remedies for the items listed in your request.
1. Please delineate the drainage area for the future built -upon area allocation on Drawing CI
and add a note stating that: (a) the area is not included in the design of Infiltration Beds 41
and #2, and (b) a stormwater control measure per 15A NCAC 2H.1008(a) and a separate
stormwater permit will be required prior to any development in the future allocation area.
The area has been delineated on sheet C1 and a note has been added to the plan as per
your request above. Sheet C1 has been included with this response.

Dear Dr. Williamson:
The Wilmington Regional Office received a Stormwater Management Permit Application for
Fore Christ Ministries on February 14, 2005 and revised applications on May 10 and May 17,
2005. A preliminary'review of that information has determined that the application is not
complete. The following information is needed to continue the stormwater review:
Please delineate the drainage area for the future built -upon area allocation on
Drawirig C1 and add a note stating that: (a) the area is not included in the design
of Infiltration Trench Beds #1 and #2, and (b) a stormwater control measure per
15A NCAC 2H.1008(a) and a separate stormwater permit will be required prior to
any development in the future allocation area.
2. Please clarify the infiltration rate. The calculations list 1.5 in/min and the
supplements list 1.5 in/hr.
Please note that this request for additional information is in response to a preliminary review.
The requested information should be received by this Office prior to May 26, 2005, or the
application will be returned as incomplete. The return of a project will necessitate resubmittal of
all required items, including the application fee.
If you need additional time to submit the information, please mail or fax your request for a time
extension to the Division at the address and fax number at the bottom of this letter. The
request must indicate the date by which you expect to submit the required information.
The construction of any impervious surfaces, other than a construction entrance under
an approved Sedimentation Erosion Control Plan, is a violation of NCGS 143-215.1 and
is subject to enforcement action pursuant to NCGS 143-215.6A.

STORMWATER AND EROSION CONTROL NARRATIVE
The Fore Christ Ministries site is located in Brunswick County on NC 179 (Old Georgetown Road)
approximately 3.4 miles south of the intersection of NC 179 and NC 904. Fore -Christ, Inc.
proposes land clearing and grubbing activities only in preparation for a future commercial building.
The total project area is 6.22 acres and the total disturbed area is calculated to be 6.0 acres.
Future stormwater runoff will be routed using curb inlets and underground pipes to an underground
infiltration trench system
Temporary diversions shall be installed around the perimeter of the site and will be routed to a
temporary sediment basin, which will be used to prevent any significant amounts of sediment from
leaving the site. These measures shall be inspected regularly, and the accumulated sediment
removed to prevent the conveyance of sediment to any off -site areas.
The primary permanent erosion control measures that will remain after the temporary measures are
removed will be permanent grassing.

construction, operation and maintenance of two underground infiltration trench systems
in compliance with the provisions of 15A NCAC 2H A 000 (hereafter referred to as the
"stormwater rules') and the approved stormwater management plans and specifications
and other supporting data as attached and on file with and approved by the Division of
Water Quality and considered a part of this permit.
This permit shall be effective from the date of issuance until May 25,' 2015, and shall be
subject to the following specified conditions and limitations:
I. DESIGN STANDARDS
This permit is effective only with respect to the nature and volume of stormwater
described in the application and other supporting data.
2. This stormwater system has been approved for the management of stormwater
runoff as described in Section 1.6 on page 3 of this permit. The stormwater
controls have been designed to handle the runoff from 54,501 square feet total
of impervious area. Overflow from both trenches discharges to vegetated swaies
that exceed 50 feet in length.
3. The tract.wiil be limited to the amount of built -upon area indicated on page 3 of
this permit, and per approved plans.
4. All stormwater collection and treatment systems must be located in either
dedicated common areas or recorded easements. The final plats for the project
will be recorded showing all such required easements, in accordance with the
approved plans.
5. The runoff from all built -upon area within the permitted drainage area of this
project must be directed into the permitted stormwater control system.

