Comment: SDO onsite at bottom of pavement was evaluated. This SDO has an active stream in it. See site history. There was some evidence of particulate matter at the stormdrain grate. Concern is that some of this particulate matter is coming from drying beds. Spoke with maintenance manager Tim Brown and told him some better housekeeping would be necessary at drying beds to keep incidental particles from reaching stormwater system. Also told him to clean old stain where wastewater had previously gotten to stormwater system so we and he could make sure no more is reaching system. Also told him to make sure next time he uses the dump truck to that the installed berm is high enough to be effective, if not then increase height. I met with Tim Brown and Andrew Mayberry. During the inspection, the following items were noted: 1) While the monitoring results were present, the majority of the Stormwater Pollution Prevention Plan (SWPPP) could not be located. The ficility is directed to locate and present a complete SWPPP for the next inspection, scheduled for March 22nd, 2018. 2) Industrial waste was observed outside of the settling basin nearest to the stormwater inlet and monitoring point. Good housekeeping practices should immediately be improved in this area. This is a high risk area, due to its proximity to the an on site stormdrain. As you mention in your letter, persons involved during this site inspection on April 29, 2009, were David R. Mursch, P.E., yourself and me. I would like to thank you for taking your time providing me, Dave and Lenoir Mirror, with a thorough overview of the stormwater program, permit requirements and recommendations we can take to achieve compliance with our stormwater permit requirements. Analytical Stormwater and Non-Stormwater Sampling __ ------ I—. _ Dave, was able to get the stormwater samples during the ain event May- 5,.2000He was also able to collect and analyze the slow running water at the bottom of the outfall location SDO#Il as you had requested. I have included all lab results with this letter. Wastewater/ Stormwater Discharge The transfer of solid wastewater sludge from the treatment pit to the dump truck for transfer to the local landfill was:addressed. Lenoir Mirror's maintenance department has implemented -a safer and better. handling -procedure to work with wastewater sludge during handling. The single most likely time sludge may spill onto the pavement is while digging=the sludge out -of a-full-pit-and-putting:it:into:an:adjacent-.dump=truck for transporting to the landfill. This,is:done-only=a-few,times:peryear. Maintenance plan to control wastewater release Maintenance will put down onto the parking{lot"a:berm,in,the-proper_location"and=arrangement so that if any residual releases do occur, while digging out the pit and transferring it to the dump truck, the wasterwater will ,rumback,into�the:treatment-pits. Other housekeeping equipment such as water hose, shovels and brooms will be utilized if needed. Emphasis will be placed on havingono:releases:at all. Please review the draft very carefully to ensure thorough understanding of the conditions and requirements it contains.. The draft permit contains the following significant changes from this facility's current permit:, - Analytical monitoringshancies: 1. Analytical monitoring parameters from the previous permit have been maintained in this permit. 2. ph has been added to the analytical monitoring requirements. 3. All analytical monitoring has been set to semi-annually during a representative storm event as defined in Part II Section B. The permittee must also document the total precipitation for each event. If no discharge occurs during the sampling period, the permittee must submit a monitoring report indicating "No Flow" within 30 days of the end of the six-month sampling period. Additionally, samples must be taken a minimum of 60 days apart, as specified in Table 2. 4. Benchmarks for analytical monitoring have been added to this draft permit. Exceedances of benchmark values require the permittee to increase monitoring, increase management actions, increase record keeping, and/or install Stormwater Best Management Practices (BMPs) in a tiered program. If the sampling results are above a benchmark value, or outside of the benchmark range, for any parameter at any outfall then the facility shall follow the Tier 1 guidelines which require a facility inspection within two weeks and implementation of a mitigation plan within two months. If during the term of this permit, the'sampling results are above the benchmark values, or outside of the benchmark range, for any specific parameter at a specific discharge outfall two times in a row (consecutive), then the, facility shall follow the Tier 2 guidelines which require a repetition of the steps listed for Tier 1 and also immediately institute monthly monitoring for all parameters at every outfall where a sampling result exceeded the benchmark value for two consecutive samples. 5. You are required to collect all of the analytical and qualitative monitoring samples during representative storm events as defined in Part II Section B. Qualitative monitoring is required regardless of representative outfall status. 6. You are responsible for all monitoring until the renewal permit is issued. See Footnote 1 of Tables 1, 4, and 5. 7. The inspection was conducted during a non- stormwater event. However, water was flowing through this manhole during the inspection. This water is likely the stream in the vicinity of.the old cutlet piles/landfills of which DWQ investigated in 1992. Please perform analytical monitoring of this stream for metals (specifically Ag and Ph) at SDO91 during base flow. Send results to the Asheville Regional Office. WasteWater/ Stortwater Discharge At the wastewater pretreatment system onsite there was evidence of residual wastewater entering the stormwater system. `['his appeared to be from the practice of transferring solids (sludge from the wastewater facility) to a dump truck for transport to the county landfill. Wastewater cannot enter the stormwater system; this situation needs to be remedied immediately. Please respond with a plan to eliminate such occurrences in the future. During the inspection a berm was mentioned to resolve this discharge to the stormwater system. if a berm is installed please include housekeeping measures that will be put into practice and included in the Stormwater Pollution Prevention Plan. These housekeeping items will need to address how residuals on pavement will be kept from reaching the stormwater system if the berm is overtopped during a storm event or if incidental spillage occurs.
