Knopf: 1. Ron Boone of the Winston-Salem Regional Office of the NC Division of Water Quality (DWQ) conducted a compliance evaluation inspection (CEI) at the subject facility on December 8, 2010. Your assistance and cooperation during the inspection was greatly appreciated. An inspection checklist is attached for your records and inspection findings are summarized below. 2. The facility is located at 2394 West Clemmonsville Road, in Winston-Salem, Forsyth County, North Carolina. There is currently no activity at the site, mining or otherwise, and consequently there are no stormwater or wastewater discharges related to mining activity. Because of this, there is no requirement for qualitative or analytical monitoring. 3. Due to the inactivity of the site and Vulcan's desire to maintain the permit in an active status, DWQ hereby suspends all monitoring/reporting requirements of the permit until such time that Vulcan resumes mining activities at the site. Vulcan must notify this office at least 90 days prior to resuming any mining activity at the site to ensure that the SP3 will meet the requirements at the site at the time activities resume and to ensure monitoring is resumed immediately when any discharge of stormwater or wastewater related to mining activity occurs. 4. You are reminded that violations of the NCG020000 general permit are subject to civil penalty assessments not to exceed $25,000 per clay, per violation. Thank you for your cooperation in this matter.
