It is my understanding that you are considering purchasing this property for the construction of a church. If you purchase the site we could reissue the stormwater permit to you if you wanted it. Depending on the size of the church, a wet detention pond may not be needed for stormwater treatment. If the total amount of built -upon area is 24% or less of the total site we would consider the project to be low density and a wet detention pond would not be required by us. This permit is effective only with respect to the nature and volume of stormwater described in the application and other supporting data. 2. This stormwater system has been approved for the management of stormwater runoff as described on page 2 of this permit, the Project Data Sheet. 3. A maximum of 68,453 square feet of built upon area is allowed on this 3.0 acre site. 4. All stormwater collection and treatment systems must be located in either dedicated common areas or recorded easements. The final plats for the project will be recorded showing all such required easements, in accordance with approved plans. 5. Approved plans and specifications for this project are incorporated by reference and are enforceable parts of the permit. Redesign or addition to the approved amount of built -upon area e. Further subdivision of the project area. In addition, the Director may determine that other revisions to the project should require a modification to the permit. 6. The Director may notify the permittee when the permitted site does not meet one or more of the minimum requirements of the permit. Within the time frame specified in the notice, the permittee shall submit a written time schedule to the Director for modifying the site to meet minimum requirements. The permittee shall provide copies of revised plans and certification in writing to the Director that the changes have been made. II. SCHEDULE OF COMPLIANCE The permittee will comply with the following schedule for construction and maintenance of the stormwater management system. The stormwater management system shall be constructed in its entirety, vegetated and operational for its intended use prior to the construction of any built -upon surfaces except roads. b. During construction, erosion shall be kept to a minimum and any eroded areas of the system will be repaired immediately. The facilities must be properly maintained and operated at all times. The approved Operation and Maintenance Plan must be followed in its entirety and maintenance must occur at the scheduled intervals. 3. The permittee shall at all times provide the operation and maintenance necessary to assure the permitted stormwater system functions at optimum efficiency including, but not limited to: a. Semiannual scheduled inspections (every 6 months) b. Sediment removal C. Mowing and revegetation of side slopes d. Immediate repair of eroded areas e. Maintenance of side slopes in accordance with approved plans and specifications f. Debris removal and unclogging of outlet structure, orifice device and catch basins and piping. Records of maintenance activities must be kept and made available upon request to authorized personnel of DWQ. The records will indicate the date, activity, name of person performing the work and what actions were taken. This permit shall become voidable unless the facilities are constructed in accordance with the conditions of this permit, the approved plans and specifications, and other supporting data. 6. Upon completion of construction and prior to operation of this permitted facility, a certification must be received from an appropriate designer for the system installed certifying that the permitted facility has been installed in accordance with this permit, the approved plans and specifications, and other supporting documentation. A copy of the approved plans and specifications shall be maintained on file by the.Permittee for a minimum of five years from the date of the completion of construction. III. GENERAL CONDITIONS This permit is not transferable. In the event there is a desire for the facilities to change ownership, or there is a name change of the Permittee, a formal permit request must be submitted to the Division of Water Quality accompanied by an application fee, documentation from the parties involved, and other supporting materials as may be appropriate. The approval of this request will be considered on its merits and may or may not be approved. 2. Failure to abide by the conditions and limitations contained in this permit may subject the Permittee to enforcement action by the Division of Water Quality, in accordance with North Carolina General Statute 143- 215.6(a) to 143-215.6(c). 3. The issuance of this permit does not preclude the Permittee from complying with any and all statutes, rules, regulations, or ordinances which may be imposed by other government agencies (local, state, and federal) which have jurisdiction. 4. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions, the Permittee shall take immediate corrective action, including those as may be required by this Division, such as the construction of additional or replacement stormwater management systems. 5. The permit may be modified, revoked and reissued or terminated for cause. The filing of a request for a permit modification, revocation and reissuance or termination does not stay any permit condition. 6. The Permittee, at least six (6) months prior to the expiration of this permit, shall submit in writing a request for an extension along with appropriate application fee. Thorpe: DEC 212006 fo, " The following additional information is being provided per your letter dated December 18, 2006: The wet detention basin has been modified to include a 6:1 submerged vegetated shelf that is 10 feet wide. It is labeled `Aquatic Bench' on the section and plan views. //2. Based on the Division of Water Quality adopted policy for fountain design a 30,000 c.f. pond could only have a maximum 118 hp pump. I could not find a pump that small that would provide the desired aesthetics. The proposed pond fountain has been deleted. �C 3. The cross section has been revised to show the aquatic bench. The forebay earth baffle has been elevated to within one foot of the storm storage level. The plan view of the forebay earth baffle has been modified to show the correct contours as well. 4. a. The 24" RCP outlet is designed with a backflow preventer. The finished elevation of the existing ditch will be 8.0'. b. When U.S. Highway 158 was widened and curb and gutter added the NCDOT installed a drainage outfall for the highway down the south side of this property to Dowdy Bay. Over time the ditch has silted in and become overgrown. Currituck County Soil Conservation Services has requested that the applicant restore the ditch and provide the County with a 15' access and maintenance easement. Proposed contours with 3:I slope have been added to the plans showing the proposed grading and stabilization of the ditch. The proposed ditch bottom will be 8-.0' and -top -elevation will be 10.0'. c. The drawdown structure moves Q = 0.15 cfs and draws the storm volume down over 3 days. Using Q=Av the mean velocity added to the ditch is 0.0125 ftls. The maximum permissible velocity for the sandy loam non colloidal soils is 2.5 ftls. A review of these revisions to your project indicates that before a State Stormwater permit can be issued the following additional information is needed: 1. The wet detention basin should have a 6:1 submerged vegetated shelf at least 10 feet wide. This shelf should be located below the permanent pool elevation. The shelf as shown on the plan is located above the permanent pool elevation. 