Enclosed is the completed renewal application for Stormwater NPDES Permit Number NCS000101.
This final submission replaces the previous application submittal dated January 30, 2012. This
application is updated with results of the last sampling period of the 5-year permit, along with other
pertinent revisions. Please record the changes indicated on the Renewal Application Form. Requested
supplemental information is provided, including the Stormwater Pollution Prevention Plan
Development and Implementation Certification. Please also note the enclosed Representative Outfall
Status(ROS)Request Form, which was also submitted previously,on January 30,2012.
In Section B of the current permit,Analytical Monitoring Requirements,Footnote 3, representative
sampling locations are listed, including all mill basins except Basin J. Per conversation with Aisha Lau,
formerly of DWQ, Basin I was supposed to be listed as representative of J, but Basin J was
inadvertently omitted from the list. Please list Basin I as the representative sampling point for Basin J.
This request is also listed on the attached Representative Outfall Status Request Form.
Also in Section B of the current permit is a Monitoring Schedule. The left-hand column, Monitoring
Period, begins with"Year 1 —Spring". However,the permit was issued in August of 2007,which made
the first monitoring period Fall. This agrees with the Start and End columns in the same Schedule.
Thus,the final Monitoring Period was Spring, 2012 and not Fall as shown in the first column. Data for
that sampling period is included in this submittal,completing this application.

NPDES STORMWATER PERMIT
Two copies of each of the following shall accompany this submittal in order for the application
to be considered complete:
(Do not submit the site Stormwater Pollution Prevention Plan)
Initials
'W 1. A current Site Map from the Stormwater Pollution Prevention Plan. The location of
industrial activities (including storage of materials, disposal areas, process areas and
loading and unloading areas), drainage structures, drainage areas for each outfall,
building locations and impervious surfaces should be clearly noted.
2. A summary of Analytical Monitoring results during the term of the existing permit
(if your permit required analytical sampling). Do not submit individual lab reports.
The summary can consist of a table including such items as outfall number,

Stormwater Best Management Practices Summary 2012

At the International Paper Mill, approximately 90% of runoff from
the main mill facility drains to the mill wastewater treatment system
(WWTS), and is treated under the mill's NPDES wastewater permit.
Runoff from the remaining 10% of the mill and from a few outlying
areas is covered in the mill Storm Water Pollution Prevention Plan
(SWPPP). The SWPPP identifies 19 stormwater basins, where
numerous Best Management Practices (BMPs) are used to control or
prevent erosion and sediment runoff.
Non-structural BMPs include the Stormwater Pollution Prevention
Plan; the Spill Prevention, Control, and Countermeasures Plan; the
Hazardous Waste Handling Program; The Facility Response Plan; and
the Waterfront Dock Transfers manual. All stormwater basins
include signs marking the boundaries of stormwater runoff and the
locations of outfalls. Activities such as chemical storage, painting,
and vehicle maintenance are forbidden in stormwater basins, and
this information is also posted. Basic good housekeeping is
enforced, and numerous mill employees participate in monthly basin
inspections. Annual training is required for all employees working in
stormwater basins. Street and storm drain maintenance is a focus in
stormwater basins. Grasses and other vegetation are maintained
wherever feasible.
Structural BMPs and other control measures are used to reduce
erosion and sedimentation while promoting groundwater recharge.
Grassy swales and meandering natural streams carry flow, and
geotextile cloth with gravel or riprap covers bare areas. Jersey
dividers protect unpaved road edges. Earthen collection ponds and
basins allow groundwater infiltration. Rock filters surround several
outfalls, and a small French drain system directs runoff to one
outfall. Riprapped swales, a retaining berm, rock checkdams, and
drainage channels direct flow.
In areas of high traffic and high runoff flow, hardened structural
BMPs direct flow, prevent erosion and pollutants from entering
runoff, and provide flood control. Recent roadway repairs in
stormwater basins focus on reducing erosion from roadways.
Concrete and paved surfaces and roads, as well as plastic ditch
liners, harden surfaces. Concrete containments, sumps, curbing, and
two oil/water separators (OWS) protect against runoff
contamination. Runoff is conveyed via pipes and concrete structures
such as catch basins, culverts, U-drains, swales, and outfall
structures. A concrete flow diverter and a concrete flow diffuser
further control flow. The OWS at #2 wood chip truck dump (Basin E)
has been re-engineered and concreted to improve runoff quality.
Planned BMPs include ongoing road rebuilding in stormwater
basins. Adjacent drainage features will be developed to convey and
control increased runoff volume and velocity from the newly paved
areas. The new drainage features will tie into newly completed,
existing drainage systems.
Significant Changes In Industrial Activities

International Paper's Riegelwood Mill has made several changes in
industrial activity that reduce the potential for stormwater pollution.
An out-of-service, ground-level pipeline was removed from Basins G
and H. Previously this pipeline had prevented effective control of
erosion and stormwater flow, resulting in 15 separate discharge
points. Now the area is shaped into one long riprap swale with
improved runoff quality and only 4 outfalls.
A water treatment chemical feed system was removed from Basin F
to an area that drains to the mill wastewater treatment system,
eliminating the chance for a chemical spill to stormwater.
New mill procedures have reduced material storage in stormwater
basins. Maintenance material storage is no longer permitted in
Basin F, and all miscellaneous materials have been removed from
the area. In Basin G, storage is now limited to currently usable
maintenance material, mainly steel and concrete, eliminating most
material storage.
Mill paving projects have reduced erosion potential in several
stormwater basins. Mill roads in Basins C, D, E, and F have all been
re-paved, and adjacent drainage systems have been added,
enhancing runoff quality. Runoff is now channeled through a gravel
filter in Basin C, and Basins D and F have new concrete swales to
direct flow. In Basin E, the collection basin ahead of the oil-water
separator was re-engineered and concreted to improve stormwater
quality. Increased flow and velocity from the new paving are
controlled by a long, corrugated ditch liner. Hillsides have been
grassed. Basin F also utilizes a concrete drainage channel and a new
concrete U-drain.
