Staff review of the plans and specifications has determined that the project, as proposed, will comply with the Stormwater Regulations set forth in Title 15A NCAC 2H.1000. We are forwarding Permit No. SW8 941015 Modification dated November 7, 1997, for the construction of Holiday Inn Sunspree Wrightsville Beach. This permit shall be effective from the date of issuance until November 7, 2007, and shall be subject to the conditions and limitations as specified therein. Please pay special attention to the Operation and Maintenance requirements in this permit. Failure to establish an adequate system for operation and maintenance of the stormwater management system will result in future compliance problems. If any parts, requirements, or limitations contained in this permit are unacceptable, you have the right to request an adjudicatory hearing upon written request within thirty (30) days following receipt of this permit. This request must be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings, P.O. Drawer 27447, Raleigh, NC 27611-7447. Unless such demands are made this permit shall be final and binding. The following items will require a modification to the permit: a. Any revision to the approved plans, regardless of size. b. Project name change. C. Transfer of ownership. d. Redesign or addition to the approved amount of built -upon area. e. Further subdivision of the project area. f. Filling in, altering, or piping of any vegetative conveyance shown on the approved plan. In addition, the Director may determine that other revisions to the project should require a modification to the permit. 7. The Director may notify the permittee when the permitted site does not meet one or more of the minimum requirements of the permit. Within the time frame specified in the notice, the permittee shall submit a written time schedule to the Director for modifying the site to meet minimum requirements. The permittee shall provide copies of revised plans and certification in writing to the Director that the changes have been made. H. SCHEDULE OF COMPLIANCE The permittee will comply with the following schedule for construction and maintenance of the stormwater management system: a. The stormwater management system shall be constructed in it's entirety, vegetated and operational for it's intended use prior to the construction of any built -upon surface except roads. b. During construction, erosion shall be kept to a minimum and any eroded areas of the system will be repaired immediately. 2. The permittee shall at all times provide the operation and maintenance necessary to assure the permitted stormwater system functions at optimum efficiency. The approved Operation and Maintenance Plan must be followed in its entirety and maintenance must occur at the scheduled intervals including, but not limited to: a. Semiannual scheduled inspections (every 6 months). b: Sediment removal when drawdown takes > 5 days. C. Mowing and revegetation of side slopes. d. Immediate repair of eroded areas. e. Maintenance of side slopes in accordance with approved plans and specifications. f. Debris removal and unclogging of catch basins and piping. g. Should the system be determined to have failed, it will be restored to design specifications, or alternate and equal treatment will be constructed. Records of maintenance activities must be kept and made available upon request to authorized personnel of DWQ. The records will indicate the date, activity, name of person performing the work and what actions were taken. 4. This permit shall become voidable unless the facilities are constructed in accordance with the conditions of this permit, the approved plans and specifications, and other supporting data. 4 State Stormwater Management Systems Permit No. SW8 941015 Modification 5. Upon completion of construction, prior to issuance of a Certificate of Occupancy, and prior to operation of this permitted facility, a certification must be received from an appropriate designer for the system installed certifying that the permitted facility has been installed in accordance with this permit, the approved plans and specifications, and other supporting documentation. Any deviations from the approved plans and specifications must be noted on the Certification. 6. A copy of the approved plans and specifications shall be maintained on file by the Permittee for a minimum of ten years from the date of the completion of construction. 7. Prior to the sale of any portion of the property, an access/maintenance easement to the stormwater facilities shall be granted in favor of the permittee if access to the stormwater facilities will be restricted by the sale of any portion of the property. III. GENERAL CONDITIONS This permit is not transferable. In the event there is a desire for the facilities to change ownership, or there is a name change of the Permittee, a formal permit request must be submitted to the Division of Water Quality accompanied by an application fee, documentation from the parties involved, and other supporting materials as may be appropriate. The approval of this request will be considered on its merits and may or may not be approved. 2. Failure to abide by the conditions and limitations contained in this permit may subject the Permittee to enforcement action by the Division of Water Quality, in accordance with North Carolina General Statute 143-215.6A to 143- 215.6C- 3. The issuance of this permit does not preclude the Permittee from complying with any and all statutes, rules, regulations, or ordinances which may be imposed by other government agencies (local, state, and federal) which have jurisdiction. 4. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions, the Permittee shall take immediate corrective action, including those as may be required by this Division, such as the construction of additional or replacement stormwater management systems. 5. The permittee grants DENR Staff permission to enter the property for the purpose of inspecting all components of the permitted stormwater management facility. 