The inspection consisted of review of the Stormwater Pollution Prevention Plan (SWPPP) and each of its components, review of the facility outfalls, as well as the review of the overall site conditions. The compliance inspection was conducted to ensure all site practices and facilities are in working order. The inspection was also conducted as assistance to Commonwealth Brands should there be any questions about the permit NCG060042. Permit: This facility has General Stormwater Permit NCG240021 to discharge stormwater from industrial activity associated with Tobacco Products [SIC 211 under the National Pollutant Discharge Elimination System (NPDES). The permit was originally issued on May 14, 1.993 and this version of the permit was issued on November 1, 2018 and will expire on May 31, 2021. Records/Reports- This facility is required to develop and maintain a SWPPP in accordance with Part II, Section A of the permit. All qualitative and analytical monitoring records are required to be maintained with the SWPPP for a minimum of five (5) years. Under this permit, analytical monitoring is required under current operating procedures. The facility records are very well organized and have all records dating back for 5 years. The SWPPP for this facility had been put together prior to arriving on site. The SWPPP looked very good and contained all required components. The facility is required to maintain employee training records in accordance with Part 1I, Section A, Item 6 of the permit. All training records were available for review during the inspection and they had been recently updated. The facility is primarily a large grouping of warehouses where the majority of the industrial activity is performed inside. There are multiple buildings on site connected by bridges and walkways. The facility is slightly sloped with the majority of the facility draining to the western side. Multiple catch basins can be seen throughout different areas of the facility. Two outfalls were observed as curb inlets along North Scales Street, these outfalls were very well maintained and did not show any sign of being disturbed by the public. Two more outfalls were located at the southern edge of the facility at rip rap outfalls. These outfalls were maintained very well and showed no sign of bad housekeeping. The overall housekeeping of the facility was very good for a facility of this size. The majority of potential impacts are kept inside the facility with only small areas of transfer located externally. Effluent Receiving Waters: Effluent from this facility drains into an unnamed tributary of Wolf Island Creek, class "C" waters of the Roanoke River Basin. Self-Monitorin Program: This facility, is required to provide Qualitative and Analytical Monitoring twice annually. The outfalls are maintained and monitoring records were observed during the inspection. There are no comments about any with the issued permit. Stormwater related to industrial activity is discharged to the City of Reidsville's Municipal Separate Storm Sewer System (MS4), this section of which ultimately discharges to Wolf Island Creek, which is'currently classified as Class C waters in the Roanoke River Basin. This facility manufactures and packages cigarettes. Documentation and Monitoring Review The Stormwater Pollution. Prevention. Plan (SP3) is complete and current. It was last rewritten in May 2012 and will be due for another update in May 2013, as required in Part 11, Section A, Paragraph 8 of the permit. Your documentation of the reviews/updates is critical to demonstrating compliance with this requirement. Qualitative and analytical monitoring have been conducted and documented as required. The Commonwealth Brands facility has four Stormwater Discharge Outfalls (SDOs) at which qualitative and analytical monitoring are performed as required. All analytical monitoring results were within benchmark values and no problems were encountered. The previous inspection, performed in March 2009, revealed several discrepancies and issues with the SP3 and Q/A monitoring. Ms. Taylor -Smith was pleased to find that not only had those problems been addressed, but that Commonwealth Brands now exceeds expectations in terms of compliance. Additional conditions for specific industry sectors have been added to the SPPP requirements in some cases. • Sections B, C: Failure to perform analytical Stormwater monitoring may result in the Division requiring that the permittee begin a monthly sampling scheme. • Sections B, C: A lower TSS benchmark of 50 mg/I for HQW, ORW, PNA and Tr Waters applies to these more sensitive waters. • Sections 8, C: The monitoring parameter Oil & Grease (O&G) has been replaced by the parameter Total Petroleum Hydrocarbons (TPH) for vehicle maintenance areas, and in some cases, other analytical monitoring requirements. • Sections B, C, D: Inability to sample due to adverse weather must be recorded in the SPPP, or in separate on -site records if your General Permit does not require an SPPP. A measurable storm event is defined in the permit. • Section D: If the permittee fails to respond effectively to problems identified by qualitative monitoring, DWQ may require the permittee to perform corrective action. Item 4c- R & A Labs has been using plastic bags to collect representative samples of stormwater flowing to SDOs with grating that is impractical to remove due to weight. They feel that their mothed is sound but we are also exploring the possibility of using sampling pumps for the collection process. Item 4d1- Fecal coliform testing will be discontinued and removed from the SP3 plan as it is not required. We had a large collection of pigeon fecal material on a section of roof at the time of the high results. This area has since been cleaned. Item 4d2- This discrepancy has been noted and if future pH samples are out of range we will begin tier one actions. Current records indicate that monitoring, along with other permit required facility inspections and so forth, were just recently initiated in June 2008. Records of monitoring and inspections prior to this are sparse, despite the fact that the permit was issued in November 2007. You should review the entire general permit in detail, to ensure all required monitoring, inspections, etc., are accomplished as required. The SP3 appears to have addressed all permit requirements, but has not yet been fully implemented. b. The SP3 and site plan reflect that there are only four stormwater discharge outfalls (SDO), yet Q/A monitoring, which is currently being completed by an outside agency, showed results from five SDOs. Additionally, the sample point designations listed in the QIA monitoring reports do not match the designations of the SDOs in the SP3 and site plan. It is therefore essentially unknown where each sample was taken, except to the individual from the agency that actually took the samples.. You stated during the inspection that the agency came on site and performed the monitoring without your knowledge, therefore you -were unaware of the samples being taken, where they were taken from, how they were taken, and so forth. Commonwealth must better coordinate the, monitoring by the outside agency, or simply conduct the monitoring themselves, so as to'know where, when and how such samples are taken, and to ensure that samples are taken at the correct locations. c. Furthermore, the arrangement of two of the SDOs, 001 and 002, begs the question of how the agency'acquired representative samples. These SDOs are actually stormwater catch basins located on Scales Street on the west side of the facility. Their arrangement makes it difficult to ascertain how the agency was able to acquire representative samples from these SDOs without coordinating with the city for the removal of the grates from the catch basins. d. Lastly, the QIA monitoring that has been accomplished revealed several conditions that were not addressed, as discussed below: The facility performed Fecal Coliform monitoring in June 2008. Results of these tests came back well above the permit benchmark of 1,000, yet no actions were taken to determine the cause of the high Fecal results and/or no documentation of such actions exists. It is noted that the facility is not required to monitor this parameter because it does not process meat products. Subsequent Fecal results were below the benchmark and it is believed that the high results may have been the result of large amounts of bird excrement that have since been cleaned up. 2. The June 2008 sampling event also resulted in one sample above the COD benchmark and two samples outside the pH range. Commonwealth should have begun tier one actions in response to these samples, as required in Part 11, Section B, of the permit, but no such actions were taken and/or no documentation of such actions exists. Please review the aforementioned part of the permit for details on the tier 1 /2 actions that are required in response to samples above permit benchmarks. Site Review 5. The site was clean and well maintained. Exposed industrial activity at the facility is minimal. You may wish to consider a No Exposure Certification '(NEC) for this site.
