Completion of decanting occurred at Roxboro on February 11, 2020. As required by the SOC,I certify under penalty of law that this document and all attachments were prepared under my direction or supervision in accordance with a system designed to assure that qualified personnel properly gather and evaluate the information submitted. Inspection activities were performed in accordance with North Carolina Environmental Management Commission (EMC) Special Order by Consent(SOC) WQ #S18-005. The Special Order by Consent describes inspection requirements as follows: No later than 90 days following the completion of decanting at each Facility, and in the same manner as in the annual surveys, Duke Energy shall conduct a comprehensive survey of areas down gradient of ash basins identifying new seeps, and documenting the physical characteristics of previously documented seeps. All examinations of seeps must include identification of seeps by approximate latitude and longitude and date-stamped digital photographs of their appearance. A report summarizing the findings of this survey, including a section analyzing the effect decanting of the basins(s) has had on seep flows, accompanied by copies of the photographs noted above, shall be submitted to the Director of DWR ("Final Seep Report"). This Final Seep Report must list any seep that has been dispositioned (as described in subparagraph (3) above)during decanting process, including an analysis of the manner of disposition. The determination of whether a seep is dispositioned rests with the Director of DWR. At, or at any time prior to, submission of the Final Seep Report, Duke Energy shall seek formal certification from the Director of DWR, certifying the disposition of any seep that Duke Energy has characterized as dispositioned. Any seeps not certified as dispositioned by the Director of DWR shall not be deemed as dispositioned. All locations listed in Table 1 of the SOC were observed and documented with photographs during the inspection. General inspection observations include: • No previously unidentified seeps were observed during the inspection. • No notable changes to seep status were observed during this inspection. • Passive decanting (removal) of free water from the West Ash Basin (WAB) was initiated on April 10, 2019 with the redirection of wastewater inflows from the WAB to the lined retention basin treatment system. Page 1-1 2020 Final Seep Report May 2020 Roxboro Steam Electric Plant-Duke Energy Progress,LLC SynTerra 1.1 Inspection Details The weather was clear before and during the inspection. Temperatures ranged from approximately 51 to 60 degrees Fahrenheit (F). No precipitation was recorded on the day of the inspection or three days before the inspection, according to the on-Site weather station. Five days prior to the inspection, on March 13, 2020, a 0.07-inch rain event was documented. The inspector reviewed previous seep inspection reports and the weekly dam safety inspection reports for each ash basin.Jake Muessen (Duke Energy System Owner) conducted the ash basin dam safety inspections and prepared the reports. Before the inspections, Mr.Jake Muessen and Mr. Eric Cleaver (Roxboro CCP Implementation Manager) were interviewed about recent rainfall, construction/stabilization projects associated with the ash basins, and general Site conditions in and around the ash basins. Observations from the dam safety inspection reports were consistent with observations made during the post-decanting inspection. The inspection included observations in the following areas (Figure 1): • WAB main dam and chimney drains • Areas northeast of the WAB main dam • Southern half of the WAB sluice lines access area • The effluent discharge canal from the WAB extension impoundment to the heated water discharge pond • The East Ash Basin (EAB) main dam • Areas proximate to the EAB main dam to the southwest and northeast; eastern side of the EAB/industrial landfill • Effluent discharge canal from the EAB extension impoundment area to the east side of the gypsum storage area • Area between the south side of the industrial landfill and unnamed tributary • Southwestern and eastern sides of the industrial landfill (south of the EAB extension impoundment area) • Areas identified as downgradient of ash basins Seep locations and inspection areas are shown on Figure 1. Observations noted during the inspection are summarized on Table 1. Photographs taken of each seep location during the inspection are included in Appendix A. Page 1-2 2020 Final Seep Report May 2020 Roxboro Steam Electric Plant-Duke Energy Progress,LLC SynTerra 2.0 FINDINGS 2.1 General Observations Inspection observations of each seep are described in Table 1. A summary of the details includes: • No new seeps were observed during the inspection. • No previously unidentified or unreported pipes were observed during inspection. • Flow conditions at seeps were normal and generally consistent with previous inspections. • Water levels in the heated water discharge pond, the unit 3 cooling tower pond, and the cooling water intake canal appeared consistent with normal levels. • Water levels in the WAB/EAB effluent discharge canals and the basin impoundments were noted at slightly lower levels. 2.2 Effects of Decanting Passive decanting of free water from the WAB began on April 10, 2019 through the cessation of wastewater inflows to the WAB. By April 10, 2019, all wastewater inflows to the WAB were ceased with the redirection of Plant wastewater to the lined retention basin treatment system. No decanting is expected or anticipated for the EAB. Duke Energy provided a Notification of Decanting Completion letter to the North Carolina Department of Environmental Quality (NCDEQ) Division of Water Resources (DWR) on February 11, 2020. The notification indicated water levels within the WAB have equalized across the north and south sides of the internal filter dike (Figure 1) at approximately 447 feet elevation. Based on elevation changes of approximately 13 feet across the filter dike, the WAB has been lowered by 90 million gallons of wastewater through the gravity drainage process. Due to the design of the ash basin, this amount of water is the most that can be removed without water allowed to flow back into the ash basin through the filter dike. No mechanical removal of water (active decanting) will occur as indicated in the SOC and prior to National Pollutant Discharge Elimination System (NPDES) permit receipt. The annual seep inspection occurred approximately a year after wastewater inflow to the WAB had ceased. There were no noticeable effects to seep flow related to passive decanting from the WAB during the inspection. Page 2-1 • 2020 Final Seep Report May 2020 Roxboro Steam Electric Plant-Duke Energy Progress,LLC SynTerra 2.3 Changes to Seep Status Mr. Eady inspected each seep to evaluate whether it met requirements to be dispositioned in accordance with the SOC. The requirements for dispositioning a seep are as follows: For purposes of this Special Order, "dispositioned" includes the following: (1) the seep is dry for at least three consecutive quarters; (2) the seep does not constitute, and does not flow to, waters of the State or Waters of the United States for three consecutive quarters; (3) the seep is no longer impacted by flow from any coal ash basin as determined by the Director of DWR in accord with applicable law and best professional judgment; or(4) the seep has been otherwise eliminated (e.g., through an engineering solution). If a seep that has been dispositioned through drying up reappears in any subsequent survey, such a seep will no longer be deemed dispositioned and can be subsequently re-dispositioned as specified above. Non-constructed seeps described in paragraph 1(h) of this SOC cannot be dispositioned through option (2) above. No changes to seep status were observed during the inspection.
