Drawer 2128 Dalton, GA 30721-2128 March 15, 2019 North Carolina Department Environmental Quality Attn: Isaiah Reed, Environmental Specialist 2090 U.S. Highway 70 Swannanoa, NC 28778 Subject: Compliance Evaluation Inspection Shaw Industries -Plant Z1 Permit No. NCG290393 Macon County, North Carolina Dear Mr. Reed: Shaw Industries; Plant Z1 in Franklin; NC would like to submit this letter in response to the findings for the February 1 P, 2019 Compliance Inspection. The results show the facility has addressed both items on the inspection report. The Inspections Summary identified two items that were in need of correction which are referenced below: 1. Exposed accumulations of sawdust/woodchips or other similarly sized material were observed on site during the inspection. The permittee is directed to immediately begin analytical monitoring at all outfalls, or remove all exposed accumulations of saw dust or other materials prior to the next inspection. 2. The facility owner that is listed with the Department is still "Zickgraf Hardwood flooring". The permittee is directed to immediately submit a change of ownership form to the department as soon as possible. Corrective Actions: Item 1: The sawdust/woodchips that were observed during the inspection have been removed and the surrounding areas cleaned. The unpaved areas at or near the sawdust/woodchips accumulations has been cleaned and backfilled with gravel. The included photos display each of the areas of interest and the improvements made. Received MAR 1 8 2019 Land Quality Section Asheville • Item 2: Shaw Industries, Plant Z1 is in the final stages of a transfer ownership of the facilities Iocated in Franklin, NC and Bryson City, NC. A change of ownership is anticipated to be completed the first week of April 2019. At that time a transfer of ownership will take place and appropriate documentation will be submitted to NCDEQ. They will go as far as they want if it is not. Acrolin and Arsenic are carcinogens...and harmful to the population of Macon County. Everyone in the area suffers from allergy's. I believe Zickraf emissions are contributing to this, and also the high cancer rates in this area. The Franklin Press and the Macon News dont even publish obituaries with cause of that state anything about cancer. There are no updated figures published about cancer rates in this county, that i know of . If you have any 2013-14 data please forward. Zickgraf is under a general stormwater permit (NPDES) which requires monitoring of their stormwater outfalls for such parameters as color, odor, clarity, Floating solids, suspended solids, foam, oil sheen, and erosion at the outfalls. They are required to monitor these semiannually during rainfall events. The facility's last monitoring report did not note any problems observed with these parameters. They are not required to sample for acrolein or arsenic at their outfalls as part of their general stormwater permit requirements. I am unaware of any water quality monitoring in this area for arsenic or acrolein, but will forward your inquiry to our Division of Water Resources for review. During the visit, the SPPP manual was reviewed, operational activities and existing stormwater exposures were discussed. At the time of the inspection the site was found to be in compliance with the permit. The inspector noted that the facility appears to maintain good housekeeping practices and records were found to be in good order. Representative Outfall Status was discussed, but does not apply because current facility operations do not require analytical monitoring. Zickgraf staff indicated that installation of new secondary containment is underway for the onsite diesel fuel tang. As required under -Part IIl.E.8 of the NPDES General Permit No. NCG210000, I am submitting a written report of the incident and corrective actions taken. [ncident Summary On January 61h, at approximately 9:45 AM, a forklift driver exiting the fueling area inadvertently caught the dispensing hose attached to the diesel fuel tank. Subsequently, the dispensing hose broke at the fueling meter located on the exterior of the secondary containment structure. As a result, approximately 5 gals. of diesel fuel was released to the surrounding area. Absorbents were used to contain and remediate the shill. Additionally, absorbent socks were place in the adjacent drainage culvert to capture any residual fuel that may have entered the culvert. As a precautionary measure, the National Response Center, the North Carolina Division of Emergency Management, and the NCDENR were contacted due to the proximity of the Little Tennessee River. Corrective/Preventative Action By design, the dispensing hose and fueling meter are currently attached to the exterior of the secondary containment structure of the diesel fuel tank. The dispensing hose and V fueling meter shall be relocated to the interior of the secondary containment structure and ballast shall be place in front of the secondary containment structure to prevent vehicles from accessing the area immediately adjacent to the diesel fuel tank and associated secondary containment structure. 1f you have any questions regarding this matter, please contact me at (828) 421-4234.
