the Owner Verification Report for the facility:
• Please remove Lynn Farley as the ORC Backup noted under permit NCO040711 and replace with Patrick
Mays, Certification #: PC-2/1009460, Cert Status: Active, ORC Designation: Primary
Also, please update the following information per the Stormwater Permit Summary report guidelines (found here):
• Facility Contact Person: Please remove Yvonne Couts and replace with Savannah Carroll — all contact
info is the same except the email is [e-mail]
• Permit Contact Person: Please remove Jim Skinner and replace with Savannah Carroll — all contact info is
the same as above
• Permit Billing Contact: Please remove Carolyn M. Underwood and replace with Peter Stumbar, Address:
