Arlington Blvd. Greenville,NC 27858 Dear Sir/Madam: This is to advise you that the Pitt County Technical Review Committee conditionally approved the soil erosion and sedimentation control plan and stormwater plan for LAUREL OAKS PHASE 3,4,5 on June 26, 2019. Conditions and requirements as a part of this approval are attached below. Copies of the draft minutes from the meeting will be available upon request prior to the next Technical Review Committee meeting. As stipulated on the financial responsibility form, you are responsible for ensuring compliance with all aspects of the approved plan and the Pitt County Soil Erosion and Sedimentation Control Ordinance. Your primary objectives include, but are not limited to, preventing offsite sedimentation, preventing sediment from entering watercourses, and establishing vegetation on all disturbed areas within 7-14 calendar days, following any completion of a phase of grading. As of April 1, 2019, all new construction activities are required to complete and submit an electronic Notice of Intent (NOI) form requesting a Certificate of Coverage (COC) under the NCG010000 Construction Stormwater General Permit. This form MUST be submitted prior to the commencement of any land disturbing activity on the above named project. The NOI form may be accessed at deq.nc.gov/NCG01. Please direct questions about the NOI form to Annette Lucas at Annette.lucas d,ncdenr.gov or Paul Clark at Paul.clarkrancdenr.gov. After you submit a complete and correct NOI Form, a COC will be emailed to you within three business days. Please be aware that your project will be covered by requirements of the NPDES Construction Stormwater General Permit NCGO10000. Please become familiar with all the requirements and conditions of this permit in order to achieve compliance. Upon discovery of noncompliance by our office, the following actions may be taken: 1. No further permits for building or other construction will be issued. 2. The authority to continue construction, other than for erosion control measures, will be terminated. 3. Penalties of up to $5000 per day may begin to accrue. Failure to immediately comply upon notice of noncompliance will result in injunctive and/or criminal action. Other Comments: 4. This erosion control plan shall expire three years following the date of approval if no land disturbing activity has been undertaken. 5. The Division of Water Quality may pursue legal action against you in order to enforce requirements of the NPDES, Phase II permit. 6. The Pitt County Soil Erosion and Sedimentation Control Ordinance authorizes County officials to inspect sites to ensure compliance with the Sedimentation Pollution Control Act. Title 15A NCAC 4B .0118(a) requires that a copy of the approved erosion control plan be on file at the job site. Also, this letter gives the notice required by G.S. 113A-61.1(a) of our right of periodic inspection to insure compliance with the approved plan. North Carolina's Sedimentation Pollution Control Act is performance-oriented, requiring protection of existing natural resources and adjoining properties. If, following the commencement of this project, the erosion and sedimentation control plan is inadequate to meet the requirements of the Sedimentation Pollution Control Act of 1973 (North Carolina General Statute 113A-51 through 66), this office may require revisions to the plan and implementation of the revisions to insure compliance with the Act. Acceptance and approval of this plan is conditioned upon your compliance with Federal and State water quality laws, regulations, and rules. In addition, local city or county ordinances or rules may also apply to this land-disturbing activity. This approval does not supersede any other permit or approval. Please note that this approval is based in part on the accuracy of the information provided in the Financial Responsibility Form, which you provided. You are requested to file an amended form if there is any change in the information included on the form. Land disturbance shall not begin until a preconstruction meeting has taken place. Show any proposed stockpile locations. 4. Any existing subsurface drain tile shall be removed or have a drainage easement placed over it. 5. Consider placing silt fence in locations that it can be utilized during home construction as well. 6. Obtain NCGO1 permit/certificate of coverage from NCDEQ prior to land disturbance. 7. Maintain soil erosion self-inspection records on site in accordance with Section 20 M. of the Pitt County Soil Erosion and Sedimentation Control Ordinance and NCGO1 permit. 8. Provide one hard and one digital copy of the revised set of plans based on conditions approved by the TRC,prior to initiating land disturbance. 9. Provide documentation exempting the stream feature that connects the existing pond in phase 4. [Engineering] 10. Add drainage easement over the swale proposed at the rear of lots 33-35 and lot 68. 11. Remove temporary turnarounds on Landon Dr. and Lyle Dr. once construction is complete. 12. Move silt fence back to limits of disturbance in Lawson Ct. 13. Install silt fence along the limits of disturbance at lots 68-80. [GUC Electric] 14. Plan is approved but the developer/engineer should provide any easements for GUC Gas that may be recorded on the plat. [Pitt County Environmental Health] 15. There are no areas for stockpiling shown in the limits of disturbance areas. 16. During construction of the roads and stockpiling of soils, heavy equipment needs to avoid the proposed septic system and repair areas to avoid compaction of the soil. Compacted soil is considered unsuitable for septic systems. 17. The house envelope, driveway envelope, water tap, and water line locations must be shown to ensure adequate room for septic system and repair areas on each lot. 18. Septic evaluations will have to be performed on each lot before permits can be issued and at this time no permits have been applied for in our office. Address water tap conditions that have been outlined in previous discussions. [General Comments] • Soils located on site include NrB2, ExA, ExB, OcB, Bb, WaB, WaC,Ly and possess the following characteristics: ■ ExA, ExB, OcB, Ly soil types have seasonal high water table. ■ WaB and WaC soil types are susceptible to soil blowing and have a high moderately rapid permeability. ■ ExA and ExB have a moderate shrink swell potential. ■ OcB soil type has a rapid infiltration characteristic. ■ Soils are prime farmland or have statewide importance for farming. Development will remove the site from agricultural production. ■ Sub surface drain tile could still possibly be on site. ■ Juniper Branch Watershed
