Sullins Dee Freeman Governor Director Secretary December 14, 2010 Mr. David Scott Carolina Commonwealth Forest Products, LLC PO Box 785 Henderson, NC 27536 Subject: Stormwater Compliance Evaluation Inspection Certificate of Coverage No. NCG210226 Carolina Commonwealth Forest Products, LLC - Henderson Vance County Dear Mr Scott: On December 7, 2010, I, Cheng Zhang of the Raleigh Regional Office (RRO) conducted a Stormwater Compliance Evaluation Inspection at Carolina Commonwealth Forest Products, LLC. You assistance was greatly appreciated. The inspection report is attached. Findings during the inspection were as follows: Certificate of Coverage (COC) No. NCG210226 of NPDES general permit NCG210000 was issued to Carolina Commonwealth Forest Products, LLC, to discharge stormwater runoff to Joes Branch and Martin Creek, a class C NSW stream, in the Tar -Pamlico River Basin. 2. The facility has not developed a Stormwater Pollution Prevention Plan (SPPP) since ownership changed in October 2001. The previous owner, Louisiana Pacific Corp.'s SPPP was on file but has not been updated and implemented by the current owner. 3. It was noted that the facility has not been conducting any timber product manufacturing since the change of ownership. The saw mill and planer from the previous owner were demolished. No piles of saw dust, wood chips, bark, mulch, or other similar material were observed on site. Vehicle maintenance was conducted in the shop; monthly usage of new motor oil was not well documented. No monitoring (qualitative or analytical) has been conducted. 4. Based on the above observation, I recommend the COC be rescinded. Please send written COC rescission request to: NC Division of Water Quality— Stormwater Pennitting Unit 1617 Mail Service Center, Raleigh, NC 27699-1617 North Carolina Division of Waler Quality 1628 .Mail Service Center Raleigh, NC 27699-1628 Phone (919) 7914200 Customer Service Internet: www.nnvaterquality.org Location: 3800 Barrett Drive Raleigh. If you elect to maintain the current COC, you trust develop and implement a SPPP, conduct all required monitoring: semi-annual qualitative monitoring at all stormwater discharge outfalls (SDOs) and possible semi-annual analytical monitoring at the SDO located on the planer site should monthly usage of new motor oil be more than 55 gallons.
