not defined, nor is the line with open circles surrounding the property. There are also several different setbacks listed. Please clarify the symbols on the Plan, and clearly define the right of ways and setbacks. 9. The Drainage Area Table on page 2 of the permit lists the impervious surface area as 4879, 8464, 8464, 8464, and 625, and the total as 30271 square feet. Please reconcile these numbers as the sum of the five numbers in the table does not equal 30271. 10.The vegetated shelf is required to have a side slope of 10:1. The detail on plan C1 shows the side slope of the entire wet detention as 3:1. Please revise the plans to show the required side slope. 11. The forebay is required to have a direct access provided for appropriate maintenance equipment as detailed in the BMP manual. Also, the forebay should be made of hardened material to facilitate ease of maintenance. The forebay should also have a fixed vertical sediment depth marker to measure sediment deposition over time. Please include these elements in the resubmittal, or address the reasons that these elements are not included. 12. Wetlands are not delineated on the plans. If no wetlands exist on the site then a note on the plans stating that none exist must be included. 13. The volume is not included in the wet detention supplement. 14.The on site Report of Geotechnical Exploration is included in the submittal. However, the boring B-4 log is not included. This location is right in the middle of the treatment unit. Furthermore, information should be included that supports either lining or not lining the wet detention area based on SHWT and soil types. The Division of Water Quality is allowed 90 days from the receipt of a completed application to issue the permit. You must submit the requested information within 30 days. If the requested information is not received within 30 days the application will be returned as incomplete. Original documents must be provided; copies are not acceptable.
