We are forwarding Permit No. SWA000099, dated June 4, 2021. This permit shall be effective from the date of issuance and shall be subject to the conditions and limitations as specified therein, and does not supersede any other agency permit that may be required. The original signed permit package is being mailed to Jerry Frakes at: Arrow Disposal Group, Inc. PO Box 472 Harrisburg, NC 28075 This project will be kept on file after 30 days at the Mooresville Regional Office.