II. SCHEDULE OF COMPLIANCE
The stormwater management system shall be constructed in its entirety,
vegetated and operational for its intended use prior to the construction of any
built -upon surface.
2. During construction, erosion shall be kept to a minimum.and any eroded areas of
the system will be repaired immediately.
3. The permittee shall at all times provide the operation and maintenance
necessary to assure the permitted stormwater system functions at optimum
efficiency. The approved Operation and Maintenance Plan must be followed in
its entirety and maintenance must occur at the scheduled intervals including, but
not limited to:
a. Semiannual scheduled inspections (every 6 months).
b. Sediment removal.
C. Mowing and revegetation of slopes and the vegetated filter.
d. Immediate repair of eroded areas.
e. Maintenance of all slopes in accordance with approved plans and
specifications.
f. Debris removal and unclogging of bypass structure, filter media, flow
spreader, catch basins, piping and vegetated filter.
g. A clear access path to the bypass structure must be available at all times.
4. Records of maintenance activities must be kept and made available upon
request to authorized personnel of DWQ. The records will indicate the date,
activity, name of person performing the work and what actions were taken.
5. The facilities shall be constructed as shown on the approved plans. This permit
shall become voidable unless the facilities are constructed in accordance with
the conditions of this permit, the approved plans and specifications, and other
supporting data.

6. Upon completion of construction, prior to issuance of a Certificate of Occupancy,
and prior to operation of this permitted facility, a certification must be received
from an appropriate designer for the system installed certifying that the permitted
facility has been installed in accordance with this permit, the approved plans and
specifications, and other supporting documentation. Any deviations from the
approved plans and specifications must be noted on the Certification. A
modification may be required for those deviations.
7. If the stormwater system was used as an Erosion Control device, it must be
restored to design condition prior to operation as a stormwater treatment device,
and prior to occupancy of the facility.
8. Access to the stormwater facilities shall be maintained via appropriate
easements at all times.
9. The permittee shall submit to the Director and shall have received approval for
revised plans, specifications, and calculations prior to construction, for any
modification to the approved plans, including, but not limited to, those listed
below:
a. Any revision to any item shown on the approved plans, including the
stormwater management measures, built -upon area, details, etc. Runoff
from the undeveloped, Future Allocation Area on the approved plans
(Drawing Cl) is not included in the design of the infiltration systems of this
permit. A separate stormwater measure(s) and permit will be required for
any development in the Future Allocation Area.
b. Project name change.
C. Transfer of ownership.
d. Redesign or addition to the approved amount of built -upon area or to the
drainage area.
e. Further subdivision, acquisition, lease or sale of all or part of the project
area. The project area is defined as all property owned by the permittee,
for which Sedimentation and Erosion Control Plan approval or a CAMA
Major permit was sought.
f. Filling in, altering, or piping of any vegetative conveyance shown on the
approved plan.
10. The permittee shall submit final site layout and grading plans for any permitted
future areas shown on the approved plans, prior to construction.
11. A copy of the approved plans and specifications shall be maintained on file by
the Permittee for a minimum of ten years from the date of the completion of
construction.
12. The Director may notify the permittee when the permitted site does not meet one
or more of the minimum requirements of the permit. Within the time frame
specified in the notice, the permittee shall submit a written time schedule to the
Director for modifying the site to meet minimum requirements. The permittee
shall provide copies of revised plans and certification in writing to the Director
that the changes have been made.

