The Department of Environmental Quality (DEQ) will be performing a National Pollutant Discharge
Elimination System (NPDES) Municipal Separate Ston-n Sewer Systern (IMS4) Permit compliance
audit for the subject pen -nit. This correspondence i� a fon-nal request to submit required pen -nit
docui-nentation prior to DEQ scheduling the audit. All submitted docurnentation will become a
pen-nanent record of the compliance audit. DEQ may request additional inron-nation prior to, during
andlor following the compliance audit. The scope and venue for the compliance audit will be
deten-nined based on a review of the provided documentation. and DEQ will notify you in writing of
the date and time for the interview and!or inspection portions Of the aUdit.
Please submit the following required permit documentation for the current permit within
seven (7) calendar days of receipt of this letter:
a. Ston-nwater Management Plan (SWMP)
b. Annual Reports not already submitted through DEQ's online BIMS SWMPA portal

d. MS4 map of outfalls and receiving waters
e. Fon-nal stormwater program implementation agreements with any other entities
f. Illicit Discharge Detection and Elimination Program Plan