2. The plans contain a note that a fountain will be used. In order to assure that the treatment efficiency of wet detention ponds is not adversely affected by fountains, the Division of Water Quality has adopted a policy for fountain design. Enclosed is a copy of this policy with design criteria. Please provide information that shows that the proposed fountain will comply with the design criteria contained in the policy. 3. The cross section of the wet detention basin on page 2 of the plans does not match the plan view on page`1-of_the plans in regard to the elevation of the underwater berm between the forebay and the main pond. Also this cross section should show the 6:1 submerged vegetated shelf. 4. The outlet elevation'6f the pond is.set at 8.0 feet. It appears to discharge to an area that is elevation 9.0 feet. This difference in elevation may result in backing water up into the pond and therefore affect the treatment efficiency of the pond. It appears that the pond outfall and emergency spillway discharges to an adjacent property, rather than to a well-defined drainage feature. Please address. Also please provide calculations to show that there is non -erosive velocity. The above requested information must be received in this office prior to January 18, 2007 or your applications will be returned as incomplete. The return of these projects will necessitate resubmittal of all required items including the application fees. McAllister, P.E., 1.0 SOIL SCIENTIST REPOKY KINGS MILL AT GRANDY, NC 1.1 Site Description RREGE ED OCT - 3 2006 DWQ-WARD The above referenced site is located on the East side of US Highway 158 across the roars from the Chamber of Commerce Building in Grandy, NC. The proposed stonn water retention pond and wastewater disposal area for the proposed Kings Mill Office Park is located on the west side of the existing house on this site. The proposed replacement disposal area is located beside the proposed primary held. Both proposed disposal fields are approximately 65 feet wide by 90 feet long each. Low pressure pipe trenches are proposed to be used to distribute the treated effluent across the field. 1.2 Soil Assessment The soils in this area have been mapped by the C.urrituck County MRCS as the Dragston soil series, Coarse -loamy, mixed, scmiactivc, thennic Aerie Endoagutilts. This site is in the Coastal Plain province and the landform fbr the proposed disposal field is a flat interstream divide. The parent material that the soil formed from is marine and fluvial sediments. Five soil profile descriptions were taken in the proposed primary and replacement field areas and four in the area of the proposed stormwater retention pond. See Figure 1 for Site Map and for location of soil profile descriptions. Stormwater Retention Pond Area The soil in this area consists of a dark colored fine sandy loam topsoil laver approximately 12 inches thick. The underlying subsoil consists of a fine sandy loam to loamy sand layer more than 24 inches thick. The soil wetness condition in this area according to reduction oxidation indicators in the soil could not be determined due to the heavy organic matter content of the soil and was assumed to be less than 12 inches below existing grade. Actual water table observations in this area ranged from 13 to 18 inches below existing grade. Replacement Field Area The soil in this area consists of a dark colored fine sandy loam topsoil layer approximately 12 inches thick_ The underlying subsoil consists of a fine sandy loam to loamy sand layer more than 24 inches thick. The soil wetness condition in tills area according to reduction oxidation indicators in the soil could not be determined due to the heavy organic matter content of the soil and was assumed to be less than 12 inches below existing grade. Actual water table observations in this area ranged from 12.5 to 18 inches below existing grade. 1.3 Estimated Permeability Rates measure the hydraulic conductivity of the soil. In order to measure the hydraulic conductivity of the saturated soil material, shallow 2-inch PVC wells will have to be installed and slug tests will have to be conducted. This will be done in the area of the proposed septic field in order to run a groundwater mounding analysis. From observation of soil type and published data for the Dragston soil series, S&ME estimates the pernneability of this soil to range from 2.0 to 6.0 inches per hour. 1.3 Soil Loading Rates S&ME, Inc. recommends installing the subsurface trench bottom in sand fill material at existing grade or higher. The recommended soil ioading rate for both the primary field and the secondary field should range between 0.6 and 0.8 gallons per day per square foot. The trench bottom Page 2 of 3. Soil Scientists Report Kings Mill at Grandy S&ME No. 1054-06-431 September 28, 2006 placement in sand fill at grade or higher will provide: at least a 12 inch vertical separation for pressure dosed effluent to the soil wetness condition. A hydraulic analysis that includes a mounding analysis will need to be conducted to support this proposed vertical separation distance. A groundwater mounding analysis may show that a water table mound develops under the wastewater disposal field. In this event, additional sand fill may be required to maintain a 12-inch vertical separation distance from the proposed trench bottom. 1.4 Surface Water Management S&ME, Inc. recommends establishing a final grade over the active disposal fields that will shed rainfall away frorn the field area and will not trap rainwater in depressions over the field. To establish healthy vegetative cover over the disposal areas proper soil liming and fertilization recommendations shall be followed for the selected turf grass cover. 1.5 Seal and Signature John R. Detailed narrative description of all stormwater treatmentlmanagement 3. Soils report to include elevation of seasonal high water table -4. The length to width ratio of the wet detention basin should be 3:1 or greater 5. The wet detention basin should have a 6:1 vegetated shelf at least 10 feet wide 6. Please consider using a drawdown orifice structure similar to the enclosed drawing. We find these to not clog frequently and to be easily maintained.