6. The permit may be modified, revoked and reissued or terminated for cause. The filing of a request for a permit modification, revocation and reissuance or termination does not stay any permit condition. Permit issued this the 7th day of November, 1997. B-3 Wilmington, North Carolina 28405 F, S&ME April 9, 1997 F Pearsall Operating Company Post Office Box 7965 Rocky Mount, North Carolina 27804 Attention: Mr. Mack Pearsall Reference: Geotechnical Report Proposed Multi -Story Hotel Wrightsville Beach, North Carolina Job No. 1063-97-140 Dear Mr. Pearsall: �-- S&ME, Inc. has completed the authorized geotechnical study at the proposed multi -story hotel site located at the existing Holiday Inn SunSpree Resort property in Wrightsville Beach, North Carolina. Subsurface conditions at the site were investigated with three test borings (Boring Nos. B-1, B-3, and B-4). Boring No. B-2 was not drilled because parked vehicles along the public access lane prevented the opening of the fence gate on the north property boundary. Each boring was drilled to a depth of 50 feet below the existing 1ground surface, and soil samples were obtained by the standard penetration test method : (ASTM D-1586) to determine the consistency of the subsurface soils. This report presents the findings of our investigation with recommendations for support of the hotel based on these findings.. PROJECT DESCRIPTION The existing Holiday Inn SunSpree Resort is located on the east side of Lumina Avenue and abuts the Atlantic Ocean. The existing structure is presently not being used as a motel due, to the extensive wind and water damage which was caused during Hurricane Fran in September 1996. ' We understand that the property owners are considering razing the existing structure and constructing a new multi -story hotel at the location shown in Figure 1. Although structural detailing of the proposed multi -story hotel has not been completed at this time, our conversations with the project structural engineer, Mr. Bill Egan of McKim & Creed Engineers, indicate that the proposed building would be supported by piles F,.!" designed to support a working compression load on the order of 75 to 100 tons. r. The site groundwater level will fluctuate with tidal and climatic changes, however, and may be higher at other times of the year. Depth Ft. Variations from the generalized subsurface profile {� described above and a more detailed description of the subsurface conditions are presented L in the attached Test Boring Records. RECOMMENDATIONS This report has been prepared in accordance with generally accepted ' geotechnical engineering practice for specific application to this project. The conclusions and recommendations contained in this report are based upon applicable standards of our practice in this geographic area. at the time this report was prepared. B-3 Wilmington, North Carolina 28405 i Pearsall Operating Company April 9, 1997 Page 3 The analyses and recommendations submitted herein are based, in part, upon the data obtained from the subsurface exploration. The nature and extent of variations between the borings will not become evident until construction: If variations appear evident, then we will re-evaluate the recommendations of this report. In the event that any changes in the nature, design, or location of the building are planned, the conclusions and recommdndations contained in this report will not be considered valid unless the changes are reviewed and conclusions modified or verified in writing. We strongly recommend that S&ME be provided the opportunity to review the final design plans and specifications in order that earthwork and foundation recommendations are properly interpreted and implemented. Foundation Support - Piles. Subsurface conditions at the site are suitable for the development of medium to relatively high capacity pile foundations depending upon pile type and tip elevation. Based on the attached test boring data and using a factor of safety of approximately two, we have estimated the following allowable working capacities for timber and concrete piles. Because the lateral deflection of the pile will vary in proportion to the applied horizontal load, larger horizontal design loads can be used provided that the increased lateral deflection can be tolerated. Pearsall Operating Company April 9, 1997 Page 4 It is our opinion that the contractor will not be able to drive precast concrete piles to a depth of 40 feet because of the presence of dense to very dense sands within the upper 40 feet. We recommend that the contractor jet each precast concrete pile to a depth F_ that is within two to three feet of the require tip elevation and then drive each pile to attain the required penetration resistance .value. The subsurface sands at this site are well suited for the construction of auger - cast piles. Our past experience with the construction of auger -cast grout piles at Wrightsville Beach has shown that this type of pile can quickly be installed to a uniform tip elevation. However, since no pile driving penetration resistance values are recorded during the Fi` construction pf auger -cast grout piles, it is not possible to confirm the working load of each pile be reviewing the field data. In addition, it is very important that an experienced construction crew install auger -cast grout piles in order to reduce the chances that F-- construction irregularities (such as "necking" of the pile) occur. We have noted in the Wilmington area that the installation of steel "H" piles is generally more expensive than installing either precast concrete or auger -cast grout piles. One advantage associated with non -displacement piles, such as steel "H" piles, is the ability r to drive these piles through dense sands. The relative ease with which steel "H", piles can he driven through dense sand strata, though, also makes it more difficult to accurately estimate pile lengths and working loads for steel "H" piles. Pile Installation. For precast concrete and steel "H" piles, we recommend that driving criteria and bearing elevations be established during driving by applying the North Carolina Building Code. We also recommend that 5 or 6 overlength piles be driven across the hotel site to establish driving criteria, pile lengths and the need to jet the piles into place prior to the start of production driving. Production piles should not be ordered until the length estimates can be verified; however, the overlength piles may be driven at the production pile locations. F-11i" The North Carolina Building Code requires that a pile load test be conducted to verify design loads exceeding 40 tons. On this basis, we recommend that one compression pile load test be conducted in accordance with the procedures presented in the North Carolina Building Code and in ASTM D-1143. The new structure will be set back 120' from the first line of vegetation. The site will be filled Generally to elevation 12.00 MSL. This level of the project will be utilized for parking and a swimming pool. The entrance to the hotel will be at level 2 - elevation 22.00 MSL accessed by vehicular ramp. The motor lobby is at this level with elevator and stair access to level 3 main lobby but the balance of the area within the building footprint at level 2 is utilized for parking and mechanical equipment. Level 3 - elevation 32.00 MSL contains the main lobby, restaurant, banquet and meeting spaces, administrative offices, and facilities for food service and housekeeping. All commercial spaces are at level 3. Levels 4 through 9 are guest room floors. The hotel is designed to serve 196 rooms. The first guest room floor is at elevation 47.00 MSL.
