Inspection Summary:
Site visit conducted regarding a Notice of Intent (NOI) application for coverage under the NPDES NCG020000 General Perm
that was received on October 21, 2019. The application submittal indicated that the site began as a single source borrow pit
with an approved erosion control plan. Plans to expand the mining activities triggered the requirement for a mining permit anc
coverage under the NCG020000 permit. A mining permit (78-51) was issued on January 7, 2020. Coverage under the
NCG020000 permit was not issued due to incomplete or missing information on the NOI. Contact was made with Clay Creed
with Z V Pate, Inc. via a telephone call on January 8, 2021 to discuss the status of the mine site and the NOI. According to
Mr. Creed, mine expansion did not occur and mining activities ceased approximately 6-9 months ago. Site visit confirmed no
pit expansion, no current mining activities and no apparent discharge outfalls or mine dewatering activities. Based on the
site observations, coverage under the NCG020000 permit is not required at this time and the NOI application can be
withdrawn upon applicant's request.
