The revisions include strengthening the forebay berm and creating a weir in the berm. The new berm will include a concrete cap on top, extending 12" below the outfall elevation, to discourage scouring as the water exits the forebay. It has been determined that a formal permit modification is not required for the proposed changes. We are forwarding you an approved copy of the revised plans for your files. Please replace the old approved plan sheets with the new ones. Please be aware that all terms and conditions of the permit, issued on December 3, 2008, remain in full force and effect. Please also understand that the approval of this revision to the approved plans for the subject State Stormwater Permit is done on a case -by -case basis. Any other changes to this project must be approved through this Office prior to construction. The issuance of this plan revision does not preclude the permittee from complying with all other applicable statutes, rules, regulations or ordinances which may have jurisdiction over the proposed activity, and obtaining a permit or approval prior to construction. The forebay outfall has been partially revised form an earthen berm to a concrete weir. The weir elevations have been revised as discussed to 21.00 and 21,50. The concrete weir apron extends 12" below the outfall elevation to discourage scouring as the water exists the forebay. If you are applying for a revision, please make sure that I receive it this week. I go through each package when it comes in to make sure that we have all of the documentation that we need. I generally ask for missing items to be submitted by COB the next day or I have to return the application. I didn't set a deadline originally because you aren't local. But, I really need the complete package by Friday or I will have to return it.
