Good afternoon Preston,
I've gone into our repository and looked at the original Permit application, which may be accessed from our website,
below is a link
Stormwater Program Laserfche Repository, once open click on Individual Permits then scroll down to
NCS000567 and click on the folder to see all the documents we have on file for your site.
I've attached two pages from the original permit application that may help with identifying the outfall.
If this doesn't help, please contact the Regional Engineering Supervisor, Tamera Eplin 336-776-9654 to schedule a site
visit.

ECOFLO has made several unsuccessful attempts to find outfall #1 during our stormwater monitoring. Since the outfall
location cannot be determined, ECOFLO would like to remove the outfall from our stormwater permit. Your help and
guidance in resolving this issue would be greatly appreciated.
Please let me know if you have any comments or questions concerning this matter.

ECOFLO is a recycling facility that receives various types of waste material from Industry and from the Houesehold Hazardous
waste program. The wastes that are collected can be in both solid and liquid forms. Up until recently, ECOFLO has stored both
used pallets, and various electronics outside of the facility near the North Fenceline. These items are no longer stored
outside of the facility.
C. For each outfall, provide the location and a description of existing structural and nonstructural control measures to reduce pollutants in storm water runoff; and a
description of the treatment the storm water receives, including the schedule and type of maintenance for control and treatment measures and the ultimate disposal
of any solid or fluid wastes other than by discharge.