Ill. GENERAL CONDITIONS
This permit is not transferable except after notice to and approval by the Director.
In the event of a change of ownership, or a name change, the permittee must
submit a formal permit transfer request to the Division of Water Quality,
accompanied by a completed name/ownership change form, documentation
from the parties involved, and other supporting materials as may be appropriate.
The approval of this request will be considered on its merits and may or may not
be approved. The permittee is responsible for compliance with all permit
conditions until such time as the Division approves the transfer request.
2. Failure to abide by the conditions and limitations contained in this permit may
subject the Permittee to enforcement action by the Division of Water Quality, in
accordance with North Carolina General Statute 143-215.6A to 143-215.6C.
3. The issuance of this permit does not preclude the Permittee from complying with
any and ai.l statutes, rules, regulations, or ordinances, which may be imposed by
other government agencies (local, state, and federal) having jurisdiction.
4. In the event that the facilities fail to perform satisfactorily, including the creation
of nuisanc'.e conditions, the Permittee shall take immediate corrective action,
including those as may be required by this Division, such as the construction of
additional :or replacement stormwater management systems.
5. The permittee grants DENR Staff permission to enter the property during normal
business hours for the purpose of inspecting all components of the permitted
stormwater management facility.
6. The permit may be modified, revoked and reissued or terminated for cause. The
filing of a request for a permit modification, revocation and reissuance or
termination does not stay any permit condition.
7. Unless specified elsewhere, permanent seeding requirements for the stormwater
control must follow the guidelines established in the North Carolina Erosion and
Sediment Control Planning and Design Manual.
8. Approved"plans and specifications for this project are incorporated by reference
and are enforceable parts of the permit.
9. The permittee shall notify the Division any name, ownership or mailing address
changes within 30 days.

The Wilmington Regional Office received a Stormwater Management Permit Application for
Fore Christ Ministries on February 14, 2005. A preliminary review of that information has
determined that the application is not complete. The following information is needed to
continue the stormwater review:
051. Please clarify the title for the signature authorization of Sections 1.2. and Vlll of
the permit application and page 4 of the supplement Form SWU-102. A principal
executive officer, in the case of corporations (VP or higher), must sign permit
4(�O applications in accordance with North Carolina Administrative Code 15A NCAC
?Section 2H.1003(e)(1) and (2) for Stormwater Management. The signature of the
consulting engineer or other agent shall be accepted on the application only if
accompanied by a letter of authorization. Dr. Williamson is listed as an "Owner"
in Section 1.2 of the application and on page 4 of the Wet Detention Basin
Supplement.
2. Please confirm that site drainage is to Shingletree Swamp. It appears from spot
elevations on the Stormwater Plan that site drainage may be west along NC 179
5, 4 away from Shingletree Swamp. If drainage is to the Calabash River, the
�1�,t 5 proposed project is within 0.5 miles of SA receiving waters, and an infiltration
t ), 1 system must be used for a 1.5-inch design rainfall.
3. Please provide basin overflow and stormwater runoff direction arrows on the
plans, including along NC 179.
4. The supplement form lists 15,618.4 sq ft of provided Permanent Pool surface
area and the stamped calculations list 15,644 sq ft. Please confirm the correct
amount with consideration for the 3' vegetative shelf.
Please note that this request for additional information is in response to a preliminary review.
The requested information should be received by this Office prior to February 23, 2005, or the
application will be returned as incomplete. The return of a project will necessitate resubmittai of
all required items, including the application fee.

Pursuant to § 55A-10-06 of the General Statutes of North Carolina, the
undersigned corporation hereby submits these Articles of Restatement for the purpose
of integrating into one document its original Articles of Incorporation and all
amendments thereto and also for the purpose of amending its Articles of Incorporation.
1. The name of the corporation is Fore Christ Ministries, Inc.
2. The text of the Restated Articles of Incorporation is attached.
3. These Restated Articles of Incorporation were adopted by the board of
directors and contain an amendment not requiring member approval
because the corporation has no members.
4. These Articles will become effective upon filing.

ARTICLES OF INCORPORATION
OF
FORE CHRIST MINISTRIES, INC.
The undersigned submits these Articles of Incorporation for the purpose of
forming a nonprofit corporation under the laws of the State of North Carolina.
1. The name of the corporation is Fore Christ Ministries, Inc.
2. The corporation is a charitable corporation.
3. The period of duration of the corporation shall be perpetual.
4. The objects and purposes for which this corporation is organized are to
operate exclusively as a church, in all of its religious purposes.
The foregoing clauses shall be construed as both objects and powers, and the
enumeration of specific powers shall not be deemed to limit or restrict in any manner
the general powers of the corporation as conferred by the laws of the State of North
Carolina now or hereafter in effect.
No part of the net earnings of the corporation shall inure to the benefit of, or be
distributable to its Directors, officers, or other private persons, except that the
corporation shall be authorized and empowered to pay reasonable compensation for
services rendered and to make payments and distributions in furtherance of the
purposes set forth in these Articles. No substantial part of the activities of the
corporation shall be the carrying on of propaganda, or otherwise attempting to
influence legislation, and the corporation shall not participate in, or intervene in
(including the publishing or distributing of statements) any political campaign on behalf
of or in opposition to any candidate for public office. Notwithstanding any other
provisions of these Articles, the corporation shall not carry on any other activities not
permitted to be carried on (a) by a corporation exempt from federal income tax under
section 501(c)(3) of the Internal Revenue Code, or the corresponding section of any
future federal tax code, or (b) by a corporation contributions to which are deductible
under section 170(c)(2) of the Internal Revenue Code, or the corresponding section of
any future federal tax code.
5. The corporation shall have no capital stock.
6. The corporation shall have no members.
7. The street address of the initial registered office and principal office of the
corporation is 2618 New Village Way, Wilmington, New Hanover County, North
Carolina 28405. The mailing address of the initial registered office and principal office
of the corporation is 2618 New Village Way, Wilmington, New Hanover County, North
Carolina 28405. The initial registered agent of the corporation at this address is
DeCarol Williamson.
B. Upon dissolution of the corporation, assets shall be distributed for one or
more exempt purposes within the meaning of section 501 {c}(3) of the Internal Revenue
Code or the corresponding section of any future federal tax code, or shall be
distributed to the federal government or to a state or local government for a public
purpose. Any such assets not so disposed of shall be disposed of by a court of
competent jurisdiction of the county in which the principal office of the corporation is
then located exclusively for such purposes or to such organization or organizations as
said court shall determine, which are organized and operated exclusively for such
purposes.
9. The affairs of the corporation shall be managed by a Board of Directors.
The number and qualifications of the Directors, together with their terms of office,
manner of election, removal, change of number, filling of vacancies, and of newly
created directorships, powers, duties, and liabilities shall be prescribed in the Bylaws
and these Articles of Incorporation. To the extent the Bylaws and these Articles of
Incorporation are inconsistent with the laws of the State of North Carolina, the Bylaws
and Articles of Incorporation shall control and shall be deemed to restrict the laws of
the State of North Carolina to the extent permitted.
10. The number of Directors constituting the initial Board of Directors shall be
five (5). The initial Board of Directors of the corporation shall consist of certain
"Founding Directors" and "Nonfounding Directors," as more specifically set forth in the
Bylaws. Any increase or decrease in the number of Directors shall be conditioned on
obtaining the prior written consent of DeCarol Williamson and Jan M. Williamson,
jointly, if they are then acting as Directors, or by th-e one remaining if only one of them
is then acting as a Director, or, if neither of them is then acting as a Director, by a
majority of the then serving successor and alternate successor Founding Directors. As
set forth in the Bylaws, the initial Founding Directors of the corporation shall be
DeCarol Williamson and .Ian M. Williamson and the initial Nonfounding Directors shall
be Mel Tari, Enrique Solano and Jerry Hannah. DeCarol Williamson and Jan WilliamsonF
shall serve as the Founding Directors for each of their lifetimes, or until such time as
either declines or is unable to serve as a Founding Director or resigns as a Founding
Director. The Bylaws allow for the appointment of successor Founding Directors and
alternate successor Founding Directors who shall be entitled to serve for his or her
lifetime, or until such time as he or she declines or is unable to serve as a Founding
Director or resigns as a Founding Director, No Founding Director, whether an original,
successor, or alternate successor, may be removed.. These Articles shall limit the
application of North Carolina General Statutes Sections 55A-8-08 and 55A-8-09 to the

extent necessary to carry out the restrictions on the removal of Founding Directors,
whether original, successor, or alternate, as set forth herein and in the Bylaws.
All Nonfounding Directors shall be appointed, and vacancies filled, as provided in
the Bylaws. The authority of the Nonfounding Directors shall be limited, as
contemplated by North Carolina General Statutes Section 55A-8-01, to those matters
specifically granted to the Nonfounding Directors in the Bylaws. Any authority
specifically granted to the Founding Directors, whether an original, successor, or
alternate successor Founding Director, in the Bylaws or these Articles of Incorporation,
including but not limited to the appointment of Founding Directors, Nonfounding
Directors and officers, as set forth in the Bylaws or herein, shall only be exercised by
the Founding Directors. The Nonfounding Directors shall have no authority to act with
respect to such matters. These Articles, to the extent permitted under North Carolina
law, shall restrict the authority of the Nonfounding Directors as described in the
Bylaws or herein.

12. Except to the extent that the North Carolina General Statutes prohibit
such limitation or elimination of the liability of Directors for breaches of duty, no
Director of the corporation shall have any personal liability arising out of an action
whether by or in the right of the corporation or otherwise for the monetary damages
for breach of any duty as a Director. No amendment or repeal of this article shall apply
to or have any effect on the liability or alleged liability of any Director of the
corporation for or with. respect to any acts or omissions of such Director occurring
prior to such amendment or repeal. The provisions of this article shall not be deemed
to limit or preclude indemnification of a Director by the corporation for any liability that
has not been eliminated by this article.
13. These Articles may be amended, altered, restated or repealed, or new
Articles adopted by the affirmative vote of a majority of the Board of Directors at any
regular or special meeting of the Board; provided, that the proposed alteration or
amendment is contained in a notice of the meeting given in accordance with the laws
of the State of North Carolina. Notwithstanding anything herein to the contrary, in no
event shall the Board of Directors amend, alter, restate, repeal or adopt any provision
that has the effect of amending in any way these Articles without obtaining the prior
approval in writing, as contemplated by North Carolina General Statutes Section 55A-
10-30, of the following persons in the order named (provided that a succeeding person

listed is not required to consent unless the preceding person (or his or her designee) is
unable to make such consent):
(a) DeCarol Williamson, in his individual capacity, or his designee;
ib) Jan M. Williamson, in her individual capacity, or her designee; and
(c) the designee as elected by a majority of the then serving successor and
alternate successor founding Directors.
Any amendment to the Articles of Incorporation shall require prior written
approval, as set forth herein, pursuant to North Carolina General Statutes Section 55A-
10-30. In no event shall the Board of Directors amend, alter, restate, repeal or adopt
any provision in such a manner as to disqualify the corporation as a qualified charitable
organization under Section 501(c)(3) of the Internal Revenue Code, or the
corresponding section of any future federal tax code.

project in Brunswick County. The work proposed is the construction of a video recording studio and
parking lot and stormwater wet detention system.
Decarol WAliarnson plans to apply for permits Through the Express Permit Application process from the
Division of Water Quality. The project is high density and the runoff from the proposed impervious areas

STORMWATER AND EROSION CONTROL NARRATIVE
The Fore Christ Ministries site is located in Brunswick County on NC 179 (Old Georgetown Road)
approximately 3.4 miles south of the intersection of NC 179 and NC 904. Fore -Christ, Inc.
proposes land clearirig and grtrbb4 activities only in preparation for a future commercial building.
The total project area is 6.11 acres and the total disturbed area is calculated to be 6.0 acres.
Future stormwater runoff v',M be routed using c.atch basins and underground pipes to a high -density
Stormwater Basin located where the temporary sediment basin is proposed -
Temporary diversions shall be installed around the perimete'r :of 'ihe site and will be routed to a
temporary sediment basin, which will be used to prevent -any significant amounts of sediment from
leaving the site. These measures shall be inspected regularly, and the accumulated sediment
removed to prevent the conveyance of sediment to any off -site areas.
The primary permanent erosion Control measures that will remain after the temporary measures are
removed will be permanent grassing.
